En studie för att bedöma säkerheten och effektiviteten av IPN10200 hos vuxna deltagare med måttliga till svåra övre ansiktslinjer (LANTIC)
En fas Ib/II, multicenter, dubbelblind, randomiserad, placebokontrollerad, dosökning och dossökningsstudie för att utvärdera säkerheten och effekten av IPN10200 för att förbättra utseendet av måttliga till svåra övre ansiktslinjer hos vuxna
Syftet med denna studie är att bedöma säkerhets- och effektprofilen för ökande doser av IPN10200 jämfört med placebo, med syftet att upptäcka de doser som erbjuder den bästa effektiviteten/säkerhetsprofilen när de används för behandling av måttlig till svår Övre ansiktslinjer.
Denna studie kommer att genomföras i tre steg. Hela studien (inklusive alla stadier) kommer att ha maximalt 448 deltagare. Protokollet är för närvarande godkänt upp till steg 1, steg 1 och steg 1/steg 2.
Steg 1 (fas Ib & II)
- Steg 1 (Fas Ib): ett dos-eskalerande första-i-människa steg hos deltagare med måttliga till svåra Glabellar Lines (GL)
- Steg 2 (Fas II): steg för dossökning hos deltagare med måttlig till svår GL jämfört med Dysport

Beskrivning
Inklusionskriterier:
- Deltagaren måste vara 18 till 65 år, inklusive, vid tidpunkten för undertecknandet av det informerade samtycket.
- Måttlig eller svår (Grad 2 eller 3) GL vid maximal sammandragning vid baslinjen, bedömd av ILA med hjälp av en validerad 4-punkts fotografisk skala.
- Måttlig eller svår (Grad 2 eller 3) GL vid maximal sammandragning vid baslinjen, bedömd av SSA med hjälp av en validerad 4-gradig kategorisk skala.
- Missnöjda eller mycket missnöjda (Betyg 2 eller 3) med sina linjer vid Baseline, enligt bedömningen av SLS.
Exklusions kriterier:
- En aktiv infektion eller andra hudproblem i det övre ansiktet inklusive GL-, FHL- och LCL-området (t. akuta akneskador eller sår).
- En historia av ögonlocksblefaroplastik eller ögonbrynslyft under de senaste 5 åren
- En historia av ansiktsnerverpares.
- Markerad ansiktsasymmetri, ptos, överdriven dermatochalasis, djupa ärrbildningar i huden eller tjock talghud.
- Alla kända medicinska tillstånd som kan sätta deltagaren i ökad risk när det gäller exponering för BoNT av någon serotyp (dvs. myasthenia gravis, Eaton-Lamberts syndrom, amyotrofisk lateralskleros, etc.)
- Har covid-19-sjukdom eller ett positivt SARS-CoV-2-test, eller närvaro av något annat tillstånd (t.ex. neuromuskulär störning eller annan störning som kan störa neuromuskulär funktion)
- Tidigare behandling med valfri BoNT-serotyp för steg 1/steg 1 eller någon nyligen genomförd behandling (inom de senaste 6 månaderna före baslinjen) med valfri BoNT-serotyp för steg 1/steg 2.
- All tidigare behandling med permanenta fillers i övre ansiktet inklusive GL-, FHL- och LCL-området.
- All tidigare behandling med långvariga dermala fillers i överansiktet inklusive GL-området under de senaste 3 åren och/eller hudavslitningar/ytbehandling (oavsett vilken interventionsteknik som använts) under de senaste 5 åren, eller fotoföryngring eller hud/vaskulär laserintervention inom 12 månader före Baseline.
- Eventuella planerade kosmetiska ansiktsoperationer under studien.
- Användning av samtidig behandling som, enligt utredarens åsikt, skulle störa utvärderingen av säkerheten eller effekten av studieinterventionen, inklusive mediciner som påverkar blödningsrubbningar (t.ex. trombocythämmande medel och/eller antikoagulantia som ges för behandling eller förebyggande av kardiovaskulära/cerebrovaskulära sjukdomar).
- Användning av mediciner som påverkar neuromuskulär överföring, såsom curare-liknande icke-depolariserande medel, linkosamider, polymyxiner, antikolinesteraser och aminoglykosidantibiotika, under de senaste 30 dagarna före Baseline.
- Användning av någon experimentell apparat inom 30 dagar eller användning av någon behandling med ett experimentellt läkemedel inom fem gånger den dokumenterade terminala halveringstiden för respektive läkemedel eller dess metaboliter eller om halveringstiden är okänd inom 30 dagar innan studiens start ( före Baseline) och under genomförandet av studien.
- Känd positiv för hepatit B-antigen, eller hepatit C-virusantikropp, eller för humant immunbristvirus (HIV) eller en diagnos av förvärvat immunbristsyndrom.
- Kliniskt diagnostiserad signifikant ångeststörning eller någon annan signifikant psykiatrisk störning (t. depression) som kan störa deltagarens deltagande i studien
- En oförmåga att avsevärt minska GL och/eller horisontella pannrytmer även genom att fysiskt sprida dem isär enligt utredarens bedömning.
- Känd allergi eller överkänslighet mot BoNT eller något hjälpämne av IPN10200 eller Dusport/Azzalure, eller allergi mot komjölksprotein.
- En historia av drog- eller alkoholmissbruk
- Gravida kvinnor, ammande kvinnor, premenopausala kvinnor eller kvinnor i fertil ålder som inte är villiga att utöva en mycket effektiv form av preventivmetod
- Manliga deltagare som inte är vasektomerade och som har kvinnliga partners i fertil ålder och som inte är villiga att använda kondom med spermiedödande medel under hela studiedeltagandet.

Experimentell: Corabotase group
Stage 1/Step 1: Several cohorts of participants will be randomized in a ratio of 3:1 (Corabotase:placebo)in a dose-escalation manner. The decision to escalate to the next dose for each cohort will be based on Data Monitoring Committee (DMC) recommendation. Stage 1/Step 2: parallel dose-ranging manner Stage 1/Step 3: each cohort will include three treatment groups randomised in a ratio of 4:1:1 (Corabotase: placebo:dysport) The decision to escalate to the next dose for each cohort will be based on DMC recommendation . Stage 2: the dose(s) chosen for administration in each region of the face will be selected on the basis of the intermediate analyses of Stage 1/Step 3.
