Asymptomatische cardiale ischemie-pilootstudie (ACIP).

ACHTERGROND:
Asymptomatische cardiale ischemie, vaak stille myocardiale ischemie genoemd, verwijst naar episodes van objectief aantoonbare voorbijgaande ischemie zonder symptomen. Jaarlijks krijgen meer dan een miljoen patiënten een acuut hartinfarct. Van de ongeveer 700.000 die levend uit het ziekenhuis worden ontslagen, zullen maar liefst 180.000 asymptomatisch zijn, maar bij een inspanningstest vóór ontslag tekenen van myocardischemie vertonen. Asymptomatische ischemie is in verband gebracht met plotseling overlijden of een hartinfarct. Geschat wordt dat in de Verenigde Staten meer dan zes miljoen patiënten een chronische, symptomatische coronaire hartziekte hebben en tot drie miljoen van deze kunnen voorbijgaande asymptomatische myocardischemie vertonen. Aangenomen wordt dat asymptomatische ischemie aanwezig is bij de meerderheid van de patiënten met coronaire hartziekte met stabiele angina pectoris, waarbij meer dan 70 procent van alle ischemische episodes asymptomatisch is. Asymptomatische ischemie na een myocardinfarct of in aanwezigheid van chronische stabiele angina kan gepaard gaan met aanzienlijk verhoogde morbiditeit en mortaliteit
Traditioneel wordt de behandeling van patiënten met coronaire hartziekte gegeven voor en geleid door de symptomen van de patiënt. Er was een groeiende trend in de richting van het herkennen van asymptomatische cardiale ischemie en volgens het belang dat gelijk is aan dat van symptomatische ischemie. Veel artsen waren van mening dat onderdrukking van asymptomatische ischemie bij patiënten met coronaire hartziekten de morbiditeit en mortaliteit zou verminderen. Dit leidde tot snel toenemende en wijdverbreide toepassingen van zowel medische als revascularisatietherapieën.
In 1989 was er een gebrek aan kennis over de relatieve werkzaamheid van verschillende behandelingsstrategieën om asymptomatische cardiale ischemie te beheersen. Gezien de geschatte hoge prevalentie van asymptomatische cardiale ischemie bij patiënten met coronaire hartziekte en het bewijs van een verhoogd risico op een ongunstige afloop, was het volksgezondheidsprobleem van voldoende omvang om een pilotstudie te rechtvaardigen om te bepalen in hoeverre asymptomatische ischemie onder controle kon worden gehouden. Als de pilootstudie haalbaarheid aantoonde, zou een klinische proef op volledige schaal worden overwogen om de impact van een effectieve behandeling van asymptomatische ischemie op overleving en cardiovasculaire morbiditeit bij patiënten met coronaire hartziekte te evalueren.
ONTWERP VERHAAL:
In totaal werden 1.959 patiënten gescreend door middel van AECG-monitoring; 49 procent had asymptomatische ischemie en 65 procent deed mee aan het onderzoek. De 618 patiënten werden gerandomiseerd naar een van de drie behandelingsstrategieën: 202 naar angina-geleide medische strategie met titratie van anti-ischemische medicatie om angina pectoris te verlichten; 202 tot angina-geleide plus AECG-ischemie-geleide medische strategie met titratie van anti-ischemische medicatie om zowel angina als AECG-ischemie te elimineren; en 212 op revascularisatie door angioplastiek of bypassoperatie. Patiënten die bèta-adrenerge blokkers of calciumantagonisten konden gebruiken, werden ook gerandomiseerd om een van de twee medische combinatieregimes te krijgen: atenolol plus nifedipine of diltiazem plus isosorbidedinitraat. Degenen die met slechts één regime konden worden behandeld, zoals astmapatiënten, werden toegewezen aan het juiste regime. De primaire uitkomstmaat was de afwezigheid van ischemie na twaalf weken. Werving eindigde in januari 1993. Klinische follow-up was gedurende 18 maanden voltooid en de overlevingsstatus vrij van MI was gedurende 24 maanden voltooid
