En randomisert, multisenter, dobbeltblind, fase 3-studie av Nivolumab, Nivolumab i kombinasjon med Ipilimumab, eller placebo som vedlikeholdsterapi hos pasienter med omfattende sykdommer småcellet lungekreft (ED-SCLC) etter fullføring av platinabasert førstelinje Kjemoterapi (CheckMate 451: CHECKpoint Pathway og nivoluMAb Clinical Trial Evaluation 451)

I denne studien må alle pasienter allerede ha fullført førstelinjekjemoterapi for å behandle småcellet lungekreft i omfattende stadium av sykdom. Hensikten med denne studien er å vise at nivolumab, eller nivolumab pluss ipilimumab etterfulgt av nivolumab i seg selv, vil forlenge total overlevelse når det administreres som konsolideringsbehandling hos pasienter som er stabile eller responderer etter kjemoterapi. Pasienter som får behandling vil bli sammenlignet med pasienter som får placebo.

Inklusjonskriterier:

  • Personer med histologisk eller cytologisk bekreftet omfattende stadiumsykdom SCLC
  • Pågående respons av stabil sykdom eller bedre etter 4 sykluser med platinabasert førstelinjekjemoterapi
  • Eastern Cooperative Oncology Group (ECOG) ytelsesstatus på 0 eller 1

Ekskluderingskriterier:

  • Personer med symptomatiske metastaser i sentralnervesystemet (CNS).
  • Personer som mottar konsoliderende bryststråling
  • Personer med aktiv, kjent eller mistenkt autoimmun sykdom er ekskludert
  • Alle bivirkninger som tilskrives tidligere anti-kreftbehandling må ha forsvunnet til grad 1 eller baseline

Deltakere som ikke hadde noen tumorvurderinger i studien og som ikke døde (eller døde etter oppstart av den påfølgende anti-kreftbehandlingen) ble sensurert på randomiseringsdatoen. Deltakere som startet enhver påfølgende anti-kreftbehandling uten en tidligere rapportert progressiv sykdom (PD) per BICR, ble sensurert ved den siste evaluerbare tumorvurderingen på eller før oppstart av den påfølgende anti-kreftbehandlingen.

Tumormutasjonsbelastning (TMB) måles ved hjelp av FoundationOne CDxTM (F1CDx)-analyse, en omfattende genomisk profil (CGP)-analyse basert på tumorvev ved baseline. TMB er definert som antall somatiske, kodende, basesubstitusjons- og indelmutasjoner per megabase av genomet undersøkt.

OS i TMB ved følgende grensepunkter: ≥10 mutasjoner/mb, < 10 mutasjoner/mb, ≥13 mutasjoner/mb, <13 mutasjoner/mb
