A febre pós-operatória é um sintoma clínico comum em pacientes após cirurgia da coluna vertebral, com incidência de 13,2%, e 64,5% dos pacientes com escoliose neuromuscular desenvolveram febre pós-operatória. A causa de febre mais frequentemente observada é o trauma cirúrgico, que está associado a respostas inflamatórias à cirurgia, e essas febres geralmente desaparecem em 2 a 3 dias. Essa causa autolimitada de febre pós-operatória não é patológica,4 e a gravidade e a duração dessas febres dependem da duração, do escopo e do tipo de cirurgia, e da inserção de um enxerto de tela. A etiologia da febre pós-operatória imediata em pacientes após implante de prótese de quadril é desconhecida e é inconveniente iniciar tratamento com antibióticos ou aumentar a dose profilática de antibióticos. No entanto, existem raros relatos de calafrios e febres ocorrendo imediatamente após cirurgia da coluna vertebral. Até onde sabemos, não houve relatos sobre o mecanismo de febre pós-operatória imediata após cirurgia da coluna vertebral.
Com base nesta evidência e na observação, quando os pacientes apresentam febre pós-operatória, quase todos os cirurgiões procuram excluir a infecção como causa. O caso apresentado neste artigo é de uma jovem de 15 anos com escoliose idiopática que foi submetida à incisão posterior da escoliose idiopática e correção para fixação do pedículo. Os sintomas clínicos de interesse, incluindo calafrios, febre alta, aumento da frequência cardíaca e aumento da pressão arterial, foram observados imediatamente durante a reanimação anestésica. Neste relatório, os fatores de risco, o tratamento e a prevenção são avaliados com base nas evidências do caso.
