La fièvre postopératoire est un symptôme clinique courant chez les patients après une chirurgie de la colonne vertébrale, avec une incidence de 13,2 %, et 64,5 % des patients atteints de scoliose neuromusculaire ont développé des fièvres postopératoires. La cause de fièvre la plus fréquemment observée est le traumatisme chirurgical, qui est associé à des réponses inflammatoires à la chirurgie, et ces fièvres disparaissent généralement en 2 à 3 jours. Cette cause spontanément résolutive de fièvre postopératoire n'est pas pathologique4 et la gravité et la durée de ces fièvres dépendent de la durée, de l'étendue et du type d'intervention chirurgicale, ainsi que de l'insertion ou non d'un greffon maillé. L'étiologie de la fièvre postopératoire immédiate chez les patients après l'implantation d'une prothèse de hanche est inconnue et il n'est pas opportun d'instaurer un traitement antibiotique ou d'augmenter la posologie prophylactique d'antibiotiques. Cependant, de rares rapports font état de frissons et de fièvres survenant immédiatement après une chirurgie de la colonne vertébrale. À notre connaissance, aucun rapport n'a été signalé sur le mécanisme de la fièvre postopératoire immédiate après une chirurgie de la colonne vertébrale.
Sur la base de ces preuves et de l'observation, lorsque les patients présentent des fièvres postopératoires, presque tous les chirurgiens cherchent à exclure l'infection comme cause. Le cas présenté dans cet article est celui d’une jeune femme de 15 ans atteinte de scoliose idiopathique ayant subi une incision de scoliose idiopathique postérieure et une correction pour fixation pédiculaire. Les symptômes cliniques intéressants, notamment des frissons, une forte fièvre, une accélération du rythme cardiaque et une augmentation de la pression artérielle, ont été observés immédiatement au cours de la réanimation anesthésique. Dans ce rapport, les facteurs de risque, le traitement et la prévention sont évalués sur la base des preuves de cas.
