Longitudinální přístup ke generování pozitivních kardiometabolických zdravotních výsledků u těžkých duševních onemocnění

Kardiometabolická onemocnění převládají u jedinců s psychotickými poruchami, významně přispívají k jejich kratší délce života, snížené kvalitě života a ekonomickému dopadu na jedince i společnost. Pro zlepšení kardiometabolického zdraví jsou zásadní účinné a individualizované intervence. Psychologické ambulance jsou pro tyto zákroky ideální díky pravidelným návštěvám pacientů a dostupnosti různorodých zdravotníků. Vyšetřovatelé vyvinuli a chtějí otestovat komplexní intervenční program pro zlepšení kardiometabolického zdraví, zlepšení kvality života a podporu zdravého životního stylu speciálně pro lidi s psychotickými poruchami na psychiatrických ambulancích v Göteborgu.

Tato klinická studie má za cíl zahrnout 644 jedinců s psychotickými poruchami ze šesti ambulancí na Klinice psychotických poruch Sahlgrenska University Hospital v Göteborgu. Dvě ambulance budou poskytovat LAGOM-intervenci, zatímco ostatní budou sloužit jako kontrolní s nabídkou „péče jako obvykle“. Intervenční skupině se dostane multidisciplinární podpory integrované do rutinních klinických postupů. Intervence zahrnuje pravidelné sledování a používání motivačních nástrojů, včetně analyzátoru tělesného složení a algoritmu predikce kardiovaskulárního rizika (QRISK3).

Model "obvyklé péče" v Göteborgu zahrnuje každoroční zdravotní prohlídky pro osoby s psychotickými poruchami. Pacienti absolvují dvě 60minutové návštěvy pro vyšetření jako krevní testy, měření krevního tlaku a kontroly hmotnosti, přičemž výsledky jsou hodnoceny pomocí standardních měřítek nebo rizikových algoritmů jako SCORE2. Lékaři mohou navrhnout doporučení k primární péči nebo doporučit změny životního stylu včetně úpravy stravy, cvičení nebo užívání návykových látek. Identifikované problémy mohou vést k jednoduchým radám nebo doporučením ke zdravotním poradcům pro podporu při odvykání kouření, stravovacím poradenství nebo skupinových aktivitách.

36 ± 6měsíční klinická studie standardizuje sběr dat na čtyřech ambulantních klinikách bez změny péče. Experimentální: Intervenční kliniky
Roční zdravotní prohlídky intervenční skupiny následují stejnou strukturu dvou návštěv jako v běžné rutinní péči, přičemž hlavní rozdíl spočívá v obsahu návštěv.

Intervenční skupina postupuje podle strukturovaného vývojového diagramu pro roční zdravotní prohlídky se zaměřením na posouzení kardiometabolického profilu s ohledem na pohlaví a etnický původ. Toto hodnocení zahrnuje sledování změn kardiometabolických parametrů spolu s celkovým kardiometabolickým rizikem pomocí SCORE2 a zda jsou splněna kritéria pro metabolický syndrom.

Návyky životního stylu jsou hodnoceny na základě zdravotního stavu, nemoci a přínosů ukončení nezdravého chování. Vzdělávací sezení vzdělávají účastníky a rodiny o spojení mezi psychotickými poruchami, volbou životního stylu a kardiometabolickým zdravím.
