Detta är en fas Ib/II, öppen, multicenter, randomiserad plattformsstudie för att utvärdera neoadjuvanta immunterapikombinationer hos deltagare med resektabel HCC. Inklusionskriterier:

  • Diagnos av HCC bekräftad antingen histologiskt eller kliniskt enligt AASLD-kriterier för patienter med cirros. För deltagare utan cirros är histologisk bekräftelse obligatorisk.
  • HCC som är mottagligt för R0 kirurgisk resektion med kurativ avsikt enligt kirurger och onkologer eller hepatologer som är involverade i vården av deltagaren. Patienter som uppvisar resektabel HCC inom eller utanför Milanos kriterier (utan extrahepatisk spridning eller makrovaskulär invasion) är berättigade.
  • Mätbar sjukdom (minst en målskada) enligt RECIST v1.1 som fastställts av utredaren
  • Eastern Cooperative Oncology Group (ECOG) prestationsstatus på 0 eller 1 inom 7 dagar före randomisering
  • Child-Pugh klass A inom 7 dagar före randomisering
  • Negativt HIV-test vid screening
  • Ingen tidigare lokoregional eller systemisk behandling för HCC
  • Tillräcklig hematologisk funktion och ändorgansfunktion
  • Dokumenterad virologisk status för hepatit

  • Förekomst av extrahepatisk sjukdom eller makrovaskulär invasion
  • Känd fibrolamellär HCC, sarcomatoid HCC, blandad kolangiokarcinom och HCC, eller andra sällsynta varianter av HCC
  • Leverencefalopati i anamnesen om det är kliniskt signifikant inom ett år före påbörjad studiebehandling
  • Måttlig eller svår ascites
  • Aktiv samtidig infektion med HBV och HCV
  • Aktiv samtidig infektion med HBV och hepatit D virusinfektion
  • Tidigare behandling med CD137-agonister eller immunkontrollpunktshämmare, inklusive anti-CTLA-4, anti-PD-1 och anti-PD-L1 terapeutiska antikroppar
  • Behandling med undersökningsterapi inom 28 dagar innan studiebehandlingen påbörjas
  • Obehandlade eller ofullständigt behandlade esofagus- och/eller gastriska varicer med blödning eller som löper hög risk för blödning

Intrahepatic recurrence was defined by the appearance of one or more intrahepatic lesions with a longest diameter of > 1 cm and a typical vascular pattern of HCC on dynamic imaging (i.e., hypervascularization in the arterial phase with washout in the portal venous or late venous phase). Extrahepatic recurrence was assessed by RECIST v1.1 as the appearance of new, measurable malignant lesions outside the liver. Data for participants who did not have documented recurrence of disease or death were censored at the day of the last tumor assessment for participants. Kaplan-Meier (K-M) method was used to estimate median RFS.

Data for participants who had not experienced EFS events were censored at the time of their last post-surgical tumor assessment. K-M method was used to estimate median RFS.

Additionally, any pathological lymph nodes (whether target or non-target) must have a reduction in short axis to <10 millimeters (mm).
