Iscrizione: Lo studio comprende due fasi. La fase 1 include focus group tra 30 donne native hawaiane/delle isole del Pacifico a rischio di cancro dell'endometrio. I focus group determineranno le influenze socioculturali su (i) comportamento alimentare, compreso l'orario dei pasti; (ii) preferenze dietetiche; (iii) piani alimentari proposti e progettazione dell'intervento; (iv) percezioni di perdita di peso e immagine corporea. Nella seconda fase dello studio, il team di studio recluterà 10 donne delle isole del Pacifico per partecipare a uno studio crossover dietetico randomizzato di 14 settimane. Ogni donna parteciperà a entrambi i bracci di intervento e agirà come proprio controllo.
Focus Group: per realizzare tutti gli aspetti dell'obiettivo 1 di questo studio, il team di studio recluterà 30 donne delle isole del Pacifico a rischio di cancro dell'endometrio per condurre focus group virtuali. Questi focus group si svolgeranno tramite videoconferenza privata, come Zoom, e verrà fornito un traduttore linguistico, se necessario. Ai focus group parteciperà un esperto della comunità di nativi hawaiani/isolani del Pacifico. Le domande saranno progettate per comprendere le influenze socioculturali sul comportamento alimentare (domande sull'alimentazione sociale, orari dei pasti e preferenze dietetiche, sollecitazione di feedback sulla progettazione della consegna dell'intervento di studio e percezioni sulla perdita di peso e sull'immagine corporea). Un rompighiaccio sarà condotto prima dell'interrogatorio formale. Le sessioni saranno registrate audio sulla videoconferenza e su un dispositivo aggiuntivo, trascritte alla lettera e verificate per verificarne l'accuratezza. Saranno presenti altri due membri del team che prenderanno appunti. Il nostro Community Expert sarà presente per aiutare a facilitare la sessione. Le domande saranno fornite all'inizio della sessione per garantire la sensibilità culturale.
Intervento dietetico: l'intervento dietetico comprende 5 visite di studio e prevede una visita di riferimento, un periodo di rodaggio di 2 settimane, un programma dietetico di controllo di 4 settimane, un periodo di interruzione di 4 settimane e un periodo di 4 settimane, da 8 a 10 ore Protocollo Restricted Eating (TRE) (alimentazione da 8 a 10 ore e periodi di digiuno da 14 a 16 ore). Un questionario di follow-up verrà inviato per posta 6 mesi dopo l'intervento. Per il programma di controllo, le donne riceveranno pranzo e cena surgelati e un menu standardizzato per la colazione e gli spuntini. Tutti i pasti e il menu della colazione/spuntino saranno progettati per essere culturalmente competenti con il feedback dei focus group e i piani dei pasti saranno personalizzati per soddisfare i requisiti energetici per il mantenimento del peso. Per il programma TRE, le donne riceveranno pranzi e cene surgelati preparati secondo il programma di controllo, ma verrà chiesto di consumare pasti giornalieri, snack e bevande contenenti calorie entro un periodo di 8-10 ore che si adatta al loro programma. Verrà chiesto loro di mantenere il loro normale programma di sonno e attività fisica. Il periodo di digiuno aumenterà durante la prima settimana (giorni 1-3, 12-14 ore al giorno, giorni 4-6, 14-16 ore al giorno, giorni 7+, 16 ore al giorno). Alle donne verrà chiesto di programmare il periodo di alimentazione alla stessa ora ogni giorno. Tutti i pasti (controllo e TRE) saranno forniti dalla Metabolic Kitchen situata presso l'Università dello Utah.
Visita 1: durante la visita di base, un coordinatore otterrà il consenso dal partecipante e gli verrà chiesto di compilare questionari che chiedono informazioni su fattori clinici e demografici, cronotipo e attività fisica e verranno prese varie misurazioni antropometriche. Dopo la visita di riferimento, i partecipanti saranno sottoposti a un periodo di rodaggio di 2 settimane. Durante questo periodo, l'obiettivo è registrare i tempi abituali delle donne in cui si mangiano e l'appetito mentre seguono la loro dieta abituale.
Visita 2: Le donne forniranno campioni di sangue e feci a digiuno e verranno effettuate misurazioni antropometriche. Dopo la visita 2, i partecipanti saranno sottoposti al programma di controllo o TRE per 4 settimane.
Visita 3: dopo 4 settimane, i partecipanti saranno sottoposti alla visita 3 e verrà chiesto di fornire campioni di sangue e feci a digiuno e verranno prese misure antropometriche. Dopo la visita 3, i partecipanti subiranno un periodo di washout di 4 settimane, durante il quale le donne potranno tornare al loro solito stile di vita.
Visita 4: La visita 4 si svolgerà dopo il periodo di sospensione di 4 settimane e includerà una consultazione telefonica per discutere la fase successiva dello studio e programmare la fornitura dei pasti. Per le successive 4 settimane, i partecipanti saranno quindi sottoposti al programma di alimentazione opposto (controllo o TRE).
Visita 5: Durante la visita finale, le donne forniranno campioni di sangue e feci a digiuno e verranno prese misure antropometriche.
Durante il periodo di run-in, la fase TRE e la fase di controllo, i partecipanti registreranno i tempi degli episodi alimentari utilizzando l'app del telefono MyCircadian Clock, completeranno 3 richiami dietetici di 24 ore, un singolo richiamo di attività fisica e 7 giorni di sonno e l'attività fisica sarà oggettivamente misurata utilizzando un dispositivo Actiwatch. La fame e la sazietà saranno valutate in modo casuale tramite l'app per telefono MyCircadian Clock.
Verrà condotto un colloquio di uscita dallo studio tramite teleconferenza Zoom per fare il punto sull'intervento e ottenere informazioni dettagliate sull'accettabilità del protocollo. Circa 6 mesi dopo la fine del periodo di intervento, le donne completeranno un questionario di follow-up inviato per posta.

