Multimodale telerevalidatie bij patiënten die CAR-T-celimmunotherapie ondergaan

CAR-T-therapie is een relatief nieuwe, innovatieve behandelstrategie voor het beheersen van refractaire hematologische kankers, waaronder sommige vormen van leukemie, lymfoom en multipel myeloom (Efficace, 2022). Deze cruciale behandeling is de standaardzorg geworden voor bepaalde maligniteiten, zoals recidiverend non-Hodgkin-lymfoom, recidiverende of refractaire acute lymfatische leukemie en recidiverend refractair multipel myeloom, waarbij sommige studies aantonen dat minstens 80% van de patiënten positief reageert op de behandeling (June, 2018; Landry, 2020). Hoewel CAR T-celtherapie heeft geleid tot aanzienlijke vooruitgang in de behandeling van hematologische maligniteiten, brengt het ook een reeks unieke toxiciteiten met zich mee. De twee meest voorkomende acute bijwerkingen zijn respectievelijk het cytokinereleasesyndroom (CRS) en neurologische toxiciteit, bekend als het immuuneffectorcel-geassocieerde neurotoxiciteitssyndroom (ICANS) (Neelapu, 2017).
Recente studies suggereren dat er nadruk moet liggen op prehabilitatie kort na het starten van het voorbereidingsproces voor CAR-T-therapie, het behouden van functie tijdens actieve behandeling en nauwe follow-up met revalidatie, vooral als neurologische gevolgen aanhouden (Obaisi, 2022; Kiefer, 2023). Effectieve interventies zijn dringend nodig om de kwaliteit van leven en functionele capaciteit te verbeteren bij patiënten met hematologische kankers die CAR-T-therapie ondergaan (Cenik, 2020; Jeevanantham, 2021). Bewegingsprogramma's zijn aangetoond van invloed te zijn op psychologische symptomen, vermoeidheid en kwaliteit van leven (Vermaete, 2013, 2014). Cruciaal is dat er tot op heden geen bewijs is gepresenteerd voor patiënten met hematologische maligniteiten die CAR-T-therapie ondergaan. Daarom zijn studies die benaderingen onderzoeken om de toegang tot multimodale kankerrevalidatiepraktijken bij deze patiënten uit te breiden, noodzakelijk om in de behoeften te voorzien van de snel groeiende populatie kankergenezenen die CAR-T-therapie krijgen. Recente systematische reviews toonden het hoge potentieel van telezorgbenaderingen aan om kankerrevalidatie te vergemakkelijken (Brick, 2022; Chang, 2022).
Home Automated Telemanagement (HAT). Het HAT-systeem is een telemedicineplatform dat patiënten ondersteunt bij het volgen van geïndividualiseerde zelfzorgplannen en zorgverleners helpt bij de behandeling en monitoring van hun patiënten volgens evidence-based richtlijnen (Finkelstein, 2012; Fleg, 2020; Shero, 2022). HAT is gebaseerd op het chronische ziektemodel (Grover, 2014) en biedt ondersteuning voor patiëntzelfmanagement, uitgebreide communicatie tussen patiënt en zorgverlener, en multidisciplinaire zorgcoördinatie (Drouin, 2015). Het hoofddoel van het HAT-systeem is om de last van chronische ziektebeheersing te minimaliseren en monitoring in de ambulante zorg te vereenvoudigen. Om dit doel te bereiken, is het HAT-systeem vereist (1) om een constante feedbacklus van informatie tussen de patiënt en zorgverleners te ondersteunen; (2) om alle routinematige repetitieve taken over te nemen; en (3) om real-time klinische beslissingsondersteuning te bieden voor zowel de patiënt als de clinicus. HAT is succesvol geïmplementeerd en getest in een verscheidenheid aan chronische ziekten.
