Postoperatieve koorts is een veel voorkomend klinisch symptoom bij patiënten na een operatie aan de wervelkolom, met een incidentie van 13,2%, en er werd waargenomen dat 64,5% van de patiënten met neuromusculaire scoliose postoperatieve koorts ontwikkelde. De meest voorkomende oorzaak van koorts is chirurgisch trauma, dat gepaard gaat met ontstekingsreacties op operaties, en dergelijke koorts verdwijnt doorgaans binnen 2-3 dagen. Deze op zichzelf beperkte oorzaak van postoperatieve koorts is niet-pathologisch,4 en de ernst en duur van dergelijke koortsen zijn afhankelijk van de duur, de omvang en het type operatie, en of er een mesh-transplantaat is ingebracht. De etiologie van onmiddellijke postoperatieve koorts bij patiënten na implantatie van een heupprothese is onbekend, en het is niet opportuun om een antibioticabehandeling te starten of de profylactische antibioticadosering te verhogen. Er zijn echter zeldzame meldingen van koude rillingen en koorts die onmiddellijk na een wervelkolomoperatie optreden. Voor zover ons bekend zijn er geen rapporten over het mechanisme van onmiddellijke postoperatieve koorts na een wervelkolomoperatie.
Op basis van dit bewijsmateriaal en observatie proberen bijna alle chirurgen, wanneer patiënten postoperatieve koorts ervaren, een infectie als oorzaak uit te sluiten. Het geval dat in dit artikel wordt gepresenteerd, is dat van een 15-jarige vrouw met idiopathische scoliose die een posterieure idiopathische scoliose-incisie onderging en correctie voor pedikelfixatie. De klinische symptomen die van belang zijn, waaronder koude rillingen, hoge koorts, een verhoogde hartslag en verhoogde bloeddruk, werden onmiddellijk waargenomen tijdens de anesthesie-reanimatie. In dit rapport worden risicofactoren, behandeling en preventie beoordeeld op basis van casusbewijs.
