接受颅脑神经外科手术的患者在术后阶段尤为脆弱。 手术后的最初几小时代表着一个高风险窗口期,因为从麻醉状态到恢复期的过渡伴随着剧烈的生理变化以及神经或全身系统不稳定的潜在风险。 当患者从深度麻醉过渡到清醒状态时,自主神经张力和皮层活动的动态波动反映了大脑从手术和麻醉应激中恢复的能力。 因此,平稳的苏醒和拔管至关重要:咳嗽、高血压或颅内压(ICP)骤升可能危及止血效果并损害脑保护。 大多数严重并发症——如颅内出血、脑水肿、缺血、脑积水或脑疝——往往在术后最初几小时内发生。

描述
纳入标准:
- 计划进行择期颅脑神经外科手术的成年患者(年龄≥18岁)。
- 计划在术后麻醉恢复室(PACU)拔管。
- 能够在术中及术后获得高质量的心电图(ECG)和脑电图(EEG)记录(预计PACU监测时间≥60分钟)。
- 术前已签署书面知情同意书。
排除标准:
- 既往存在严重心律失常(例如:心房颤动、频发异位搏动)或依赖起搏器。
- 已知的严重自主神经病变(例如:晚期糖尿病、伴有自主神经衰竭的帕金森病)。
- 术前存在与脑电图异常相关的状况(癫痫持续状态、未控制的癫痫发作、镇静/催眠药物过量)。
- 术中或术后即刻发生需要持续机械通气或入住重症监护室(ICU)的事件(例如:大出血、术中心脏骤停)。
- 无法提供知情同意(例如:认知障碍、存在语言障碍且无翻译人员协助)。
- 拒绝签署知情同意书。
