Thiamine-interventie en cognitie bij oudere volwassenen die coronaire bypass-transplantatie ondergaan - een gerandomiseerde klinische studie.
Het doel van deze studie is om een beter inzicht te krijgen in de associatie tussen hersenveranderingen en cognitieve stoornissen bij patiënten met coronaire hartziekten die een coronaire bypass-transplantatie (CABG) ondergaan, en of een goedkope thiamine-interventie kan worden gebruikt om postoperatieve transplantaties te verminderen. -CABG cognitieve problemen bij CHD-patiënten.

Met behulp van een dubbelblind, gerandomiseerd, longitudinaal onderzoeksontwerp met twee groepen zullen 52 patiënten met coronaire hartziekte (CHD) die coronaire bypass-transplantatie (CABG) ondergaan (leeftijd 60-80 jaar; 26 thiaminebehandeling en 26 placebo) deelnemen aan cognitieve beoordeling en evaluatie van bloedthiamine-, lactaat- en ontstekingsmarkerniveaus.
Wij stellen voor dat thiamine-infusie zal helpen bij het verminderen van lactaat- en ontstekingsmarkerniveaus, zoals waargenomen bij andere aandoeningen.
De bevindingen uit deze studie kunnen dienen als een nieuwe en innovatieve behandelstrategie voor bescherming tegen afnemende cognitie, en dus betere resultaten, en verbeterde kwaliteit van leven en dagelijkse activiteiten.
Deze klinische proefstudie zal de vereiste gegevens opleveren over de voordelen van een goedkope thiamine-interventie die zou kunnen worden geïmplementeerd in een grootschalige klinische proef om post-CABG cognitieve tekorten bij oudere CHZ te verminderen en zo vroege dementie te verminderen en de sociale functie te verbeteren. , verhoog de kwaliteit van leven en dagelijkse activiteiten, en verlaag de gezondheidszorgkosten bij deze ernstige oudere CHD-patiëntenpopulatie.

Inclusiecriteria:
- Patiënten met coronaire hartziekte (CHD) die een bypasstransplantatie (CABG) ondergaan
- Thiaminetekort vóór CABG
- Europees systeem voor cardiale operatieve risico-evaluatie II (EuroSCORE II) >1,5%
- Chirurgie buiten de pomp
Uitsluitingscriteria:
- Dementie bij baseline [Montreal Cognitive Assessment (MoCA) <21 binnen 5 dagen vóór CABG]
- Huidige inname van thiamine
- Bekende thiamine-allergie
- Ongecontroleerde bloedglucosewaarden
- Wegens ziekte kan geen toestemming worden gegeven
- Geschiedenis van hyperlactatemie
- Recente (binnen enkele jaren en/of tot aan het oordeel van de PI/co-PI’s) hersenincidenten (aanval of hoofdtrauma resulterend in bewustzijnsverlies en/of hersenschudding)
- Hartinfarct
- Gediagnosticeerde psychiatrische ziekten (klinische depressie, schizofrenie, manische depressie)
- Patiënten met een voorgeschiedenis van alcohol- of middelenmisbruik
- Acute of chronische infecties (tuberculose, hepatitis of encefalopathie)
- Gediagnosticeerde neurodegeneratieve ziekten (de ziekte van Alzheimer of Parkinson)
- Chronische immunodeficiëntie (inclusief HIV)
- Ook aangeboren hersenafwijkingen worden uitgesloten

De SF-36 is een formulier met 36 vragen dat door de deelnemer wordt ingevuld en door de onderzoeker wordt gescoord.
De score varieert van 0 tot 100, waarbij hogere scores een betere gezondheidsstatus aangeven.

Wij zullen er alles aan doen om gegevens tijdig te publiceren.
Zodra de bevindingen met betrekking tot veranderingen in lactaat-, thiamine- en ontstekingsniveaus in het bloed, gebaseerd op bloedpathologie- en cognitiebeoordelingsgegevens voor en na thiamine-interventie bij CHD-patiënten voor en na CABG, zijn gepubliceerd of tegen het einde van de prestatieperiode, afhankelijk van wat zich het eerst voordoet De bloedspiegels, cognitieve gegevens, demografische gegevens, stemming en klinische gegevens (zonder individuele identificatiegegevens) zullen worden gedeponeerd in een door de NIH ondersteunde domeinspecifieke gegevensuitwisselingsrepository (NIMH Data Archive).
Alle gegevens worden gelabeld met consistente, unieke identificatiegegevens, waardoor ze gemakkelijk vindbaar zijn in de gegevensopslagplaats voor geïnteresseerde onderzoekers.
Gegevens zullen voor altijd beschikbaar zijn nadat ze met andere onderzoekers zijn gedeeld met de gegevensopslagplaats voor secundaire analyses.
