Descrizione
Criterio di inclusione:
- Il soggetto deve essere un paziente cardiochirurgico elettivo o urgente sottoposto a bypass cardiopolmonare presso il NorthShore University HealthSystem.
- Soggetto maschio o femmina di età compresa tra 21 e 90 anni, al momento del consenso.
- Soggetto che può acconsentire in inglese.
- Soggetti che sono idonei per l'estubazione accelerata come definito da quei pazienti che pianificano di essere estubati entro 24 ore dalla fine dell'intervento e in modo ottimale entro il tempo di riferimento STS di 6 ore dalla fine dell'intervento.
Criteri di esclusione:
- Soggetti sottoposti a cardiochirurgia d'urgenza.
- Soggetti che non possono dare il consenso in inglese.
- Soggetti che non possono essere estubati entro 24 ore dalla fine dell'intervento chirurgico.
- Soggetti con disturbi neuromuscolari.
- Soggetti in ossigeno domiciliare.
- Soggetti che hanno conosciuto allergie o reazioni al rocuronio o al sugammadex.
- Soggetti con necessità anticipata di intubazione prolungata da parte del team di trattamento clinico.
- Soggetti con una storia di abuso di oppioidi.
- Argomenti sul supporto circolatorio meccanico.
- Soggetti con malattia renale allo stadio terminale che richiedono dialisi.

Sperimentale: Sugammadex
Singolo bolo endovenoso (IV) di sugammadex a 2 mg/kg (conta delle contrazioni 2-4 su 4) o 4 mg/kg (conta delle contrazioni inferiore a 2 su 4).

Quindici minuti dopo l'arrivo in terapia intensiva, ai soggetti verrà somministrato sugammadex (2 mg/kg ipotizzando un conteggio delle contrazioni di 2-4 di 4 o 4 mg/kg assumendo un conteggio delle contrazioni inferiore a 2 di 4) dal fornitore di anestesia.
Cinque minuti dopo la somministrazione, verrà applicato un monitor neuromuscolare quantitativo (TetraGraph, Senzime, Uppsala, Svezia) e verrà registrato il rapporto TOF.

Society of Thoracic Surgery (STS) Quality Benchmark of Early Extubation Criteria
Lasso di tempo: within 6 hours of end of surgery

Will compare the difference in the number of subjects who receive sugammadex and meet the STS 6-hour extubation criteria from the end of surgery vs. those that do not receive neuromuscular blockade reversal.
The Society of Thoracic Surgery defines early extubation as extubation within six hours of completion of cardiac surgery and uses this as a reportable benchmark.

The final train-of-four ratio was determined at time of extubation using a commercially available quantitative neuromuscular monitor and categorized as greater than or equal to 0.9 or less than 0.9. In order to assess the depth of neuromuscular blockade and recovery from paralyzing anesthesia drugs, a nerve is rapidly stimulated four times creating four muscle twitches. The Train-of-Four ratio is the amplitude of the fourth muscle twitch divided by the amplitude of the first twitch. A TOF Ratio of 0.9 or greater is usually indicative of adequate recovery from muscle paralysis. |

Post-Extubation Hypoxemia
Lasso di tempo: Average of 6 hour intervals over first 24 hours post-operatively
|
Post-extubation hypoxemic episodes were defined according to the Berlin criteria where PaO2/FiO2 ratios were approximated using previously validated SpO2/FiO2 ratios every 6 hours for 24 hours post-extubation.
SpO2/FiO2 < 235 corresponded to moderate to severe hypoxemia and 235-315 corresponded to mild hypoxemia.
Reported ratio values were calculated as the average of data collected (every 6 hours) during the 24-hour period post-extubation.