- Femmine native delle Hawaii / delle isole del Pacifico di età pari o superiore a 18 anni
- Sovrappeso o obeso (BMI>25 kg/m2) O con una storia di diabete non insulino-dipendente O con una storia di iperplasia endometriale atipica complessa
- Avere un cellulare funzionante in grado di scaricare un'app
- In grado di utilizzare il cellulare durante il giorno (ad esempio al lavoro)
- Non un lavoratore notturno

Sperimentale: Time Restricted Eating (TRE) Schedule, Then Control
In this arm of the 14-week randomized dietary crossover study, participants will undergo a 2-week run-in period, followed by an 8-hour TRE protocol (comprising 8 hours of feeding and 16 hours of fasting periods), a 4-week washout period, and a 4-week control diet schedule.

Women will receive prepared frozen lunch and dinner meals as per the control schedule, but will be asked to consume daily meals, snacks, and calorie-containing beverages within an 8 to 10-hour period that fits their schedule.
They will be asked to maintain their regular sleep and physical activity schedule.
Fasting period will ramp up during the first week (e.g., Days 1-3, 12-14 h per day, Days 4-6, 14-16 h per day, Days 7+, 16 h per day).
Women will be asked to schedule the feeding period at the same time each day.
They will be allowed to consume water and 1-2 servings of very low-calorie drinks and foods (e.g., herbal tea, diet drinks, mints, gum containing < 4 kcal/serving) to increase compliance.
All meals (control and TRE) will be provided by the Metabolic Kitchen developed in collaboration with the department of Nutrition and Integrative Physiology.
Women will receive frozen lunch/dinner meals and a standardized breakfast and snacks menu.
All meals and the breakfast/snack menu will be designed to be culturally competent.
The meal plans will be individualized to meet weight maintenance energy requirements.

TRE adherence is achieved when participants fast for 14-18 hours per day during the TRE phase, as measured by a mealtime log using the MyCircadianClock phone app.
