Estudo de Tratamento com Peginterferon para Pacientes Inativos com Hepatite B Crônica
Estudo do mundo real do tratamento com peginterferon alfa-2b para pacientes inativos com hepatite B crônica: estudo E-Cure
Existem cerca de 400 milhões de pacientes com infecção crônica pelo vírus da hepatite B (HBV) em todo o mundo, representando uma séria ameaça à segurança da saúde pública global. Na China, a infecção pelo VHB ocorreu principalmente no período perinatal ou em lactentes, e cerca de 10% dos pacientes no estágio de tolerância imune transitam espontaneamente para o estágio de eliminação imune a cada ano e tornam-se infecção crônica pelo VHB HBeAg-negativo, resultando em um aumento significativo na número de pacientes inativos com hepatite B crônica (CHB).
Nos últimos anos, diferentes diretrizes não chegaram a um consenso sobre a necessidade de iniciar a terapia antiviral para pacientes inativos com CHB: nas diretrizes da Associação do Pacífico Asiático para o Estudo do Fígado (APASL)-2015 e da Associação Americana para o Estudo das Doenças do Fígado (AASLD )-2018, a terapia antiviral geralmente não é recomendada para esse grupo de pacientes, e o acompanhamento ambulatorial regular é recomendado. A diretriz da Associação Europeia para o Estudo do Fígado (EASL)-2017 sugere que pessoas com histórico familiar de cirrose e câncer de fígado neste estágio podem ser tratadas com terapia antiviral, mesmo que não atendam às indicações da terapia antiviral. De acordo com as Diretrizes para a Prevenção e Tratamento da Hepatite B Crônica (versão 2019) da China, a terapia antiviral ainda é recomendada para alguns pacientes com status de portador inativo do HBsAg que são positivos para o DNA do VHB e atendem às indicações de tratamento. Estudos têm mostrado que alguns pacientes no estágio de tolerância imunológica podem entrar no estágio de eliminação imune e ter exacerbação da hepatite. Pacientes com CHB inativa têm potencial para desenvolver CHB HBeAg-negativo, e estudos de acompanhamento de longo prazo nessa população indicaram o risco de carcinoma hepatocelular. Com a popularização do conceito de cura funcional da hepatite B crônica, cada vez mais pessoas com HBC inativa têm forte desejo de tratamento. Nos últimos anos, vários estudos demonstraram que a terapia com interferon peguilado pode alcançar alta taxa de cura funcional em pacientes com HBC inativa.
O objetivo deste estudo é estabelecer um estudo nacional multicêntrico, prospectivo do mundo real para comparar a eficácia de diferentes regimes de tratamento antiviral para pacientes com HBC inativa e buscar os fatores de cura funcional.

Descrição
Critério de inclusão:
- Idade 18-60, sem limitação de gênero
- HBsAg é positivo há mais de 6 meses
- O antígeno da hepatite B (HBeAg) é negativo e o anti-HBe é positivo
- O DNA do VHB sérico é inferior a 2.000 UI/mL
- Alanina aminotransferase (ALT) e/ou Aspartato aminotransferase (AST) é normal
- Nenhum medicamento antiviral (incluindo análogo de nucleos(t)ide e interferon) foi usado antes da inscrição
- Boa adesão e consentimento informado assinado voluntariamente
Critério de exclusão:
- Alérgico ao interferon α-2b peguilado
- Qualquer indicação de cirrose hepática
- Coinfecção com vírus da hepatite A (HAV), vírus da hepatite C (HCV), vírus da hepatite D (HDV), vírus da hepatite E (HEV) ou vírus da imunodeficiência humana (HIV)
- Combinado com outras doenças hepáticas (incluindo doenças hepáticas metabólicas genéticas, alcoólicas, relacionadas a drogas, etc.)
- Existem lesões graves em órgãos importantes, como coração, pulmão, rim, cérebro e fundo
- Pacientes com doenças autoimunes, diabetes instável ou doenças da tireoide (hipertireoidismo ou hipotireoidismo)
- Câncer de fígado confirmado ou suspeito ou outros tumores malignos
- Pacientes após ou se preparando para transplante de órgãos
- Contagem de leucócitos no sangue periférico < 3,5×109/L e/ou contagem de plaquetas < 80×109/L
- Em tratamento imunossupressor
- Grávida ou gravidez planejada em curto prazo ou pacientes em lactação
- Abuso de álcool (a ingestão média de álcool é superior a 40 g/d em homens ou 20 g/d em mulheres) ou viciados em drogas
- História atual ou passada de doenças mentais ou psicológicas
- Outras condições que os investigadores considerem inadequadas para o estudo.

Análogos orais de nucleos(t)ídeos (NAs) (Entecavir (ETV), comprimidos de fumarato de tenofovir disoproxil (TDF) ou fumarato de tenofovir alafenamida (TAF)) são usados por 12 a 24 semanas, seguidos por terapia combinada com peginterferon α-2b.
O curso máximo é de 96 semanas.
O período de seguimento é de 144 semanas.