Patiëntunits worden gebruikt in patiëntwoningen om patiënten te begeleiden bij het volgen van hun behandelplannen, inclusief oefen- en dieetvoorschriften, om de voortgang en symptomen van patiënten te monitoren, en om te communiceren met zorgverleners. Elk webgeschikt apparaat kan dienen als patiëntunit, waaronder touchscreen-tablets, VR-headsets en smartphones. De interface van de thuisunit onderging rigoureuze bruikbaarheidstesten (Jeong, 2015; Finkelstein, 2021; Gabriel, 2023) en is zeer eenvoudig te bedienen, zelfs door mensen zonder enige eerdere computerervaring. De clinicusunits (elk webgeschikt apparaat) worden gebruikt om geïndividualiseerde behandelplannen op te zetten en bij te werken, patiëntzelftestresultaten te analyseren en door de computer gegenereerde waarschuwingen te beoordelen. De HAT-server implementeert geautomatiseerde beslissingsondersteuning op basis van geïndividualiseerde waarschuwingsinstellingen en realtime monitoring van patiëntzelftestgegevens. In deze studie zal het HAT-systeem zich richten op het leveren van een multimodaal kankerrevalidatieprogramma bestaande uit geïndividualiseerde therapeutische fysieke oefeningen, voedingsplan, gedragstherapie en op maat gemaakte educatieve empowerment gericht op de sociale, cognitieve, reproductieve en beroepsmatige behoeften van bloedkankergenezenen.
Het voorgestelde multimodale telerevalidatiemodel stelt een revalidatietherapieteam in staat om geïndividualiseerde revalidatieplannen op te zetten via een webgebaseerd zorgbeheerportaal en de patiëntvoortgang online te monitoren. Patiënten thuis volgen een veilig en effectief gepersonaliseerd oefen- en voedingsplan begeleid door interactieve touchscreen-technologie gecombineerd met gedragstherapie, sociale ondersteuning, en interactieve educatie en empowerment. Het ontwerp van het telerevalidatiesysteem is gebaseerd op de cloudgebaseerde Internet-of-Things-architectuur die real-time monitoring van cardiovasculaire parameters en oefenprestaties mogelijk maakt. Het inspanningsniveau van de patiënt tijdens het sporten wordt automatisch geïdentificeerd door een gevalideerd AI-gestuurd algoritme dat de veiligheid en effectiviteit van de oefeningen ondersteunt. Het uiteindelijke doel van dit pilot haalbaarheidsproject is om de mate van impact vast te stellen van het voorgestelde patiëntgerichte kanker telerevalidatiemodel op ziektegerelateerde kwaliteit van leven, en functionele en symptoomuitkomsten en voldoende bewijs te verkrijgen voor een definitieve gerandomiseerde klinische studie die deze aanpak in een multicenterstudie evalueert.
Deze studie zal bepalen of patiënten met hematologische maligniteiten die CAR-T-therapie ondergaan voldoende acceptatie en therapietrouw aan de multimodale kanker telerevalidatie zullen tonen, en of dit geassocieerd zal zijn met een positief traject van kwaliteit van leven, fitheid en door patiënten gerapporteerde uitkomsten. Deze trial is een exploratief sequentieel mixed methods ontwerp om de haalbaarheid te beoordelen van een patiëntgerichte, cultureel aangepaste telemedicinebenadering voor multimodale kankerrevalidatie bij patiënten met hematologische maligniteiten die CAR-T-therapie ondergaan.
