BAGGRUND:
Asymptomatisk hjerteiskæmi, ofte betegnet som stille myokardieiskæmi, refererer til episoder med objektivt påviselig forbigående iskæmi i fravær af symptomer. Årligt lider mere end en million patienter af akut myokardieinfarkt. Af de omkring 700.000, der udskrives levende fra hospitalet, vil hele 180.000 være asymptomatiske, men have tegn på myokardieiskæmi på en træningstest før udskrivelsen. Asymptomatisk iskæmi er blevet forbundet med pludselig død eller myokardieinfarkt. Det anslås, at over seks millioner patienter i USA har kronisk, symptomatisk koronar hjertesygdom, og op til tre millioner af disse kan udvise forbigående asymptomatisk myokardieiskæmi. Asymptomatisk iskæmi menes at være til stede hos de fleste patienter med koronar hjertesygdom med stabil angina pectoris, hvor over 70 procent af alle iskæmiske episoder er asymptomatiske. Asymptomatisk iskæmi efter myokardieinfarkt eller ved tilstedeværelse af kronisk stabil angina kan være forbundet med væsentligt øget morbiditet og mortalitet
Traditionelt har behandling af patienter med koronar hjertesygdom været givet for og styret af patienternes symptomer. Der var en voksende tendens til at erkende asymptomatisk hjerteiskæmi og i overensstemmelse hermed lige så vigtig som symptomatisk iskæmi. Mange læger mente, at undertrykkelse af asymptomatisk iskæmi hos patienter med koronar hjertesygdom ville reducere morbiditet og dødelighed. Dette førte til hurtigt voksende og udbredte anvendelser af både medicinske og revaskulariseringsterapier.
I 1989 var der mangel på viden om den relative effektivitet af forskellige behandlingsstrategier til at kontrollere asymptomatisk hjerteiskæmi. I betragtning af den estimerede høje forekomst af asymptomatisk hjerteiskæmi hos patienter med koronar hjertesygdom og tegn på øget risiko for uheldigt udfald, var folkesundhedsproblemet af tilstrækkelig størrelse til at berettige et pilotstudie for at bestemme, i hvilket omfang asymptomatisk iskæmi kunne kontrolleres. Hvis pilotstudiet viste gennemførlighed, ville et fuldskala klinisk forsøg derefter blive overvejet for at evaluere virkningen af effektiv behandling af asymptomatisk iskæmi på overlevelse og kardiovaskulær morbiditet hos patienter med koronar hjertesygdom.
DESIGN FORTÆLLING:
I alt 1.959 patienter blev screenet ved AECG-monitorering; 49 procent havde asymptomatisk iskæmi, og 65 procent var indskrevet i undersøgelsen. De 618 patienter blev randomiseret til en af de tre behandlingsstrategier: 202 til angina-guidet medicinsk strategi med titrering af anti-iskæmisk medicin for at lindre angina; 202 til angina-guidet plus AECG iskæmi-guidet medicinsk strategi med titrering af anti-iskæmisk medicin for at eliminere både angina og AECG iskæmi; og 212 til revaskularisering ved angioplastik eller bypass-kirurgi. Patienter, der var i stand til at tage enten beta-adrenerge blokerende midler eller calciumantagonister, blev også randomiseret til at modtage en af to medicinske kombinationsregimer: atenolol plus nifedipin eller diltiazem plus isosorbiddinitrat. De, der kun kunne behandles med én kur, såsom astmatiske patienter, blev tildelt den passende kur. Det primære resultat var fravær af iskæmi efter 12 uger. Ansættelsen sluttede i januar 1993. Klinisk baseret opfølgning blev afsluttet i 18 måneder, og overlevelsesstatus fri for MI blev afsluttet i 24 måneder
