Detaillierte Beschreibung
Einschreibung: Das Studium umfasst zwei Phasen. Phase 1 umfasst Fokusgruppen unter 30 hawaiianischen/pazifischen Inselbewohnerinnen mit einem Risiko für Gebärmutterkrebs. Fokusgruppen werden soziokulturelle Einflüsse auf (i) das Ernährungsverhalten, einschließlich des Zeitpunkts der Mahlzeiten; (ii) Ernährungspräferenzen; (iii) vorgeschlagene Essenspläne und Interventionsdesign; (iv) Wahrnehmung von Gewichtsverlust und Körperbild. In der zweiten Studienphase wird das Studienteam 10 pazifische Inselbewohnerinnen für die Teilnahme an einer 14-wöchigen randomisierten Crossover-Ernährungsstudie rekrutieren. Jede Frau nimmt an beiden Interventionsarmen teil und fungiert als ihre eigene Kontrolle.
Fokusgruppe: Um alle Aspekte von Ziel 1 dieser Studie zu erreichen, wird das Studienteam 30 pazifische Inselbewohnerinnen mit einem Risiko für Gebärmutterkrebs rekrutieren, um virtuelle Fokusgruppen durchzuführen. Diese Fokusgruppen finden über private Videokonferenzen wie Zoom statt und bei Bedarf wird ein Sprachübersetzer zur Verfügung gestellt. An den Fokusgruppen wird ein Experte der hawaiianischen/pazifischen Inselbewohnergemeinschaft teilnehmen. Die Fragen sollen dazu dienen, soziokulturelle Einflüsse auf das Ernährungsverhalten zu verstehen (Fragen zu sozialem Essen, Essenszeiten und Ernährungspräferenzen, Einholen von Feedback zum Design der Studienintervention sowie zur Wahrnehmung von Gewichtsverlust und Körperbild). Vor der formellen Befragung wird ein Eisbrecher durchgeführt. Die Sitzungen werden auf der Videokonferenz und einem zusätzlichen Gerät aufgezeichnet, wörtlich transkribiert und auf Richtigkeit überprüft. Zwei weitere Teammitglieder werden als Protokollanten anwesend sein. Unser Community-Experte wird anwesend sein, um die Sitzung zu moderieren. Zu Beginn der Sitzung werden Fragen gestellt, um die kulturelle Sensibilität sicherzustellen.
Ernährungsintervention: Die Ernährungsintervention umfasst 5 Studienbesuche und umfasst einen Basisbesuch, eine 2-wöchige Einlaufphase, einen 4-wöchigen Kontrolldiätplan, eine 4-wöchige Auswaschphase und eine 4-wöchige Zeit von 8 bis 10 Stunden TRE-Protokoll (Restricted Eating) (8 bis 10 Stunden Fütterung und 14 bis 16 Stunden Fastenperioden). Ein Folgefragebogen wird 6 Monate nach der Intervention verschickt. Für den Kontrollplan erhalten Frauen gefrorene Mittag- und Abendessen sowie ein standardisiertes Frühstücks- und Snackmenü. Alle Mahlzeiten und das Frühstücks-/Snackmenü werden mit dem Feedback der Fokusgruppen so gestaltet, dass sie kulturell kompetent sind, und die Essenspläne werden individuell angepasst, um den Energiebedarf zur Gewichtserhaltung zu decken. Für den TRE-Plan erhalten Frauen gemäß dem Kontrollplan zubereitete gefrorene Mittag- und Abendessen, werden jedoch gebeten, innerhalb eines zu ihrem Zeitplan passenden Zeitraums von 8 bis 10 Stunden täglich Mahlzeiten, Snacks und kalorienhaltige Getränke zu sich zu nehmen. Sie werden gebeten, ihren regelmäßigen Schlaf- und Bewegungsplan einzuhalten. Die Fastenzeit nimmt in der ersten Woche zu (Tage 1–3, 12–14 Stunden pro Tag, Tage 4–6, 14–16 Stunden pro Tag, Tage 7+, 16 Stunden pro Tag). Frauen werden gebeten, die Essenszeit jeden Tag zur gleichen Zeit einzuplanen. Alle Mahlzeiten (Kontrolle und TRE) werden von der Metabolic Kitchen an der University of Utah bereitgestellt.
Besuch 1: Während des Basisbesuchs holt ein Koordinator die Zustimmung des Teilnehmers ein und er wird gebeten, Fragebögen auszufüllen, in denen er nach klinischen und demografischen Faktoren, Chronotyp und körperlicher Aktivität fragt, und es werden verschiedene anthropometrische Messungen durchgeführt. Nach dem Basisbesuch durchlaufen die Teilnehmer eine zweiwöchige Einlaufphase. Während dieser Zeit besteht das Ziel darin, die üblichen Essattacken und den Appetit der Frauen zu erfassen und gleichzeitig ihre gewohnte Ernährung einzuhalten.
Besuch 2: Frauen stellen Nüchternblut- und Stuhlproben zur Verfügung und es werden anthropometrische Messungen durchgeführt. Nach Besuch 2 durchlaufen die Teilnehmer 4 Wochen lang den Kontroll- oder TRE-Plan.
Besuch 3: Nach 4 Wochen durchlaufen die Teilnehmer Besuch 3 und werden gebeten, Nüchternblut- und Stuhlproben abzugeben, und es werden anthropometrische Messungen durchgeführt. Nach Besuch 3 durchlaufen die Teilnehmer eine vierwöchige Auswaschphase, in der die Frauen zu ihrem gewohnten Lebensstil zurückkehren können.
Besuch 4: Besuch 4 findet nach der 4-wöchigen Auswaschphase statt und beinhaltet eine telefonische Beratung, um die nächste Studienphase zu besprechen und die Essensversorgung zu planen. In den folgenden 4 Wochen erhalten die Teilnehmer dann den entgegengesetzten Fütterungsplan (entweder Kontrolle oder TRE).
Besuch 5: Beim letzten Besuch werden den Frauen Nüchternblut- und Stuhlproben entnommen und es werden anthropometrische Messungen durchgeführt.
Während der Einlaufphase, der TRE-Phase und der Kontrollphase zeichnen die Teilnehmer den Zeitpunkt der Essepisoden mit der MyCircadian Clock-Telefon-App auf, führen 3 x 24-Stunden-Diätrückrufe, einen einzelnen Rückruf zu körperlicher Aktivität und 7 Tage Schlaf durch und körperliche Aktivität wird objektiv mit einem Actiwatch-Gerät gemessen. Hunger und Sättigung werden nach dem Zufallsprinzip über die MyCircadian Clock-Telefon-App bewertet.
Über eine Zoom-Telefonkonferenz wird ein Interview zum Abschluss der Studie geführt, um die Intervention nachzubesprechen und detaillierte Informationen über die Akzeptanz des Protokolls zu erhalten. Ungefähr 6 Monate nach dem Ende des Interventionszeitraums füllen die Frauen einen per Post versandten Folgefragebogen aus.

