Tiaminintervensjon og kognisjon hos eldre voksne som gjennomgår koronararteriebypasstransplantasjon - en randomisert klinisk studie.
Hensikten med denne studien er å få en bedre forståelse av sammenhengen mellom hjerneforandringer og kognitive underskudd hos koronar hjertesykdom (CHD) pasienter som gjennomgår koronar bypass grafting (CABG) og om en rimelig tiaminintervensjon kan brukes til å redusere posten. -CABG kognitive problemer i CHD fag.

Detaljert beskrivelse
Ved å bruke en to-gruppe, dobbeltblind randomisert, longitudinell studiedesign, vil 52 koronar hjertesykdom (CHD) pasienter som gjennomgår koronar bypass grafting (CABG) (alder 60-80 år; 26 tiaminbehandling og 26 placebo) delta i kognitiv vurdering og evaluering av nivåer av tiamin, laktat og inflammatoriske markører i blodet.
Vi foreslår at tiamininfusjon vil bidra til å redusere laktat- og inflammatoriske markørnivåer, som observert ved andre tilstander.
Funnene fra denne studien kan tjene som en ny og innovativ behandlingsstrategi for beskyttelse mot sviktende kognisjon, og dermed bedre resultater, og forbedret livskvalitet og daglige aktiviteter.
Denne kliniske studien vil gi nødvendige data om fordelene ved en rimelig tiaminintervensjon som kan implementeres i en storskala klinisk studie for å redusere post-CABG kognitive underskudd i eldre CHD, og dermed redusere tidlig demens, forbedre sosial funksjon , øke livskvaliteten og daglige aktiviteter, og redusere helsekostnader i denne alvorlige eldre CHD-pasientpopulasjonen.

Inklusjonskriterier:
- Pasienter med koronar hjertesykdom (CHD) planlagt for bypass grafting (CABG)
- Tiaminmangel før CABG
- Europeisk system for hjerteoperativ risikovurdering II (EuroSCORE II) >1,5 %
- Off-pump kirurgi
Ekskluderingskriterier:
- Demens ved baseline [Montreal Cognitive Assessment (MoCA) <21 innen 5 dager før CABG]
- Nåværende inntak av tiamin
- Kjent tiaminallergi
- Ukontrollerte blodsukkernivåer
- Kan ikke gi samtykke på grunn av sykdom
- Historie med hyperlaktatemi
- Nylige (innen flere år og/eller opp til PI/co-PI-enes vurdering) cerebrale hendelser (anfall eller hodetraume som resulterer i bevissthetstap og/eller hjernerystelse)
