Hypothese: Er wordt aangenomen dat CHF gedeeltelijk wordt gemedieerd door ontsteking. De primaire hypothesen zijn verankerd in de premisse dat de pathofysiologie van CHF deels wordt aangestuurd door ontsteking die voortkomt uit een lage diversiteit, dysbiotische darmmicrobiota via ten minste drie mechanismen, waaronder: i) verrijking van gram-negatieve lipopolysaccharide (LPS)-producenten; ii) verrijking van organismen met potentieel voor het produceren van uremische toxines, met name in de context van verminderde nierfunctie, wat vaak voorkomt bij CHF; iii) uitputting van producenten van korteketenvetzuren (SCFA's), waarvan bekend is dat ze belangrijke ontstekingsremmende moleculen zijn. De onderzoekers veronderstellen dat IMT-therapie de darmdiversiteit zal vergroten, ontsteking zal verminderen en de functionele capaciteit en biomarkers van hemodynamische stress zal verbeteren.
Fase van klinische studie: 1
Studieontwerp en deelnemers: Eéncentrum, open-label studie naar de veiligheid en haalbaarheid van IMT bij patiënten met CHF. Na toestemming worden personen van 18 jaar en ouder met de diagnose CHF, niet als gevolg van acute myocarditis of infiltratieve ziekte, ingeschreven om gedurende 7 dagen antibioticavoorbehandeling te ondergaan, gevolgd door dagelijkse IMT gedurende nog eens 7 dagen. Voorafgaand aan antibiotica en IMT worden ontvangers gescreend op inclusie-/exclusiecriteria, geïnterviewd over medische geschiedenis en medicatie, en geven zij toestemming. Daarnaast worden voorafgaand aan IMT en antibioticavoorbehandeling basismonsters van bloed en feces verzameld, en ondergaan patiënten een zesminuten wandeltest, echocardiogram en kwaliteit-van-leven vragenlijst (emPHasis-10, een gevalideerde korte vragenlijst om de gezondheidsgerelateerde kwaliteit van leven bij mensen met CHF te beoordelen). Een extra fecest monster wordt verzameld na 7 dagen antibioticavoorbehandeling en vóór de start van actieve IMT-therapie. Patiënten ondergaan follow-up via telefoon, video of persoonlijk bezoek, of online enquête over symptomen op dagen 1-21, en vervolgens maandelijks tot 6 maanden na IMT om te screenen op ernstige bijwerkingen (SAE's) en bijwerkingen (AE's). Screening op SAE's en AE's wordt gedaan met behulp van een symptomenvragenlijst en door patiënten tijdens ons interview te bevragen. Bloedmonsters en een fecest monster worden verzameld van deelnemers op één en zes maanden na IMT om circulerende ontstekingsmediatoren te beoordelen en veranderingen in het microbioom van de ontvanger te evalueren (engraftment kinetiek). Na zes maanden ondergaan patiënten, wanneer logistiek mogelijk en klinisch geïndiceerd, ook een herhaalde zesminuten wandeltest, echocardiogram en kwaliteit-van-leven vragenlijst.

Beschrijving
Inclusiecriteria:
- Diagnose van hartfalen met verminderde ejectiefractie (HFrEF) of hartfalen met behouden ejectiefractie (HFpEF), New York Heart Association Klasse II-IV
- Stabiele behandeling voor CHF gedurende één maand voorafgaand aan inschrijving
- In staat om capsules in te slikken
- In staat om bloedmonster en ontlastingsmonster te verstrekken
- Verklaarde bereidheid om alle studieprocedures na te leven en beschikbaarheid voor de duur van de proef voor follow-up via telefoon, persoonlijk, e-mail, en/of videobezoeken of correspondentie.
Exclusiecriteria:
- Dysfagie voor pillen
- Klinisch actieve inflammatoire darmziekte
- Voorgeschiedenis van coeliakie
- Op de wachtlijst voor transplantatie, en verwachte plaatsing op transplantatielijst of LVAD-plaatsing in de komende 6 maanden
- Acute myocarditis
- Infiltratieve en hypertrofische cardiomyopathieën
- Nierziekte die dialyse vereist
- Zwangerschap of borstvoeding. Een zwangerschapstest zal worden afgenomen bij vrouwen in de vruchtbare leeftijd tijdens het screeningsbezoek of dag 1 (voor ontvangst van IMT). Patiënten met een positieve zwangerschapstest zullen worden uitgesloten. Een negatief resultaat is vereist voor proefpersonen die vrouwen in de vruchtbare leeftijd zijn om IMT-behandeling te ontvangen. Patiënten zullen worden geadviseerd om zwangerschap te vermijden, wat de standaardzorg is voor patiënten met CHF.
- Levensverwachting van < 6 maanden
- Aanwezigheid van ileostoma of colostoma
- Patiënten die immunosuppressiva gebruiken (calcineurineremmers, prednison ≥ 20 mg/dag, methotrexaat, azathioprine, immunosuppressieve biologische middelen, JAK-remmers).
- Patiënten met neutropenie (een absoluut neutrofiel aantal < 0,5 x 109 cellen/L) verkregen via een volledig bloedbeeld met differentiaal bij screening
- Voorgeschiedenis van vaste orgaan- of beenmergtransplantatie
- Verwacht frequent antibioticagebruik (patiënten met frequente urineweginfecties of sinusitis)
- Voorgeschiedenis van ernstige anafylactische voedselallergie
- Patiënten die kankerchemotherapie, immunotherapie of bestraling ondergaan

Experimenteel: Deelnemers met chronisch hartfalen (CHF)
Antibiotica preconditionering: Het antibioticumconditioneringsregime vindt plaats in de 7 dagen voorafgaand aan de IMT-therapie. Antibioticumconditionering bestaat uit 250 mg Vancomycine tweemaal daags gedurende 7 dagen + 500 mg Neomycine tweemaal daags gedurende 7 dagen. IMT-dosering: Een enkele dosis bestaat uit vier, maat 00 capsules van MTP-101C. Een individuele studie deelnemer zal het geneesmiddel alleen uit één enkele batch nemen. Elke batch bevat microbiota gescheiden en bereid uit de ontlasting van de individuele donor en bevat de data van donatie. Elke dosis (vier capsules) wordt dagelijks 's ochtends op een lege maag ingenomen. Helder vloeibaar voedsel is toegestaan, en een normaal ontbijt kan twee uur later volgen. Alternatief kunnen de capsules na vier uur van helder vloeibaar voedsel worden ingenomen, en geen ander voedsel dan helder vloeibaar voedsel is toegestaan voor twee uur nadat de capsules zijn ingenomen. De IMT-behandeling wordt gedurende zeven dagen toegediend. Echter, als de patiënt wordt opgenomen in het ziekenhuis of de behandeling wil stoppen, zal deze worden gestopt. |
Compound MTP-101-C werd oorspronkelijk ontwikkeld voor de behandeling van het recidiverend C. difficile-infectiesyndroom (rCDI) na voltooiing van de anti-C. difficile antibioticakuur.
Fecale microbiota wordt bereid uit ontlasting van gezonde menselijke donoren, die worden gescreend en getest op infectieuze en niet-infectieuze ziekten.
Rauwe ontlasting wordt gehomogeniseerd en gefilterd om de microbiota te scheiden.
De fecale microbiota wordt bevroren in aanwezigheid van een lyoprotectant (trehalose), gevriesdroogd en ingekapseld in hypromellose-capsules (DRcaps van Capsugel, Morristown, NJ).
Elke capsule bevat ≥ 1 x 10¹¹ en ≤ 2,0 x 10¹¹ bacteriële cellen.
