Detta är ett randomiserat kontrollerat pilotförsök av en multipelintervention för att främja fysisk aktivitet hos vuxna i medelåldern (MASTERY), jämfört med ett uppmärksamhetsmatchat kontrolltillstånd. I MASTERY kommer deltagarna att ha interaktiva, tvåvägs textmeddelandesessioner (5-10 minuters tvåvägstextkommunikation med ett automatiskt textmeddelandeprogram) en gång i veckan i 12 veckor. Budskapen kommer att fokusera på 3 distinkta områden i samma ordning varje vecka: (1) välmåendebaserade färdigheter och tilldelade aktiviteter, anpassade för vuxna i mitten av livet, (2) fysisk aktivitetsinnehåll fokuserat på att sätta upp och nå veckovisa aktivitetsmål, och ( 3) specifika färdigheter för att hantera medelåldersrelaterade stressorer som fungerar som hinder för aktivitet. Det primära resultatet är objektivt mätt måttlig till kraftig fysisk aktivitet (MVPA). Sekundära resultat inkluderar genomförbarhet och acceptans och andra aktivitetsrelaterade, psykologiska och funktionella resultat efter 12 veckor.

Inklusionskriterier:

  1. Medellivsstatus (ålder 45-64 vid inskrivning).
  2. Låg fysisk aktivitet vid baslinjen (≤150 minuter/vecka av MVPA]) som initialt självrapporterats via den välvaliderade IPAQ:n och sedan bekräftad via accelerometer efter 7 dagars användning.

Exklusions kriterier:

  1. Befintlig kranskärlssjukdom (CAD; diagnostiserats via hjärtkateterisering med standarddefinitioner av kransartärstenos eller tidigare ACS).
  2. Ett orelaterade tillstånd som begränsar fysisk aktivitet (t.ex. ambulering med käpp eller rollator, nyligen genomförd eller planerad operation eller annat tillstånd som gör att förmågan att öka måttlig till fysisk aktivitet skulle vara begränsad).
  3. Deltagande i andra program fokuserade på hjärtförebyggande eller välbefinnande (t.ex. hjärtrehabilitering).
  4. En kognitiv störning som utesluter deltagande eller informerat samtycke, bedömd med hjälp av en screening med sex punkter7 utformad för att bedöma lämpligheten för forskningsdeltagande.
  5. Oförmåga att tala/skriva flytande på engelska.
  6. Ingen tillgång till en SMS-kompatibel telefon.

Experimentell: MASTERY Intervention
Deltagarna kommer att slutföra en positiv psykologisk aktivitet, arbeta mot ett fysisk aktivitetsmål och använda en stressreducerande teknik, och kommer sedan att slutföra sms-sessioner varje vecka. Under de första sex veckorna kommer deltagarna att granska aktiviteterna de utförde föregående vecka, introduceras för nytt material, välja aktiviteter att utföra den veckan och sätta ett nytt fysisk aktivitetsmål under sms-sessionerna. Under de sista 6 veckorna kommer deltagarna att granska framstegen mot tidigare mål, sätta nya fysiska aktivitetsmål och välja positiva psykologi och stressreducerande färdigheter att använda den veckan. Slutligen kommer deltagarna under programmets gång att genomföra tre korta samtal med en studietränare för att diskutera framsteg.
I MASTERY kommer deltagarna att ha interaktiva, tvåvägs textmeddelandesessioner (5-10 minuters tvåvägstextkommunikation med ett automatiskt textmeddelandeprogram) en gång i veckan i 12 veckor. Dessutom kommer studietränare att genomföra fyra korta samtal med deltagarna (vecka 1, 4, 7 och 12) för att introducera programmet, bedöma framsteg, svara på frågor och ge stöd.
Aktiv komparator: Uppmärksam kontroll
Deltagarna kommer att få samma fysiska aktivitetskomponent som noterats i MASTERY tillsammans med tilläggsmeddelanden som ger utbildning och vägledning om fysisk aktivitet, från vårt tidigare arbete. Kontrolldeltagare kommer inte att få PP eller midlife-relaterat innehåll.
Deltagare i kontrollvillkoret kommer också att få sms en gång i veckan. För att säkerställa tidsmatchning kommer programmet att ha 4 telefonincheckningar (vid fysisk aktivitet) som i MASTERY.

