Een studie om de veiligheid en werkzaamheid van IPN10200 te beoordelen bij volwassen deelnemers met matige tot ernstige bovenste gezichtslijnen (LANTIC)
Een fase Ib/II, multicentrisch, dubbelblind, gerandomiseerd, placebogecontroleerd, dosisescalatie- en dosisbepalingsonderzoek om de veiligheid en werkzaamheid van IPN10200 te evalueren bij het verbeteren van de zichtbaarheid van matige tot ernstige bovenste gezichtslijnen bij volwassenen
Het doel van deze studie is om het veiligheids- en werkzaamheidsprofiel van toenemende doses IPN10200 te beoordelen in vergelijking met placebo, met als doel de dosis(sen) te ontdekken die het beste werkzaamheids-/veiligheidsprofiel bieden bij gebruik voor de behandeling van matige tot ernstige Bovenste gezichtslijnen.
Dit onderzoek zal in drie fasen worden uitgevoerd. De volledige studie (inclusief alle stadia) zal maximaal 448 deelnemers hebben. Het protocol is momenteel goedgekeurd tot fase 1, stap 1 en fase 1/stap 2.
Fase 1 (fase Ib & II)
- Stap 1 (Fase Ib): een dosis-escalatie eerste stap bij mensen bij deelnemers met matige tot ernstige fronslijnen (GL)
- Stap 2 (Fase II): dosisbepalende stap bij deelnemers met matige tot ernstige GL in vergelijking met Dysport

Beschrijving
Inclusiecriteria:
- Deelnemer moet 18 tot en met 65 jaar oud zijn op het moment van ondertekening van de geïnformeerde toestemming.
- Matige of ernstige (graad 2 of 3) GL bij maximale contractie bij baseline, zoals beoordeeld door de ILA met behulp van een gevalideerde 4-punts fotografische schaal.
- Matige of ernstige (graad 2 of 3) GL bij maximale contractie bij baseline, zoals beoordeeld door de SSA met behulp van een gevalideerde 4-punts categorische schaal.
- Ontevreden of zeer ontevreden (graad 2 of 3) met hun lijnen op baseline, zoals beoordeeld door de SLS.
Uitsluitingscriteria:
- Een actieve infectie of andere huidproblemen in het bovenvlak inclusief het GL-, FHL- en LCL-gebied (bijv. acute acne laesies of zweren).
- Een voorgeschiedenis van ooglidcorrectie of wenkbrauwlift in de afgelopen 5 jaar
- Een geschiedenis van aangezichtszenuwverlamming.
- Duidelijke gezichtsasymmetrie, ptosis, overmatige dermatochalasis, diepe huidlittekens of dikke talghuid.
- Elke bekende medische aandoening die de deelnemer een verhoogd risico kan geven met betrekking tot blootstelling aan BoNT van elk serotype (d.w.z. myasthenia gravis, Eaton-Lambert-syndroom, amyotrofische laterale sclerose, enz.)
- Heeft COVID-19-ziekte of een positieve SARS-CoV-2-test, of de aanwezigheid van een andere aandoening (bijv. neuromusculaire aandoening of andere aandoening die de neuromusculaire functie kan verstoren)
- Eerdere behandeling met een BoNT-serotype voor fase 1/stap 1 of een recente herbehandeling (in de afgelopen 6 maanden voorafgaand aan baseline) met een BoNT-serotype voor fase 1/stap 2.
- Elke eerdere behandeling met permanente fillers in het bovenvlak, inclusief het GL-, FHL- en LCL-gebied.
- Elke eerdere behandeling met langdurige huidvullers in het bovenvlak inclusief het GL-gebied in de afgelopen 3 jaar en/of schaafwonden/resurfacing van de huid (ongeacht de gebruikte interventietechniek) in de afgelopen 5 jaar, of fotoverjonging of huid-/vasculaire laserinterventie binnen de 12 maanden voorafgaand aan Baseline.
- Elke geplande cosmetische gezichtsoperatie tijdens het onderzoek.
- Gebruik van gelijktijdige therapie die, naar de mening van de onderzoeker, de evaluatie van de veiligheid of werkzaamheid van de studie-interventie zou verstoren, inclusief medicijnen die bloedingsstoornissen beïnvloeden (bijv. plaatjesaggregatieremmers en/of anticoagulantia die worden gegeven voor de behandeling of preventie van cardiovasculaire/cerebrovasculaire aandoeningen).
- Gebruik van medicijnen die de neuromusculaire transmissie beïnvloeden, zoals curare-achtige niet-depolariserende middelen, lincosamiden, polymyxines, anticholinesterasen en aminoglycoside-antibiotica, in de afgelopen 30 dagen voorafgaand aan de baseline.
- Gebruik van een experimenteel apparaat binnen 30 dagen of gebruik van een behandeling met een experimenteel geneesmiddel binnen vijf keer de gedocumenteerde terminale halfwaardetijd van het respectieve geneesmiddel of zijn metabolieten of als de halfwaardetijd onbekend is binnen 30 dagen voorafgaand aan de start van het onderzoek ( voorafgaand aan Baseline) en tijdens de uitvoering van het onderzoek.
- Bekend positief voor hepatitis B-antigeen of hepatitis C-virusantilichaam, of voor humaan immunodeficiëntievirus (HIV) of een diagnose van verworven immunodeficiëntiesyndroom.
- Klinisch gediagnosticeerde significante angststoornis of enige andere significante psychiatrische stoornis (bijv. depressie) die de deelname van de deelnemer aan het onderzoek zou kunnen belemmeren
- Een onvermogen om GL en/of horizontale voorhoofdrhytiden substantieel te verminderen, zelfs door ze fysiek uit elkaar te spreiden, zoals bepaald door de onderzoeker.
- Bekende allergie of overgevoeligheid voor BoNT of een van de hulpstoffen van IPN10200 of Dusport/Azzalure, of allergie voor koemelkeiwit.
- Een geschiedenis van drugs- of alcoholmisbruik
- Zwangere vrouwen, vrouwen die borstvoeding geven, vrouwen in de pre-menopauze of vrouwen die zwanger kunnen worden en niet bereid zijn een zeer effectieve vorm van anticonceptie toe te passen
- Mannelijke deelnemers die niet gesteriliseerd zijn en die vrouwelijke partners hebben die zwanger kunnen worden en die niet bereid zijn condooms met zaaddodend middel te gebruiken tijdens hun deelname aan het onderzoek.
