Påmelding: Studiet omfatter to faser. Fase 1 inkluderer fokusgrupper blant 30 innfødte Hawaiian/Pacific Islander kvinner med risiko for endometriekreft. Fokusgrupper vil bestemme sosiokulturelle påvirkninger på (i) kostholdsatferd inkludert tidspunkt for måltider; (ii) kostholdspreferanser; (iii) foreslåtte måltidsplaner og intervensjonsdesign; (iv) oppfatninger av vekttap og kroppsbilde. I den andre studiefasen vil studieteamet rekruttere 10 Pacific Islander-kvinner til å delta i en 14-ukers randomisert diett-crossover-studie. Hver kvinne vil delta i begge intervensjonsarmer og fungere som sin egen kontroll.
Fokusgruppe: For å oppnå alle aspekter av mål 1 av denne studien, vil studieteamet rekruttere 30 Pacific Islander-kvinner med risiko for endometriekreft for å gjennomføre virtuelle fokusgrupper. Disse fokusgruppene vil finne sted via privat videokonferanse, som Zoom, og en språkoversetter vil bli gitt, etter behov. Fokusgruppene vil bli deltatt av en innfødt Hawaiian/Pacific Islander Community Expert. Spørsmål vil bli utformet for å forstå sosiokulturelle påvirkninger på kostholdsatferd (spørsmål om sosial spising, timing av måltider og kostholdspreferanser, innhenting av tilbakemelding på design av studieintervensjon, og oppfatninger av vekttap og kroppsbilde). En isbryter vil bli utført før det formelle avhøret. Økter vil bli tatt opp med lyd på videokonferansen og en ekstra enhet, transkribert ordrett og sjekket for nøyaktighet. Ytterligere to teammedlemmer vil være til stede som notater. Vår samfunnsekspert vil være til stede for å hjelpe til med å tilrettelegge økten. Spørsmål vil bli gitt i begynnelsen av økten for å sikre kulturell sensitivitet.
Diettintervensjon: Diettintervensjonen inkluderer 5 studiebesøk, og har et baseline-besøk, 2-ukers innkjøringsperiode, 4-ukers kontrolldiettplan, 4-ukers utvaskingsperiode og en 4-ukers, 8 til 10-timers tid. Restricted Eating (TRE)-protokoll (8 til 10-timers fôring og 14 til 16-timers fasteperioder). Et oppfølgingsskjema vil bli sendt 6 måneder etter intervensjonen. For kontrollplanen vil kvinner motta frosne lunsj- og middagsmåltider og en standardisert frokost- og snacksmeny. Alle måltider og frokost-/snacksmenyen vil være utformet for å være kulturelt kompetente med tilbakemeldinger fra fokusgruppene, og måltidsplaner vil bli individualisert for å møte energikravene til vektvedlikehold. For TRE-planen vil kvinner motta tilberedte frosne lunsj- og middagsmåltider i henhold til kontrollplanen, men vil bli bedt om å innta daglige måltider, snacks og kaloriholdige drikker innen en 8 til 10-timers periode som passer deres timeplan. De vil bli bedt om å opprettholde sin vanlige søvn- og fysiske aktivitetsplan. Fasteperioden vil øke i løpet av den første uken (dager 1-3, 12-14 timer per dag, dag 4-6, 14-16 timer per dag, dager 7+, 16 timer per dag). Kvinner vil bli bedt om å planlegge spiseperioden til samme tid hver dag. Alle måltider (kontroll og TRE) vil bli levert av Metabolic Kitchen som ligger ved University of Utah.
Besøk 1: Under baseline-besøket vil en koordinator innhente samtykke fra deltakeren, og de vil bli bedt om å konkurrere spørreskjemaer som spør om kliniske og demografiske faktorer, kronotype og fysisk aktivitet, og ulike antropometriske målinger vil bli tatt. Etter baseline-besøket vil deltakerne gjennomgå en 2-ukers innkjøringsperiode. I løpet av denne tiden er målet å registrere kvinners vanlige tider med spiseepisoder og appetitt mens de følger deres vanlige diett.
Besøk 2: Kvinner vil gi fastende blod- og avføringsprøver, og det vil bli tatt antropometriske målinger. Etter besøk 2 vil deltakerne gjennomgå kontroll- eller TRE-planen i 4 uker.
Besøk 3: Etter 4 uker vil deltakerne gjennomgå besøk 3 og vil bli bedt om å gi fastende blod- og avføringsprøver, og antropometriske tiltak vil bli tatt. Etter besøk 3 vil deltakerne gjennomgå en 4-ukers utvaskingsperiode, hvor kvinner kan gå tilbake til sin vanlige livsstil.
Besøk 4: Besøk 4 vil finne sted etter den 4-ukers utvaskingsperioden, og vil inkludere en telefonkonsultasjon for å diskutere neste studiefase og planlegge måltider. I de påfølgende 4 ukene vil deltakerne deretter gjennomgå motsatt fôringsplan (enten kontroll eller TRE).
Besøk 5: Under det siste besøket vil kvinner gi fastende blod- og avføringsprøver, og det vil bli tatt antropometriske tiltak.
I løpet av innkjøringsperioden, TRE-fasen og kontrollfasen vil deltakerne registrere tidspunktet for spiseepisoder ved hjelp av telefonappen MyCircadian Clock, fullføre 3 x 24-timers diettinnkalling, en enkelt fysisk aktivitetsgjenkalling og 7-dagers søvn og fysisk aktivitet vil bli målt objektivt ved hjelp av en Actiwatch-enhet. Sult og metthet vil bli evaluert tilfeldig gjennom telefonappen MyCircadian Clock.
Et studieavslutningsintervju vil bli gjennomført via Zoom-konferansesamtale for å debriefe om intervensjonen og innhente detaljert informasjon om aksept av protokollen. Omtrent 6 måneder etter slutten av intervensjonsperioden vil kvinner fylle ut et oppfølgingsskjema med posten.

- Overvektig eller fedme (BMI>25 kg/m2) ELLER har en historie med ikke-insulinavhengig diabetes ELLER har en historie med kompleks atypisk endometriehyperplasi
- Ha en fungerende mobiltelefon som kan laste ned en app

Eksperimentell: Time Restricted Eating (TRE) Schedule, Then Control
In this arm of the 14-week randomized dietary crossover study, participants will undergo a 2-week run-in period, followed by an 8-hour TRE protocol (comprising 8 hours of feeding and 16 hours of fasting periods), a 4-week washout period, and a 4-week control diet schedule.

Women will receive prepared frozen lunch and dinner meals as per the control schedule, but will be asked to consume daily meals, snacks, and calorie-containing beverages within an 8 to 10-hour period that fits their schedule.
They will be asked to maintain their regular sleep and physical activity schedule.
Fasting period will ramp up during the first week (e.g., Days 1-3, 12-14 h per day, Days 4-6, 14-16 h per day, Days 7+, 16 h per day).
Women will be asked to schedule the feeding period at the same time each day.
They will be allowed to consume water and 1-2 servings of very low-calorie drinks and foods (e.g., herbal tea, diet drinks, mints, gum containing < 4 kcal/serving) to increase compliance.
All meals (control and TRE) will be provided by the Metabolic Kitchen developed in collaboration with the department of Nutrition and Integrative Physiology.
Women will receive frozen lunch/dinner meals and a standardized breakfast and snacks menu.
All meals and the breakfast/snack menu will be designed to be culturally competent.
The meal plans will be individualized to meet weight maintenance energy requirements.

TRE adherence is achieved when participants fast for 14-18 hours per day during the TRE phase, as measured by a mealtime log using the MyCircadianClock phone app.