ActiGraph accelerometrar är validerade som mått på fysisk aktivitet och har använts i ett flertal studier av fysisk aktivitet hos patienter med medicinsk sjukdom. To assess the feasibility of the MASTERY intervention the automated platform will record rates of the proportion of text message sessions in which participants respond and set a physical activity goal. The "proportion of text message sessions completed" will be calculated by dividing the number of sessions in which participants respond and set a physical activity goal by the total number of sessions initiated by the text message program.

Mean Utility Ratings of Positive Psychology Text Messages

In this trial, participants will wear the accelerometer for one week at baseline, 6 weeks, and 12 weeks. We will report change from baseline to 6 weeks and 12 weeks in the average number of steps per day.

Change in Self-reported Physical Activity

The International Physical Activity Questionnaire (IPAQ) is a well-validated 7-day physical activity recall assessment for physical activity. Activity will be measured by the number of MET-minutes of moderate or greater activity per week, and change will be assessed by subtracting the baseline score from the 6- and 12-week scores.

Change in Positive Affect (Positive Affect Items of the Positive and Negative Affect Schedule [PANAS])

The positive affect items on the Positive and Negative Affect Schedule (PANAS), a well-validated scale used in other intervention trials and in patients with medical illnesses, will be used to measure positive affect (Range: 10-50). Change will be calculated by subtracting the score at baseline from the score at 6 and 12 weeks. Higher scores indicate higher levels of positive affect.

Change in Optimism (Life Orientation Test - Revised [LOT-R])

The Life Orientation Test-Revised is a well-validated 6-item instrument used to measure dispositional optimism (Range: 0-24). Change will be calculated by subtracting the score at baseline from the score at 6 and 12 weeks. Higher scores indicate higher levels of optimism.

Change in Life Satisfaction (Satisfaction With Life Scale [SWLS])

The Satisfaction with Life Scale is a 5-item scale designed to measure global cognitive judgments of one's life satisfaction (Range: 5-35). Higher scores reflect higher levels of life satisfaction. Change will be calculated by subtracting the score at baseline from the score at 6 and 12 weeks.

Change in Depressive Symptoms (Hospital Anxiety and Depression Scale Depression Subscale [HADS-D])

The Hospital Anxiety and Depression Scale depression subscale (HADS-D) will be used to measure depression. This is a well-validated scale with few somatic symptom items that can confound mood/anxiety assessment in medically-ill patients (Range: 0-21). Change will be calculated by subtracting the score at baseline from the score at 6 and 12 weeks. Higher scores indicate higher levels of depression.

Change in Anxiety (Hospital Anxiety and Depression Scale Anxiety Subscale [HADS-A])

The Hospital Anxiety and Depression Scale (HADS)-anxiety subscale will be used to measure depression. This is a well-validated scale with few somatic symptom items that can confound mood/anxiety assessment in medically-ill patients (Range: 0-21). Change will be calculated by subtracting the score at baseline from the score at 6 and 12 weeks. Higher scores indicate higher levels of anxiety.

Change in Exercise Self-efficacy (Self-efficacy for Exercise Scale [SES])

The Self-efficacy for Exercise Scale will be used to measure exercise self-efficacy. This is a 9-item scale used to assess an individual's beliefs in their ability to continue exercising on a three time per week basis at moderate intensities for 20+ minutes per session in the future. (Range: 0-90). Change will be calculated by subtracting the score at baseline from the score at 6 and 12 weeks. Higher scores indicate higher levels of self-efficacy.

Change in Perceived Constraints (Perceived Constraints Subscale of the MIDUS Sense of Control Measure)

The 8-item subscale of the MIDUS Sense of Control measure will be used to measure internal locus of control - perceived constraints (Range: 8-56). Change will be calculated by subtracting the score at baseline from the score at 6 and 12 weeks. Lower scores indicate higher levels of perceived constraints.

Change in Personal Mastery (Personal Mastery Subscale of the MIDUS Sense of Control Measure)
