AERO CAD: Een prospectieve, multicenter, single-arm studie ter evaluatie van het Shockwave C2 Aero Coronary IVL Systeem bij gecalcificeerde coronaire hartziekte
De AERO CAD-studie is een prospectief, multicentrisch, single-arm, post-marketing klinisch onderzoek dat de procedurele prestaties en klinische resultaten evalueert van het Shockwave C2 Aero Coronary Intravascular Lithotripsy (IVL)-systeem bij patiënten met gecalcificeerde coronaire arterieziekte die percutane coronaire interventie (PCI) ondergaan.
De studie zal tot 200 proefpersonen inschrijven op maximaal 8 Amerikaanse locaties. Het primaire doel is om real-world bewijs te genereren over de prestaties van het apparaat en de klinische resultaten, inclusief falen van de doel-laesie na 30 dagen en procedureel succes.

De AERO CAD-studie is ontworpen om de klinische prestaties en resultaten in de praktijk te karakteriseren die geassocieerd zijn met de commercieel beschikbare Shockwave C2 Aero Coronaire IVL-katheter.
Coronaire arteriële verkalking vormt een grote uitdaging tijdens PCI, wat vaak leidt tot suboptimale stentexpansie en nadelige klinische uitkomsten. Intravasculaire lithotripsie (IVL) is naar voren gekomen als een calciummodificatietechnologie die lesievoorbereiding bij lage ballondrukken vergemakkelijkt.
De C2 Aero IVL-katheter vertegenwoordigt een volgende generatie-iteratie met verbeterde afleverbaarheid, een lager profiel, verbeterde hydrofiele coating en verbeterde ballonduurzaamheid. Deze studie evalueert het gebruik ervan in de routinematige klinische praktijk.
Dit is een prospectieve, eenarmige, interventionele post-marketingstudie die patiënten inschrijft die PCI ondergaan met IVL en stentplaatsing volgens de standaardzorg. Er zijn geen aanvullende procedures vereist buiten de standaard klinische praktijk.
Proefpersonen worden gevolgd tot ontslag, na 30 dagen en na 1 jaar.

Inclusiecriteria:
- Leeftijd ≥18 jaar
- Patiënt die percutane coronaire interventie (PCI) ondergaat voor coronaire hartziekte
- Doellaesie(s) bedoeld voor behandeling met intravasculaire lithotripsie (IVL) en stentplaatsing volgens de standaardzorg
- In staat om geïnformeerde toestemming te geven
- Bereidheid en vermogen om te voldoen aan de vervolgvereisten van de studie
Exclusiecriteria:
- ST-elevatie myocardinfarct (STEMI) bij presentatie
- Cardiogene shock op het moment van de procedure
- In-stent restenose (ISR) doellaesie
- Bekende contra-indicatie voor antiplaatjestherapie of antistollingstherapie
- Bekende intolerantie of contra-indicatie voor contrastmiddel die niet vatbaar is voor premedicatie
- Deelname aan een andere studie met een onderzoekend geneesmiddel of apparaat die de eindpunten van deze studie zou kunnen beïnvloeden
- Levensverwachting <1 jaar
- Onvermogen of onwil om te voldoen aan de studieprocedures of follow-up
