Ocular Hypertension Treatment Study (OHTS) (OHTS)
For at bestemme, om medicinsk reduktion af intraokulært tryk forhindrer eller forsinker indtræden af glaukomatøst synsfelttab og/eller optisk diskbeskadigelse hos okulær hypertensive deltagere, der vurderes at have moderat risiko for at udvikle åbenvinklet glaukom.
At producere naturhistoriske data for at hjælpe med at identificere patienter med størst risiko for at udvikle åbenvinklet glaukom og dem, der med størst sandsynlighed vil drage fordel af tidlig medicinsk behandling.
At kvantificere risikofaktorer for udvikling af åbenvinklet glaukom blandt okulært hypertensive individer.

OHTS fase 3 vil genundersøge forsøgsdeltagere mere end 20 år efter tilmelding for at dokumentere klinisk status og forekomsten og sværhedsgraden af selvrapporterede funktionelle begrænsninger. De 279 deltagere, der udviklede POAG i OHTS fase 1 eller 2, vil have mere end 10 års post-POAG opfølgning i fase 3. Tidspunktet for genundersøgelse ved 20 år er meningsfuldt, fordi 20 år nærmer sig den gennemsnitlige forventede levetid for OHT patienter i 60'erne og 70'erne og halvdelen af den gennemsnitlige forventede levetid for patienter i 40'erne og 50'erne. For første gang vil patienter med okulær hypertension og klinikere have data af høj kvalitet om den langsigtede risiko for at udvikle POAG og funktionelle begrænsninger forbundet med sygdommen. Disse data vil lette patientcentreret pleje, så patienter og klinikere kan beslutte sig for den passende hyppighed af tests og undersøgelser og den potentielle fordel ved forebyggende behandling.
Grøn stær er en af de førende årsager til blindhed i USA og andre industrialiserede lande. Det anslås, at 2 millioner mennesker i USA har glaukom, og at 80.000 af disse personer er juridisk blinde af sygdommen. Blandt afroamerikanere er glaukom nu anerkendt som den førende årsag til blindhed.
Forhøjet intraokulært tryk (IOP), en almindelig tilstand, der påvirker 3 til 6 millioner mennesker i USA, menes at være den førende risikofaktor for udvikling af åbenvinklet glaukom. Der er ingen konsensus om, at medicinsk reduktion af intraokulært tryk forhindrer eller forsinker indtræden af synsfelt og/eller synsnerveskade hos okulær hypertensive personer.
På trods af manglen på overbevisende beviser for effektiviteten af medicinsk behandling ved okulær hypertension, bliver ca. 1,5 millioner glaukommistænkte i USA behandlet med dyre okulær hypotensive medicin, der har potentialet for alvorlige og endda livstruende bivirkninger.
Det er klart, at der er behov for et velkontrolleret klinisk forsøg for at bestemme, om medicinsk reduktion af IOP kan forhindre eller forsinke begyndelsen af glaukomskade hos okulær hypertensive personer. Først da kan klinikere og patienter træffe rationelle valg, og sundhedsplanlæggere sikre, at begrænsede medicinske ressourcer tildeles på en sikker og omkostningseffektiv måde.
Ocular Hypertension Treatment Study (OHTS) er et langsigtet, randomiseret, kontrolleret multicenter klinisk forsøg. Okulære hypertensive personer, der vurderes at have moderat risiko for at udvikle primært åbenvinklet glaukom, tildeles tilfældigt til enten kun tæt observation eller et trinvist medicinsk regime. Medicinsk behandling består af alle kommercielt tilgængelige topiske okulære hypotensive øjendråber.
Efter afslutning af baseline-målinger (IOP, synsfelter, diskfotos) og randomisering følges forsøgspersonerne i minimum 5 år med automatisk tærskel, central statisk perimetri (Humphrey program 30-2) to gange årligt og stereoskopiske optiske diskfotografier én gang årligt. Undersøgelsens endepunkter er reproducerbart synsfelttab og/eller progressiv optisk diskbeskadigelse i begge øjne hos en patient, der tilskrives glaukom af en maskeret endepunktskomité. Alle visuelle felter og fotografier af optiske diske læses på en maskeret måde i læsecentre.
I 1991 Baltimore Eye Survey blev afroamerikanere vist at have en forekomst af åbenvinklet glaukom fire til fem gange højere end hvide. I betragtning af denne høje forekomst af glaukom i den afroamerikanske befolkning, er det vigtigt at rekruttere og følge en passende prøve af afroamerikanske forsøgspersoner i forsøget (ca. 25 procent af den samlede patientprøve).
Ved afslutningen af denne undersøgelse bør praktiserende læger være i stand til at foretage rimelige skøn over risikoen for individuelle okulær hypertensive patienter og bestemme, hvilke okulær hypertensive personer, der mest sandsynligt vil have gavn af tidlig profylaktisk medicinsk behandling.