- Einheimische hawaiianische/pazifische Inselbewohnerinnen im Alter von 18 Jahren oder älter
- Übergewichtig oder fettleibig (BMI > 25 kg/m2) ODER eine Vorgeschichte von nicht-insulinabhängigem Diabetes ODER eine Vorgeschichte mit komplexer atypischer Endometriumhyperplasie
- Besitzen Sie ein funktionierendes Mobiltelefon, das eine App herunterladen kann
- Kann das Mobiltelefon tagsüber nutzen (z. B. bei der Arbeit)
- Kein Nachtschichtarbeiter

Experimental: Time Restricted Eating (TRE) Schedule, Then Control
In this arm of the 14-week randomized dietary crossover study, participants will undergo a 2-week run-in period, followed by an 8-hour TRE protocol (comprising 8 hours of feeding and 16 hours of fasting periods), a 4-week washout period, and a 4-week control diet schedule.

Women will receive prepared frozen lunch and dinner meals as per the control schedule, but will be asked to consume daily meals, snacks, and calorie-containing beverages within an 8 to 10-hour period that fits their schedule.
They will be asked to maintain their regular sleep and physical activity schedule.
Fasting period will ramp up during the first week (e.g., Days 1-3, 12-14 h per day, Days 4-6, 14-16 h per day, Days 7+, 16 h per day).
Women will be asked to schedule the feeding period at the same time each day.
They will be allowed to consume water and 1-2 servings of very low-calorie drinks and foods (e.g., herbal tea, diet drinks, mints, gum containing < 4 kcal/serving) to increase compliance.
All meals (control and TRE) will be provided by the Metabolic Kitchen developed in collaboration with the department of Nutrition and Integrative Physiology.
Women will receive frozen lunch/dinner meals and a standardized breakfast and snacks menu.
All meals and the breakfast/snack menu will be designed to be culturally competent.
The meal plans will be individualized to meet weight maintenance energy requirements.
