背景:

学习目标:

方法:
研究设计是整群随机试验。 在卡尔加里区,研究人员将随机分配一半调查人员的 ED 医生接受 MTBI 的决策支持,另一半接受 PE 的决策支持。
在卡尔加里区之外,研究人员将按地点随机分配,因此一半地点将获得 MTBI 的决策支持,一半将获得 PE 的决策支持。 随机化的每一半将作为另一组的对照。 调查人员在卡尔加里区由医生随机化的原因是因为卡尔加里的计算机化医生医嘱输入系统允许我们实施这样的随机化,而在卡尔加里区外,医嘱输入系统的异质性使得随机化不可行医生,但按地点随机化是可行的。

纳入标准:
- 患者:所有因头部受伤和疑似肺栓塞而到预先确定的艾伯塔省急诊科就诊的患者都将作为评估 CT 适用性的目标。 对于 CT 头部,16 岁以上且 CEDIS 分类代码为“头部受伤”的患者将符合条件。 对于年龄在 18-85 岁且 CEDIS 分类代码为“胸痛(非心脏特征)”、“呼吸急促”或“晕厥/先兆晕厥”的 CT 肺血管造影 (CTPA) 患者,将符合条件。
- 卫生专业人员:参与该项目将属于专业职责和授权范围内,因为许多(如果不是全部)这些人将得到每个领域的地区领导层的支持。 拟议的干预措施具有教育性和自愿性;由于在某些临床情况下,临床判断有望取代临床指南,因此不会采取强制措施来强制医生普遍遵守。
