Inklusjonskriterier:
- Fellesskapet må være lokalisert i en av de fire deltakende statene: Kentucky, Ohio, Massachusetts eller New York.
- Minst 30 % av samfunnene som er valgt i hver stat må være landlige.
- På tvers av alle HCS-samfunnene i hver stat må det være minimum 150 opioidrelaterte overdosedødsfall (med minst 22 opioidrelaterte overdosedødsfall opplevd av landlige samfunn) og en rate på minst 25 opioidrelaterte overdosedødsfall pr. 100 000 personer, basert på 2016-data.
- Samfunnet må uttrykke vilje til å ta opp i sin responsstrategi implementering av MOUD, overdoseforebyggende opplæring og naloksondistribusjon over hele samfunnet.
- Samfunnet må uttrykke vilje til å utvikle partnerskap på tvers av helsevesen, atferdshelse og rettsmiljøer for evidensbasert praksis for å håndtere opioidmisbruk, OUD og overdoser.
Ekskluderingskriterier:
- Fellesskap som ikke oppfylte de nevnte inklusjonskriteriene ble ekskludert fra HCS.

Eksperimentell: Bølge 1 - Intervensjon
Fellesskap i Wave 1 vil motta CTH-intervensjonen i løpet av de første 30 månedene av forsøket.

Gjennomsnittlig frekvens av akuttmedisinsk tjeneste (EMS) kjører for en mistenkt overdosering av opioider per 1000 voksne for å vurdere potensielle bivirkninger og alvorlige bivirkninger i samfunnet. Dette er data om fellesskapsnivå for EMS -kjøringer (mistenkte opioid overdoseringshendelser i samfunnsområdene), men ikke dødsfall. Vi måtte bruke et surrogatmål (EMS kjører: ikke det samme som dødsfall, noen kan ha dødd, men andre kan ha blitt lagret) siden de decedent/dødsattestdataene hadde for lang etterslep. AE-kriterium: ORO (opioidrelatert overdose) for en enkelt måned økte> 3 standardavvik (SDS) over den bevegelige AVG de siste tre månedene. Vi planlegger å gjøre studiemetoder, data og resultater tilgjengelig for alle som er interessert i å redusere byrden av opioidkrisen. Datadelingsplanen vil være i samsvar med NIH HEAL Initiative ClinicalTrials.govSM Retningslinjer for offentlig tilgang og datadeling, NIH-retningslinjene for datadeling og NIH-retningslinjene for formidling av NIH-finansiert informasjon om kliniske forsøk, og NIH-regelen for registrering av kliniske forsøk og innsending av resultatinformasjon.
Datadelingsplanen vil følge NIHs krav til deling av data via opprettelse av datasett til offentlig bruk. En strukturert prosess vil bli brukt for å evaluere risikoen for re-identifikasjon basert på veiledningsdokumenter fra US DHHS og HIPAA.
