Natriumtransport: Genetik och hypertoni

BAKGRUND:
Blodtrycket har en kontinuerlig unimodal fördelning i den allmänna befolkningen. Individer med nivåer förhöjda över 80:e percentilen lider av hypertoni, en viktig orsak till kardiovaskulär sjuklighet och dödlighet. Över 95 procent av hypertonierna har ingen annan primär sjukdom och sägs ha essentiell hypertoni. Stamtavla och tvillingstudier indikerar att genetiska skillnader står för en stor del av den interindividuella variationen i blodtryck. År 1984, när studien började, hade inga genetiska loci ännu identifierats som var användbara markörer för essentiell hypertoni eller fungerade som sonder i den biologiska grunden för interindividuell blodtrycksvariation. Miljöfaktorer påverkar också blodtrycket; ökat intag av natriumklorid i kosten, i synnerhet, höjer blodtrycket och kan bidra till utvecklingen av essentiell hypertoni hos vissa människor. Det antogs att genetiskt bestämda förändringar i metabolismen av natriumjoner inträffar i en delmängd av befolkningen som har ökad benägenhet att utveckla förhöjt blodtryck vid exponering för det vanliga intervallet av natriumintag via kosten
Rapporter om förändrad natriumtransport i de röda blodkropparna hos individer med essentiell hypertoni och deras avkomma stimulerade intresset för denna hypotes. Flera förslag erbjöds för att förklara hur förändringar i natriumtransport liknande de som observeras i röda blodkroppar kan leda till förhöjt blodtryck när det finns i njurtubuli eller vaskulär glatt muskulatur. Studier av natrium-litium-mottransportsystemet indikerade att natriumtransportvägar för röda blodkroppar kan ge informativa fenotyper som relaterar blodtryck och essentiell hypertoni närmare till genetiska faktorer.
Flera laboratorier har bekräftat att den maximala hastigheten för natrium-litium mottransport ökar i röda blodkroppar från individer med essentiell hypertoni. Natrium-litium-mottransporten är normal vid sekundära former av hypertoni och ökade nivåer av essentiell hypertoni kan inte hänföras till ålder, kroppsstorlek, kön eller intag av salt i kosten. De högsta nivåerna observeras hos individer med en positiv familjehistoria av essentiell hypertoni och de lägsta nivåerna förekommer hos normotensiva kontroller utan hypertensiva släktingar.
DESIGNBERÄTTELSE:
När studien startade 1984 var målet att fastställa om genetiska förändringar i vägarna för natriumtransport i de röda blodkropparna hos barn kunde förutsäga deras risk att utveckla primär hypertoni i vuxen ålder. Cirka 600 barn mellan 7 och 18 år valdes slumpmässigt ut som indexfall för familjestudier. Varje deltagande medlem i de studerade stamtavlor genomgick venpunktion för uttag av helblod. En sjukdomshistoria erhölls från varje familjemedlem som besökte kliniken och en fysisk undersökning genomfördes. Befintliga journaler för alla levande och avlidna vuxna släktingar i de 600 antavlorna granskades. Dessutom genomgick 40 normotensiva vuxna medlemmar av 300 stamtavlor metabolisk balans och njurclearance studier, samt ambulatoriska blodtrycksregistreringar. Segregationsanalys av stamtavladata utfördes för att bestämma det mest sannolika sättet för genetisk överföring av natrium-litium-mottransportfenotypen. Genetisk baslinjeinformation om natriumlitium var relaterad till förekomsten av primär hypertoni i familjer med indexbarn. Ytterligare fenotyper involverade i blodtrycksreglering studerades också inklusive renal proximal tubulär natriumreabsorption, atrial natriuretisk peptid, natrium-kaliumadenosintrifosfatpump och natrium-kalium-samtransport.
Studien förnyades flera gånger, den sista 1992 för att fortsätta arbetet på den genetiska grunden för interindividuell variation i risken för essentiell hypertoni i befolkningen i stort med hjälp av Rochester Family Heart Study (RFHS). RFHS användes för att ta itu med tre huvudfrågor: 1) Vilka mellanliggande biokemiska, fysiologiska och antropomorfa egenskaper som förutspådde blodtrycket och bidrog till patogenesen av essentiell hypertoni 2) Hade allelisk variation i en eller flera gener stora effekter på någon av dessa intermediärer egenskaper? 3) Förbättrade information om allelvariation förutsägelsen av risken för essentiell hypertoni utöver vad som tillhandahölls genom mått på de mellanliggande egenskaperna?
