Een gerandomiseerde, multicenter, dubbelblinde, fase 3-studie van nivolumab, nivolumab in combinatie met ipilimumab of placebo als onderhoudstherapie bij proefpersonen met kleincellige longkanker (ED-SCLC) in een uitgebreid stadium na voltooiing van op platina gebaseerde eerstelijnsbehandeling Chemotherapie (CheckMate 451: CHECKpoint Pathway en nivoluMAb Clinical Trial Evaluation 451)

In deze studie moeten alle patiënten al eerstelijns chemotherapie hebben ondergaan om kleincellige longkanker in een uitgebreid stadium te behandelen. Het doel van deze studie is om aan te tonen dat nivolumab, of nivolumab plus ipilimumab gevolgd door nivolumab alleen, de algehele overleving zal verlengen wanneer het wordt toegediend als consolidatiebehandeling bij patiënten die stabiel zijn of reageren na chemotherapie. Patiënten die worden behandeld, worden vergeleken met patiënten die een placebo krijgen.

Beschrijving

Inclusiecriteria:

  • Proefpersonen met histologisch of cytologisch bevestigd SCLC in een uitgebreid stadium van de ziekte
  • Aanhoudende respons van stabiele ziekte of beter na 4 cycli van op platina gebaseerde eerstelijnschemotherapie
  • Eastern Cooperative Oncology Group (ECOG) prestatiestatus van 0 of 1

Uitsluitingscriteria:

  • Proefpersonen met symptomatische metastasen van het centrale zenuwstelsel (CZS).
  • Onderwerpen die consoliderende thoraxstraling ontvangen
  • Proefpersonen met een actieve, bekende of vermoede auto-immuunziekte zijn uitgesloten
  • Alle bijwerkingen die zijn toegeschreven aan eerdere antikankertherapie moeten zijn verdwenen tot graad 1 of baseline

De OS werd opgevolgd tijdens de behandeling met het geblindeerde onderzoeksgeneesmiddel en elke 3 maanden via persoonlijk of telefonisch contact nadat de deelnemer stopte met het geblindeerde onderzoeksgeneesmiddel.

PFS werd gedefinieerd als de tijd tussen de datum van randomisatie en de eerste datum van gedocumenteerde progressie zoals bepaald door Blind Independent Central Review (BICR) of overlijden door welke oorzaak dan ook, afhankelijk van wat zich het eerst voordeed. Deelnemers die stierven zonder gerapporteerde progressie werden beschouwd als progressief op de datum van overlijden. Deelnemers die geen vooruitgang boekten of stierven, werden gecensureerd op de datum van hun laatste evalueerbare tumorbeoordeling op of voorafgaand aan de start van de daaropvolgende antikankertherapie. Deelnemers die tijdens de studie geen tumorbeoordelingen hadden en niet stierven (of stierven na het starten van de daaropvolgende antikankertherapie) werden gecensureerd op hun datum van randomisatie. Deelnemers die met een volgende antikankertherapie begonnen zonder een eerder gemelde progressieve ziekte (PD) volgens BICR, werden gecensureerd bij de laatste evalueerbare tumorbeoordeling op of voorafgaand aan de start van de daaropvolgende antikankertherapie.

De tumormutatielast (TMB) wordt gemeten met behulp van de FoundationOne CDxTM (F1CDx)-assay, een uitgebreide genomische profielassay (CGP) op basis van baseline tumorweefsel. TMB wordt gedefinieerd als het aantal somatische, coderings-, basesubstitutie- en indelmutaties per megabase van het onderzochte genoom.

OS in TMB volgens de volgende afkappunten: ≥10 mutaties/mb, <10 mutaties/mb, ≥13 mutaties/mb, <13 mutaties/mb