In aanmerking komende patiënten ondergaan een vier weken durende multimodale prehabilitatie vóór de start van de CAR-T-therapie en ontvangen vervolgens continue online empowerment en educatieondersteuning tijdens CAR-T-therapie terwijl ze chemotherapie, CAR-T-infusie, toxiciteitsmonitoring en herstel ondergaan voor maximaal vier weken, afhankelijk van een specifiek CAR-T-behandelingsregime. Dit wordt gevolgd door een twaalf weken durende multimodale telerevalidatie na voltooiing van de behandeling en ziekenhuisontslag. Een uitgebreide beoordeling wordt uitgevoerd aan het begin van de studie, en de patiënten krijgen individueel weerstand-, aerobe en ontspanningsoefeningen voorgeschreven, en een voedingsplan voor 4 weken prehabilitatie op basis van patiëntfitheidniveaus en voedingsstatus. Een mobiele app zal interactieve educatie bieden over kankeroverleving, vroege mobilisatie en zelfzorg na ontslag tijdens CAR-T-behandeling, bestaande uit poliklinische chemotherapie, CART-infusie en klinische opname voor toxiciteitsmonitoring. Na ziekenhuisontslag wordt een 12 weken durend multimodaal revalidatieplan aangeboden via het telerevalidatiesysteem dat gebruikmaakt van gevalideerde AI-gestuurde real-time beoordeling van oefeninspanning (Smiley, 2024). Sociale ondersteuning wordt geleverd via groepstelecounselingsessies. Geautomatiseerde analyse van dagelijkse oefen- en voedingslogboeken zal het telerevalidatieteam waarschuwen over lage therapietrouw van patiënten en gepersonaliseerde feedback door het telerevalidatieteam stimuleren. De acceptatie, bruikbaarheid, haalbaarheid en therapietrouw worden aan het einde van de studie beoordeeld zoals beschreven in het Analyseplan. Ten slotte zal deze studie kwaliteit van leven, fitheid, lichaamssamenstelling, kankersymptomen, gedragskenmerken en tevredenheid met zorg beoordelen voor en na de multimodale revalidatie geleverd via een telemedicineplatform.

Experimenteel: Multimodale kanker telerevalidatie
Elke ingeschreven patiënt ondergaat een vier weken durende multimodale prehabilitatie voorafgaand aan de start van de CAR-T-therapie. Tijdens de CAR-T-therapie gebruiken de patiënten een interactieve app die ondersteuning biedt bij kankerzelfzorgeducatie en vroege mobilisatie, terwijl ze chemotherapie, infusie, ziekenhuisbewaking voor toxiciteit en herstel ondergaan (de duur hangt af van het specifieke CAR-T-behandelingsregime). Na voltooiing van de CAR-T-procedures en ontslag uit het ziekenhuis ondergaan de patiënten een 12 weken durende thuisgebaseerde multimodale telerehabilitatie. Gepersonaliseerde revalidatieplannen via een webgezondheidsportaal. Patiënten thuis volgen een veilig en effectief gepersonaliseerd bewegings- en voedingsplan, begeleid door interactieve touchscreentechnologie in combinatie met gedragsbegeleiding, sociale ondersteuning en interactieve educatie en empowerment. |
De HAT (Home Automated Telemanagement) patiënteneenheid begeleidt patiënten bij het volgen van hun behandelplannen, inclusief oefen- en dieetvoorschriften, monitort de voortgang en symptomen van patiënten en faciliteert communicatie met zorgverleners. Deelnemers ontvangen een internetverbinding apparaat, zoals een aanraakscherm tablet of een virtual reality (VR) headset, als onderdeel van de patiënteneenheid, afhankelijk van hun persoonlijke voorkeur. Deelnemers krijgen ook een pulsoximeter om hun hartslag en zuurstofsaturatie te monitoren, en een fitbit om dagelijkse fysieke activiteit en stappen te meten. |

EORTC QLQ-C30 is een vragenlijst om de gezondheidsgerelateerde kwaliteit van leven bij kankerpatiënten te beoordelen. Patiënten geven aan welke symptomen/problemen ze de afgelopen week hebben ervaren, van "1 Helemaal niet" tot "4 Heel veel". De beoordeling rapporteert gegevens als 15 subschalen met scores van 0-100. Een hoge score duidt op een hoger responsniveau. Een hoge functionele schaalscore (FS) vertegenwoordigt een hoog/gezond niveau van functioneren en omvat Lichamelijk, Rol, Emotioneel, Cognitief en Sociaal functioneren. Een hoge symptoomschaalscore (SS) vertegenwoordigt een hoog niveau van symptomatologie/problemen en omvat Vermoeidheid, Misselijkheid en braken, Pijn, Kortademigheid, Slapeloosheid, Eetlustverlies, Constipatie, Diarree, Financiële problemen en Algemene gezondheidstoestand/KvL. Telerehab-gebruikershoudingen meten de algehele gevoelens van patiënten over revalidatietechnologie en diensten op afstand.
