Critério de inclusão:
- A comunidade deve estar localizada em um dos quatro estados participantes: Kentucky, Ohio, Massachusetts ou Nova York.
- Pelo menos 30% das comunidades selecionadas dentro de cada estado devem ser rurais.
- Em todas as comunidades HCS dentro de cada estado, deve haver um mínimo de 150 mortes por overdose relacionadas a opioides (com pelo menos 22 mortes por overdose relacionadas a opioides experimentadas pelas comunidades rurais) e uma taxa de pelo menos 25 mortes por overdose relacionadas a opioides por 100.000 pessoas, com base em dados de 2016.
- A comunidade deve expressar vontade de abordar em sua estratégia de resposta a implementação de MOUD, treinamento de prevenção de overdose e distribuição de naloxona em sua comunidade.
- A comunidade deve expressar vontade de desenvolver parcerias em cuidados de saúde, saúde comportamental e configurações de justiça para práticas baseadas em evidências para lidar com o uso indevido de opioides, OUD e overdoses.
Critério de exclusão:
- As comunidades que não atenderam aos critérios de inclusão supracitados foram excluídas do SAD.

Experimental: Onda 1 - Intervenção
As comunidades na Onda 1 receberão a intervenção CTH durante os primeiros 30 meses do estudo.
A intervenção incluirá 3 componentes: envolvimento da comunidade para ajudar as principais partes interessadas na aplicação de práticas baseadas em evidências para lidar com sua crise de opioides, um menu de práticas baseadas em evidências para as comunidades selecionarem e implementarem e uma campanha de comunicação para aumentar a demanda por práticas para lidar com overdose e transtorno de uso de opiáceos.

A intervenção Comunidades que curam (CTH) é uma intervenção engajada na comunidade projetada para aumentar a adoção de um conjunto integrado de práticas baseadas em evidências fornecidas em cuidados de saúde, saúde comportamental, justiça e outros ambientes baseados na comunidade.
Inclui 3 componentes: envolvimento da comunidade para ajudar as principais partes interessadas na aplicação de práticas baseadas em evidências para lidar com sua crise de opioides, um menu de práticas baseadas em evidências para as comunidades selecionarem e implementarem e uma campanha de comunicação para criar demanda por práticas baseadas em evidências para abordar overdose e transtorno do uso de opioides.

Contagem de indivíduos com um ou mais dos seguintes seguintes durante o período do estudo de doze meses (meses 19-30), e não em uma janela de tempo especificada anterior (3 meses): risco de uso contínuo de opióides (novo episódio de opióides com duração de pelo menos 31 dias); iniciar o tratamento opióide com liberação prolongada ou opióide de ação prolongada; dosagem alta incidente (média ≥ 90 mg de morfina por dia); e/ou incidentes opióides sobrepostos e benzodiazepina por ≥ 30 dias.

Incidência de eventos adversos por 1000 membros da comunidade

A taxa média de Serviço Médico de Emergência (EMS) concorre a uma suspeita de overdose de opióides por 1.000 adultos para avaliar possíveis eventos adversos e eventos adversos graves na comunidade. São dados de nível comunitário das execuções do EMS (suspeitos de eventos de overdose de opióides nas áreas comunitárias), mas não mortes. Tivemos que usar uma medida substituta (o EMS corre: não é o mesmo que as mortes, alguns podem ter morrido, mas outros podem ter sido salvos), uma vez que os dados do falecido/atestado de óbito tinham muito tempo. Critério de AE: Oro (overdose relacionada a opióides) por um único mês aumentou> 3 desvios padrão (SDS) acima do AVG em movimento dos 3 meses anteriores. Critério SAE: a taxa de ORO por um único mês aumentou> 4 SDs acima do AVG em movimento dos 3 meses anteriores. O SD foi determinado a partir das taxas mensais das comunidades para os últimos 12 meses disponíveis, antes do início da intervenção, usando uma estimativa combinada entre as comunidades. Não monitoramos no nível de participante individual. |
