Opis
Kryteria przyjęcia:
- Uczestnik musi być planowym lub pilnym pacjentem kardiochirurgicznym poddawanym zabiegowi krążenia pozaustrojowego w systemie opieki zdrowotnej NorthShore University.
- Mężczyzna lub kobieta w wieku od 21 do 90 lat w momencie wyrażenia zgody.
- Podmiot, który może wyrazić zgodę w języku angielskim.
- Pacjenci, którzy kwalifikują się do szybkiej ścieżki ekstubacji zgodnie z definicją tych pacjentów, którzy planują ekstubację w ciągu 24 godzin od zakończenia operacji, a optymalnie w ciągu 6-godzinnego wzorcowego czasu STS od zakończenia operacji.
Kryteria wyłączenia:
- Pacjenci poddawani pilnej operacji kardiochirurgicznej.
- Osoby, które nie mogą wyrazić zgody w języku angielskim.
- Osoby, które nie kwalifikują się do ekstubacji w ciągu 24 godzin od zakończenia operacji.
- Osoby z zaburzeniami nerwowo-mięśniowymi.
- Osoby na domowym tlenie.
- Pacjenci, u których stwierdzono alergie lub reakcje na rokuronium lub sugammadeks.
- Osoby z przewidywaną potrzebą przedłużonej intubacji przez zespół leczenia klinicznego.
- Osoby z historią nadużywania opioidów.
- Osoby korzystające z mechanicznego wspomagania krążenia.
- Osoby ze schyłkową niewydolnością nerek wymagającą dializy.

Eksperymentalny: Sugammadeks
Pojedynczy bolus dożylny (iv.) sugammadeksu w dawce 2 mg/kg (liczba drgań 2-4 z 4) lub 4 mg/kg (liczba drgań mniejsza niż 2 z 4).

Piętnaście minut po przybyciu na OIOM pacjentom zostanie podany sugammadeks (2 mg/kg przy założeniu liczby skurczów 2-4 z 4 lub 4 mg/kg przy założeniu liczby skurczów mniejszej niż 2 z 4) przez dostawcę znieczulenia.
Pięć minut po podaniu zostanie zastosowany ilościowy monitor nerwowo-mięśniowy (TetraGraph, Senzime, Uppsala, Szwecja) i zostanie zarejestrowany stosunek TOF.

Society of Thoracic Surgery (STS) Quality Benchmark of Early Extubation Criteria
Ramy czasowe: within 6 hours of end of surgery

Will compare the difference in the number of subjects who receive sugammadex and meet the STS 6-hour extubation criteria from the end of surgery vs. those that do not receive neuromuscular blockade reversal.
The Society of Thoracic Surgery defines early extubation as extubation within six hours of completion of cardiac surgery and uses this as a reportable benchmark.

The final train-of-four ratio was determined at time of extubation using a commercially available quantitative neuromuscular monitor and categorized as greater than or equal to 0.9 or less than 0.9. In order to assess the depth of neuromuscular blockade and recovery from paralyzing anesthesia drugs, a nerve is rapidly stimulated four times creating four muscle twitches. The Train-of-Four ratio is the amplitude of the fourth muscle twitch divided by the amplitude of the first twitch. A TOF Ratio of 0.9 or greater is usually indicative of adequate recovery from muscle paralysis. |

Post-Extubation Hypoxemia
Ramy czasowe: Average of 6 hour intervals over first 24 hours post-operatively
|
Post-extubation hypoxemic episodes were defined according to the Berlin criteria where PaO2/FiO2 ratios were approximated using previously validated SpO2/FiO2 ratios every 6 hours for 24 hours post-extubation.
SpO2/FiO2 < 235 corresponded to moderate to severe hypoxemia and 235-315 corresponded to mild hypoxemia.
Reported ratio values were calculated as the average of data collected (every 6 hours) during the 24-hour period post-extubation.
