Effekten av en enstaka dos av intranasalt oxytocin för att förebygga posttraumatiskt stressyndrom (PTSD)

Effektiviteten av den närliggande administreringen av oxytocin efter en traumatisk händelse för att förhindra utvecklingen av posttraumatiskt stressyndrom
Denna studie är utformad för att testa hypotesen att en enstaka administrering av intranasal oxytocin inom 6 timmar efter trauma underlättar den fysiologiska återhämtningen efter traumat, och förhindrar därigenom utvecklingen av posttraumatisk stressstörning (PTSD) under månaderna efter händelsen.
I avsaknad av sådan behandling (dvs under placebotillstånd) antar vi att en större andel personer kommer att utveckla PTSD (d.v.s. misslyckas med att återhämta sig från akuta effekter).

Detaljerad beskrivning
Detta är en dubbelblind, placebokontrollerad studie där traumaoffer randomiseras till att få en enda intranasal administrering av antingen Oxytocin (40 IE) eller placebo inom de första sex timmarna efter traumaexponering.
För att ge en förbehandlingsbaslinje kommer deltagarna att få en medicinsk och psykologisk utvärdering före behandlingen.
Efter två veckor kommer forskarassistenten eller studiepsykiatern att utföra beteendebedömningar och fullständiga historikdetaljer som hänför sig till PTSD-riskfaktorer.
Deltagarna kommer att bedömas igen av studiepsykiatern eller forskningsassistenterna vid 1, 3, 6 och 13 månader.
Kvalificerade försökspersoner kommer att inkludera män och kvinnor över 18 år, som har blivit utsatta för en händelse som uppfyller DSM-IV "A.1"-kriteriet för traumaexponering, och som lämnar skriftligt, informerat samtycke till att delta i studien.
För att rekrytera personer som är mer benägna att löpa risk för utveckling av PTSD kommer vi endast att randomisera personer som uttrycker markant ångest, känslomässigt lidande eller dissociation, enligt bedömningen av Visual Analog Scales.
Potentiella deltagare kommer att rekryteras från traumaoffer som anländer till Chaim Sheba Medical Centers akutrum.

Inklusionskriterier:
- Personer över 18 år, som har utsatts för en händelse som uppfyller DSM-IV "A.1"-kriteriet för traumaexponering, uttrycker påtaglig ångest och/eller känslomässig ångest och/eller dissociation, enligt bedömningen av Visual Analog Scales
- Den traumatiska händelsen inträffade upp till sex timmar före ankomsten till akuten
- Personen kan och är villig att ge skriftligt, informerat samtycke för att delta i studien.
Exklusions kriterier:
- Fysisk skada som skulle kontraindikera deltagande eller störa en försökspersons förmåga att ge informerat samtycke eller samarbeta med screening eller insamling av initiala åtgärder. Exempel inkluderar allvarliga brännskador, livshotande medicinskt eller kirurgiskt tillstånd, tillstånd som kräver kirurgiskt ingripande under allmän anestesi, enligt vad som anges av förkortad skadeskala (AIS), eller genom klinisk bedömning;
- Huvudskada som involverar förvirring, medvetslöshet eller minnesförlust;
- Medicinska tillstånd där administrering av oxytocin kan skada patienten, såsom patienter med hjärt-kärlsjukdom eller intrakraniell massa.
- Vikt under 45 eller över 100 kg.
- Graviditet (i suggestiva fall kommer ett graviditetstest att utföras);
- Traumatisk exponering som återspeglar pågående utsatthet (t.ex. våld i hemmet) som personen sannolikt kommer att utsättas för igen under studieperioden.
- Öppen psykopatologi, berusning eller påverkad av substanser.
- Bevis eller historia av schizofreni, bipolär, andra psykotiska tillstånd, autism;
- Tidigare historia av PTSD;
- Aktuell eller tidigare historia av demens, minnesförlust eller annan kognitiv störning som föregick traumaexponering;
- Bedömd allvarlig självmordsrisk.
