CAR-T-terapi er en relativt ny og innovativ behandlingsstrategi for å håndtere refraktære hematologiske kreftformer, inkludert noen typer leukemi, lymfom og myelomatose (Efficace, 2022). Denne sentrale behandlingen har blitt standardbehandling for visse kreftformer, som tilbakevendende non-Hodgkins lymfom, tilbakevendende eller refraktær akutt lymfatisk leukemi og tilbakevendende refraktær myelomatose, med noen studier som viser at minst 80 % av pasientene har positiv respons på behandlingen (June, 2018; Landry, 2020). Selv om CAR T-celleterapi har ført til betydelige fremskritt i behandlingen av hematologiske kreftformer, fører den også med seg en rekke unike bivirkninger. De to vanligste akutte bivirkningene er henholdsvis cytokinutløsningssyndrom (CRS) og nevrologisk toksisitet kjent som immun effektor celle-assosiert nevrotoksisitetssyndrom (ICANS) (Neelapu, 2017).
Nylige studier antyder at det bør legges vekt på prehabilitering kort tid etter oppstart av utredningsprosessen for CAR-T-terapi, opprettholdelse av funksjon under aktiv behandling og tett oppfølging med rehabilitering spesielt hvis nevrologiske følgetilstander vedvarer (Obaisi, 2022; Kiefer, 2023). Effektive tiltak er presserende nødvendige for å forbedre livskvalitet og funksjonsevne hos pasienter med hematologisk kreft som gjennomgår CAR-T-terapi (Cenik, 2020; Jeevanantham, 2021). Fysisk aktivitetsprogrammer har vist seg å påvirke psykologiske symptomer, tretthet og livskvalitet (Vermaete, 2013, 2014). Avgjørende er det at det til dags dato ikke foreligger bevis for pasienter med hematologiske kreftformer som gjennomgår CAR-T-terapi. Derfor er studier som utforsker tilnærminger for å utvide tilgangen til multimodal kreftrehabilitering hos disse pasientene nødvendige for å møte behovene til den raskt voksende populasjonen av kreftoverlevere som mottar CAR-T-terapi. Nylige systematiske oversikter har vist høyt potensial for telehelsetilnærminger for å legge til rette for kreftrehabilitering (Brick, 2022; Chang, 2022).
Hjemmeautomatisert telehåndtering (HAT). HAT-systemet er en telemedisinplattform som støtter pasienter i å følge individuelle egenomsorgsplaner og assisterer helsepersonell i behandling og overvåking av sine pasienter i henhold til evidensbaserte retningslinjer (Finkelstein, 2012; Fleg, 2020; Shero, 2022). HAT er basert på kronisk sykdomsomsorgsmodell (Grover, 2014) og gir støtte til pasientselvstyre, omfattende pasient-behandler kommunikasjon og tverrfaglig omsorgskoordinering (Drouin, 2015). Hovedmålet med HAT-systemet er å minimere byrden ved kronisk sykdomshåndtering og å forenkle overvåking i ambulant omsorg. For å oppnå dette målet kreves det at HAT-systemet (1) støtter en konstant informasjonsretursløyfe mellom pasient og helsepersonell; (2) tar over alle rutinemessige gjentakende oppgaver; og (3) gir sanntids klinisk beslutningsstøtte både for pasient og kliniker. HAT har blitt vellykket implementert og testet i en rekke kroniske sykdommer.
Pasientenheter brukes i pasientens hjem for å veilede pasienter i å følge sine behandlingsplaner inkludert øvelses- og kostholdsresept, overvåke pasientenes fremgang og symptomer, og for å kommunisere med helsepersonell. Enhver nettbasert enhet kan tjene som pasientenhet inkludert berøringsskjermtabletter, VR-briller og smarttelefoner. Hjemmeenhetsgrensesnittet har gjennomgått omfattende brukervennlighetstesting (Jeong, 2015; Finkelstein, 2021; Gabriel, 2023) og er svært enkelt å betjene, selv for dem uten tidligere datarerfaring. Kliniker-enhetene (enhver nettbasert enhet) brukes til å sette opp og oppdatere individuelle behandlingsplaner, analysere pasientens selvtesteresultater og gjennomgå datagenererte varsler. HAT-serveren implementerer datastøttet beslutningsstøtte basert på individuell varslingsinnstilling og sanntidsovervåking av pasientens selvtestedata. I denne studien vil HAT-systemet fokusere på levering av et multimodal kreftrehabiliteringsprogram som omfatter individuell terapeutisk fysisk trening, ernæringsplan, atferdsrådgivning og tilpasset utdanningsstyrking rettet mot de sosiale, kognitive, reproduktive og yrkesmessige behovene til blodkreftoverlevere.
Den foreslåtte multmodale telerehabiliteringsmodellen lar et rehabiliteringsterapi-team sette opp individuelle rehabiliteringsplaner ved hjelp av en nettbasert omsorgshåndteringsportal og overvåke pasientens fremgang på nett. Pasienter hjemme følger en trygg og effektiv personlig trenings- og ernæringsplan veiledet av interaktiv berøringsskjermteknologi kombinert med atferdsrådgivning, sosial støtte, og interaktiv utdanning og styrking. Designet av telerehabiliteringssystemet er basert på skybasert Internet-of-Things arkitektur som tillater sanntidsovervåking av kardiovaskulære parametre og treningsytelse. Pasientens anstrengelsesnivå under trening identifiseres automatisk av en validert AI-drevet algoritme som støtter treningssikkerhet og effektivitet. Det endelige målet med dette pilotgjennomførbarhetsprosjektet er å fastslå omfanget av påvirkningen fra den foreslåtte pasientsentrerte krefttelerehabiliteringsmodellen på sykdomsspesifikk livskvalitet, og funksjons- og symptomutfall, og å skaffe tilstrekkelig bevis for en definitiv randomisert klinisk studie som evaluerer denne tilnærmingen i en multicentersstudie.
