BAKGRUNN:
Asymptomatisk hjerteiskemi, ofte betegnet som stille myokardiskemi, refererer til episoder med objektivt påviselig forbigående iskemi i fravær av symptomer. Årlig lider mer enn en million pasienter av akutt hjerteinfarkt. Av de rundt 700 000 som skrives ut i live fra sykehuset, vil så mange som 180 000 være asymptomatiske, men ha tegn på myokardiskemi på en treningstest før utskrivning. Asymptomatisk iskemi har vært knyttet til plutselig død eller hjerteinfarkt. Det er anslått at i USA har over seks millioner pasienter kronisk, symptomatisk koronar hjertesykdom, og opptil tre millioner av disse kan ha forbigående asymptomatisk myokardiskemi. Asymptomatisk iskemi antas å være tilstede hos de fleste pasienter med koronar hjertesykdom med stabil angina pectoris, over 70 prosent av alle iskemiske episoder er asymptomatiske. Asymptomatisk iskemi etter hjerteinfarkt eller i nærvær av kronisk stabil angina kan være assosiert med betydelig økt sykelighet og dødelighet
Tradisjonelt har behandling av pasienter med koronar hjertesykdom vært gitt for og styrt av pasientenes symptomer. Det var en økende trend i retning av å gjenkjenne asymptomatisk hjerteiskemi og dermed like stor betydning som symptomatisk iskemi. Mange leger mente at undertrykkelse av asymptomatisk iskemi hos pasienter med koronar hjertesykdom ville redusere sykelighet og dødelighet. Dette førte til raskt økende og utbredte anvendelser av både medisinske og revaskulariseringsterapier.
I 1989 var det mangel på kunnskap om den relative effekten av ulike behandlingsstrategier for å kontrollere asymptomatisk hjerteiskemi. Gitt den estimerte høye prevalensen av asymptomatisk hjerteiskemi hos pasienter med koronar hjertesykdom og bevis på økt risiko for uønsket utfall, var folkehelseproblemet av tilstrekkelig omfang til å berettige en pilotstudie for å avgjøre i hvilken grad asymptomatisk iskemi kunne kontrolleres. Hvis pilotstudien viser gjennomførbarhet, vil en fullskala klinisk studie bli vurdert for å evaluere effekten av effektiv behandling av asymptomatisk iskemi på overlevelse og kardiovaskulær sykelighet hos pasienter med koronar hjertesykdom.
DESIGN NARRATIV:
Totalt 1 959 pasienter ble screenet ved AECG-overvåking; 49 prosent hadde asymptomatisk iskemi, og 65 prosent ble registrert i studien. De 618 pasientene ble randomisert til en av de tre behandlingsstrategiene: 202 til angina-veiledet medisinsk strategi med titrering av anti-iskemisk medisin for å lindre angina; 202 til angina-veiledet pluss AECG-iskemi-veiledet medisinsk strategi med titrering av anti-iskemisk medisin for å eliminere både angina og AECG-iskemi; og 212 til revaskularisering ved angioplastikk eller bypasskirurgi. Pasienter som kunne ta enten beta-adrenerge blokkere eller kalsiumantagonister ble også randomisert til å motta ett av to medisinske kombinasjonsregimer: atenolol pluss nifedipin eller diltiazem pluss isosorbiddinitrat. De som kunne behandles med bare ett regime, for eksempel astmatiske pasienter, ble tildelt riktig regime. Det primære resultatet var fravær av iskemi etter tolv uker. Rekrutteringen ble avsluttet i januar 1993. Klinisk basert oppfølging ble fullført i 18 måneder og overlevelsesstatus fri for MI ble fullført i 24 måneder
