Inscripción: El estudio incluye dos fases. La Fase 1 incluye grupos de enfoque entre 30 mujeres nativas de Hawái/Islas del Pacífico en riesgo de cáncer de endometrio. Los grupos focales determinarán las influencias socioculturales en (i) el comportamiento dietético, incluido el horario de las comidas; (ii) preferencias dietéticas; (iii) planes de alimentación propuestos y diseño de intervención; (iv) percepciones de pérdida de peso e imagen corporal. En la segunda fase del estudio, el equipo del estudio reclutará a 10 mujeres de las islas del Pacífico para que participen en un estudio cruzado aleatorizado de dietas de 14 semanas. Cada mujer participará en ambos brazos de intervención y actuará como su propio control.
Grupo de enfoque: Para lograr todos los aspectos del Objetivo 1 de este estudio, el equipo del estudio reclutará a 30 mujeres de las islas del Pacífico en riesgo de cáncer de endometrio para realizar grupos de enfoque virtuales. Estos grupos focales se llevarán a cabo a través de videoconferencias privadas, como Zoom, y se proporcionará un traductor de idiomas, según sea necesario. A los grupos de enfoque asistirá un experto de la comunidad de nativos de Hawái/isleños del Pacífico. Las preguntas se diseñarán para comprender las influencias socioculturales en el comportamiento dietético (preguntas sobre la alimentación social, el horario de las comidas y las preferencias dietéticas, solicitando comentarios sobre el diseño de la entrega de la intervención del estudio y las percepciones de la pérdida de peso y la imagen corporal). Las sesiones se grabarán en audio en la videoconferencia y en un dispositivo adicional, se transcribirán palabra por palabra y se verificará su precisión. Dos miembros adicionales del equipo estarán presentes como tomadores de notas. Nuestro experto de la comunidad estará presente para ayudar a facilitar la sesión. Se proporcionarán preguntas al comienzo de la sesión para garantizar la sensibilidad cultural.
Intervención dietética: La intervención dietética incluye 5 visitas de estudio y presenta una visita inicial, un período de preinclusión de 2 semanas, un cronograma de dieta de control de 4 semanas, un período de lavado de 4 semanas y un período de tiempo de 8 a 10 horas de 4 semanas. Protocolo de alimentación restringida (TRE) (períodos de alimentación de 8 a 10 horas y períodos de ayuno de 14 a 16 horas). Se enviará por correo un cuestionario de seguimiento 6 meses después de la intervención. Para el horario de control, las mujeres recibirán comidas y cenas congeladas y un menú estandarizado de desayuno y meriendas. Todas las comidas y el menú de desayuno/merienda se diseñarán para que sean culturalmente competentes con los comentarios de los grupos de enfoque, y los planes de comidas se personalizarán para cumplir con los requisitos de energía para mantener el peso. Para el horario de TRE, las mujeres recibirán almuerzos y cenas congelados preparados según el horario de control, pero se les pedirá que consuman comidas diarias, refrigerios y bebidas que contengan calorías dentro de un período de 8 a 10 horas que se ajuste a su horario. Se les pedirá que mantengan su horario regular de sueño y actividad física. El período de ayuno aumentará durante la primera semana (Días 1-3, 12-14 h por día, Días 4-6, 14-16 h por día, Días 7+, 16 h por día). Se les pedirá a las mujeres que programen el período de alimentación a la misma hora todos los días. Todas las comidas (control y TRE) serán proporcionadas por Metabolic Kitchen ubicada en la Universidad de Utah.
Visita 1: Durante la visita de referencia, un coordinador obtendrá el consentimiento del participante y se le pedirá que complete cuestionarios sobre factores clínicos y demográficos, cronotipo y actividad física, y se tomarán varias medidas antropométricas. Después de la visita inicial, los participantes se someterán a un período de adaptación de 2 semanas. Durante este tiempo, el objetivo es registrar los tiempos habituales de los episodios de alimentación y el apetito de las mujeres mientras siguen su dieta habitual.
Visita 2: Las mujeres proporcionarán muestras de sangre y heces en ayunas y se tomarán medidas antropométricas. Después de la visita 2, los participantes se someterán al programa de control o TRE durante 4 semanas.
Visita 3: A las 4 semanas se realizará la visita 3 y se solicitarán muestras de sangre y heces en ayunas y se tomarán medidas antropométricas. Después de la visita 3, las participantes se someterán a un período de lavado de 4 semanas, en el que las mujeres podrán volver a su estilo de vida habitual.
Visita 4: la visita 4 tendrá lugar después del período de lavado de 4 semanas e incluirá una consulta telefónica para discutir la siguiente fase del estudio y programar la provisión de comidas. Durante las siguientes 4 semanas, los participantes se someterán al programa de alimentación opuesto (ya sea control o TRE).
Visita 5: Durante la visita final, las mujeres proporcionarán muestras de sangre y heces en ayunas, y se tomarán medidas antropométricas.
Durante el período inicial, la fase TRE y la fase de control, los participantes registrarán el tiempo de los episodios de alimentación con la aplicación de teléfono MyCircadian Clock, completarán 3 recordatorios de dieta de 24 horas, un único recordatorio de actividad física y 7 días de sueño. y la actividad física se medirá objetivamente mediante un dispositivo Actiwatch. El hambre y la saciedad se evaluarán aleatoriamente a través de la aplicación de teléfono MyCircadian Clock.
Se realizará una entrevista de salida del estudio a través de una conferencia telefónica de Zoom para informar sobre la intervención y obtener información detallada sobre la aceptabilidad del protocolo. Aproximadamente 6 meses después del final del período de intervención, las mujeres completarán un cuestionario de seguimiento enviado por correo.

Experimental: Time Restricted Eating (TRE) Schedule, Then Control
In this arm of the 14-week randomized dietary crossover study, participants will undergo a 2-week run-in period, followed by an 8-hour TRE protocol (comprising 8 hours of feeding and 16 hours of fasting periods), a 4-week washout period, and a 4-week control diet schedule.

Women will receive prepared frozen lunch and dinner meals as per the control schedule, but will be asked to consume daily meals, snacks, and calorie-containing beverages within an 8 to 10-hour period that fits their schedule.
They will be asked to maintain their regular sleep and physical activity schedule.
Fasting period will ramp up during the first week (e.g., Days 1-3, 12-14 h per day, Days 4-6, 14-16 h per day, Days 7+, 16 h per day).
Women will be asked to schedule the feeding period at the same time each day.
They will be allowed to consume water and 1-2 servings of very low-calorie drinks and foods (e.g., herbal tea, diet drinks, mints, gum containing < 4 kcal/serving) to increase compliance.
All meals (control and TRE) will be provided by the Metabolic Kitchen developed in collaboration with the department of Nutrition and Integrative Physiology.
Women will receive frozen lunch/dinner meals and a standardized breakfast and snacks menu.
All meals and the breakfast/snack menu will be designed to be culturally competent.
The meal plans will be individualized to meet weight maintenance energy requirements.