Denne studien vil fastslå om pasienter med hematologiske kreftformer som gjennomgår CAR-T-terapi vil vise tilstrekkelig aksept og overholdelse av den multmodale krefttelerehabiliteringen, og om dette vil være assosiert med en positiv utvikling i livskvalitet, fysisk form og pasientrapporterte utfall. Denne studien har en utforskende sekvensiell blandet metodedesign for å vurdere gjennomførbarheten av en pasientsentrert, kulturelt tilpasset telemedisintilnærming for multimodal kreftrehabilitering hos pasienter med hematologiske kreftformer som gjennomgår CAR-T-terapi.
Kvalifiserte pasienter vil gjennomgå en fireukers multimodal prehabilitering før oppstart av CAR-T-terapien og deretter motta kontinuerlig nettbasert styrking og utdanningsstøtte under CAR-T-terapi mens de gjennomgår kjemoterapi, CAR-T-infusjon, toksisitetsovervåking og rekonvalesens i opptil fire uker, avhengig av spesifikk CAR-T-behandlingsregime. Dette vil etterfølges av en tolvukers multimodal telerehabilitering etter behandlingsfullføring og utskrivelse fra sykehus. En omfattende vurdering vil bli gjennomført ved studiestart, og pasientene vil individuelt få foreskrevet styrke-, kondisjons- og avspenningsøvelser, og en ernæringsplan for 4-ukers prehabilitering basert på pasientens fysiske form og ernæringstilstand. En mobilapp vil gi interaktiv utdanning om kreftoverlevelse, tidlig mobilisering og egenomsorg etter utskrivelse under CAR-T-behandling som omfatter poliklinisk kjemoterapi, CAR-T-infusjon og innleggelse for toksisitetsovervåking. Etter sykehusutskrivelse vil en 12-ukers multimodal rehabiliteringsplan bli levert via telerehabiliteringssystemet som bruker validert AI-drevet sanntidsvurdering av treningsanstrengelse (Smiley, 2024). Sosial støtte vil leveres via gruppetelekonsultasjoner. Automatisert analyse av daglige trenings- og kostholdslogger vil varsle telerehabiliteringsteamet om lav pasientoverholdelse og utløse tilpasset tilbakemelding fra telerehabiliteringsteamet. Aksept, brukervennlighet, gjennomførbarhet og overholdelse vil bli vurdert ved studiens slutt som beskrevet i analyseplanen. Til slutt vil denne studien vurdere livskvalitet, fysisk form, kroppssammensetning, kreftsymptomer, atferdskarakteristikker og tilfredshet med omsorg før og etter den multmodale rehabiliteringen levert via en telemedisinplattform.

- kommersiell FDA-godkjent CAR-T-behandling planlagt i ≥4 uker etter inkludering
- Eastern Cooperative Oncology Group (ECOG) ytelsestatus 0-2.
- Tilgang til en fungerende telefonlinje i hjemmet eller en mobiltelefon.
Eksklusjonskriterier:
- har ustabil angina, ukontrollert hypertensjon, nylig hjerteinfarkt, pacemaker, smertefulle eller ustabile knokkelmetastaser, eller nylige skjelettfrakturer;
- deltar i et regulært treningsrehabiliteringsprogram;
- har planer om flytting i løpet av de neste 6 månedene;
- deltar i en annen klinisk studie.

Eksperimentell: Multimodal krefttelerehabilitering
Hver inkludert pasient vil gjennomgå en fire ukers multimodal prehabilitering før start av CAR-T-terapien. Under CAR-T-terapien vil pasientene bruke en interaktiv app som støtter kreftomsorgsutdanning og tidlig mobilisering mens de gjennomgår kjemoterapi, infusjon, in-hospital toksisitetsmonitorering og rekonvalesens (varigheten avhenger av det spesifikke CAR-T-behandlingsregimet). Etter fullføring av CAR-T-prosedyrene og utskrivelse fra sykehuset, vil pasientene gjennomgå en 12 ukers hjemmebasert multimodal telerehabilitering. Individualiserte rehabiliteringsplaner ved bruk av en nettbasert omsorgsledelsesportal. Pasienter hjemme følger en trygg og effektiv personlig tilpasset trenings- og ernæringsplan veiledet av interaktiv berøringsskjermteknologi kombinert med atferdsrådgivning, sosial støtte og interaktiv utdanning og styrking. |
HAT (Home Automated Telemanagement) pasientenhet vil veilede pasienter i å følge behandlingsplanene sine, inkludert øvelser og diettoppskrifter, overvåke pasientenes fremgang og symptomer, og kommunikasjon med helsepersonell. Deltakere vil motta en internettaktivert enhet, enten en berøringsskjermplate eller et virtual reality (VR)-headset, som en del av pasientenheten, basert på deres personlige preferanse.
