Beschreibung
Einschlusskriterien:
- Männliche und weibliche Teilnehmer, die zum Zeitpunkt der Unterzeichnung der Einverständniserklärung (ICF) mindestens 18 Jahre alt sind.
- Histologisch oder zytologisch bestätigtes Cholangiokarzinom, das zuvor unbehandelt war und als nicht resezierbar und/oder metastasierend gilt (Stadium IV gemäß dem American Joint Committee on Cancer (AJCC) Cancer Staging Manual).
- Röntgenologisch messbare oder auswertbare Erkrankung durch CT oder MRT gemäß RECIST v1.1-Kriterien.
- Leistungsstatus der Eastern Cooperative Oncology Group 0 bis 1.
- Dokumentierte FGFR2-Umlagerung.
- Bereitschaft, eine Schwangerschaft oder das Zeugen von Kindern zu vermeiden.
Ausschlusskriterien:
- Erhaltene vorherige systemische Krebstherapie für nicht resezierbare und/oder metastasierte Erkrankungen (ohne adjuvante/neo-adjuvante Behandlung, die mindestens 6 Monate vor der Einschreibung abgeschlossen wurde, und Teilnehmer, die eine Behandlung für lokal fortgeschrittene Erkrankungen mit transarterieller Chemoembolisation oder selektiver interner Strahlentherapie erhalten haben , wenn ein klarer Hinweis auf eine radiologische Progression vor der Einschreibung beobachtet wird oder nach Änderung 6 (oder Änderung 5-JP2) eingeschrieben wurde und der Teilnehmer 1 Zyklus Gemcitabin plus Cisplatin erhielt [der Beginn des Studienmedikaments {Zyklus 1 Tag 1} muss bei sein mindestens 14 Tage und ≤ 4 Wochen {28 Tage} ab der letzten Dosis von Gemcitabin plus Cisplatin]).
- Child-Pugh B und C.
- Toxizitäten im Zusammenhang mit früheren Therapien müssen zum Zeitpunkt des Screenings Common Terminology Criteria for Adverse Events (CTCAE) v5.0 ≤ Grad 1 sein.
- Gleichzeitige Krebstherapie, außer den in dieser Studie getesteten Therapien.
- Der Teilnehmer ist ein Kandidat für eine potenziell kurative Operation.
- Aktuelle Hinweise auf klinisch signifikante Hornhaut- (einschließlich, aber nicht beschränkt auf bullöse/bandige Keratopathie, Hornhautabrieb, Entzündung/Ulzeration und Keratokonjunktivitis) oder Netzhauterkrankung (einschließlich, aber nicht beschränkt auf zentrale seröse Retinopathie, Makula-/Netzhautdegeneration, diabetische Retinopathie, Netzhautablösung ) wie durch augenärztliche Untersuchung bestätigt.
- Strahlentherapie, die innerhalb von 4 Wochen nach Einschreibung/Randomisierung/erster Dosis der Studienbehandlung verabreicht wird.
- Bekannte Metastasen des Zentralnervensystems (ZNS) oder unkontrollierte Krampfanfälle in der Vorgeschichte.

- Vorgeschichte einer Calcium- und Phosphat-Hämostasestörung oder eines systemischen mineralischen Ungleichgewichts mit ektopischer Verkalkung von Weichgeweben (Ausnahme: häufig beobachtete Verkalkungen in Weichgeweben wie Haut, Niere, Sehnen oder Gefäßen aufgrund von Verletzungen, Krankheiten und Alterung, in Abwesenheit von systemisches Mineralungleichgewicht).
- Signifikante gastrointestinale Störungen, die die Resorption, den Metabolismus oder die Ausscheidung von Pemigatinib beeinträchtigen könnten.
- Klinisch signifikante oder unkontrollierte Herzerkrankung.
- Vorgeschichte oder Vorhandensein eines abnormalen EKGs, das nach Ansicht des Prüfarztes klinisch bedeutsam ist.
- Chronische oder aktuelle aktive Infektionskrankheit, die eine systemische Antibiotika- oder antimykotische oder antivirale Behandlung innerhalb von 2 Wochen vor der Einschreibung erfordert (Teilnehmer mit asymptomatischen chronischen Infektionen mit prophylaktischer Behandlung sind erlaubt). Hinweis: HIV-positive Teilnehmer sind zugelassen, wenn alle folgenden Kriterien erfüllt sind: CD4+-Zahl ≥ 300/µL, nicht nachweisbare Viruslast, Erhalt einer antiretroviralen Therapie, die nicht mit dem Studienmedikament interagiert, und keine HIV/AIDS-assoziierte opportunistische Infektion letzten 12 Monate.
- Anwendung von potenten CYP3A4-Inhibitoren oder -Induktoren oder moderaten CYP3A4-Induktoren innerhalb von 14 Tagen oder 5 Halbwertszeiten (je nachdem, was länger ist) vor der ersten Dosis der Studienbehandlung. Hinweis: Moderate CYP3A4-Hemmer sind nicht verboten
- Bekannte Überempfindlichkeit oder schwere Reaktion auf Pemigatinib, Gemcitabin, Cisplatin oder deren Hilfsstoffe.
- Unzureichende Erholung von Toxizität und/oder Komplikationen nach einer größeren Operation vor Beginn der Therapie.

Experimental: Pemigatinib

Pemigatinib wird in der im Protokoll definierten Dosis einmal täglich oral als kontinuierliches Therapieschema verabreicht (ein Zyklus dauert 3 Wochen).

Aktiver Komparator: Gemcitabin + Cisplatin
Teilnehmer, bei denen es während der Behandlung mit Gemcitabin + Cisplatin oder während der Nachbeobachtungsphase und vor Beginn einer neuen Krebstherapie zu einer Krankheitsprogression kommt, sind für die Umstellung auf Pemigatinib berechtigt.

Gemcitabin 1000 mg/m^2 verabreicht als intravenöse Infusion an den Tagen 1 und 8 jedes 3-wöchigen Zyklus für bis zu 8 Zyklen.

The EORTC QLQ-C30 is a 30-item scale composed of both multi-item scales and single-item measures.

All scales and single-item measures range in score from 0 to 100.
