Descripción
Criterios de inclusión:
- La comunidad debe estar ubicada en uno de los cuatro estados participantes: Kentucky, Ohio, Massachusetts o Nueva York.
- Al menos el 30% de las comunidades seleccionadas dentro de cada estado deben ser rurales.
- En todas las comunidades de HCS dentro de cada estado, debe haber un mínimo de 150 muertes por sobredosis relacionadas con opioides (con al menos 22 muertes por sobredosis relacionadas con opioides experimentadas por las comunidades rurales) y una tasa de al menos 25 muertes por sobredosis relacionadas con opioides por año. 100.000 personas, según datos de 2016.
- La comunidad debe expresar su voluntad de abordar en su estrategia de respuesta la implementación de MOUD, la capacitación para la prevención de sobredosis y la distribución de naloxona en su comunidad.
- La comunidad debe expresar su voluntad de desarrollar asociaciones en entornos de atención médica, salud conductual y justicia para prácticas basadas en evidencia para abordar el uso indebido de opioides, OUD y sobredosis.

- Las comunidades que no cumplieron con los criterios de inclusión antes mencionados fueron excluidas de la HCS.

Experimental: Ola 1 - Intervención
Las comunidades en Wave 1 recibirán la intervención de CTH durante los primeros 30 meses de la prueba.

La tasa media de servicio médico de emergencia (EMS) se ejecuta para una supuesta sobredosis de opioides por cada 1000 adultos para evaluar posibles eventos adversos y eventos adversos graves en la comunidad. Estos son los datos a nivel comunitario de las ejecuciones EMS (presuntos eventos de sobredosis de opioides en las áreas de la comunidad), pero no las muertes. Tuvimos que usar una medida sustituta (EMS corre: no es lo mismo que las muertes, algunas pueden haber muerto, pero otros pueden haber sido guardados) ya que los datos de certificados de difunto/muerte tuvieron un retraso demasiado largo. Criterio AE: Tasa de ORO (sobredosis relacionada con los opioides) durante un solo mes aumentó> 3 desviaciones estándar (SDS) por encima del AVG móvil de los 3 meses anteriores. Criterio SAE: Tasa de ORO durante un solo mes aumentó> 4 SDS por encima del AVG móvil de los 3 meses anteriores. Descripción del plan IPD
