Descripción detallada
Los participantes elegibles para este estudio tienen un diagnóstico de DS-TB pulmonar.
Este es un estudio multicéntrico multicéntrico, abierto, aleatorizado y de búsqueda de dosis, que consiste en hasta 26 semanas de período de tratamiento.
Después de un período de detección de hasta 14 días, los participantes elegibles serán aleatorizados en el estudio.
La aleatorización se estratificará por la presencia de cavitación bilateral en la detección de rayos X de tórax (sí o no). Después del final del período de tratamiento, los participantes se seguirán hasta 12 meses después de la aleatorización.

Criterios de inclusión:
- Ser capaz de dar su consentimiento informado por escrito antes del inicio de cualquier procedimiento o tratamiento relacionado con el ensayo y ser capaz, en opinión del investigador, de cumplir con todos los requisitos del ensayo.
- Participantes masculinos o femeninos entre 18 y 65 años de edad (inclusive) en la visita de selección.
- Peso corporal ≥ 35,0 kg en la visita de selección.
- Tuberculosis pulmonar recién diagnosticada, sensible a rifampicina e isoniazida (en la muestra de cribado).
- Capaz de producir esputo espontáneamente.
- Las mujeres en edad fértil (FOCBP) deben aceptar usar 2 métodos anticonceptivos diferentes aprobados o permanecer abstinentes durante su participación en el ensayo y durante 12 semanas después de la última dosis de IMP o dosis de RHEZ.

Criterio de exclusión:
- Se sabe o se sospecha que los participantes tienen resistencia a la rifampicina, isoniazida, etambutol, pirazinamida, DLM o BDQ, ya sea confirmado por el laboratorio o según el historial epidemiológico, en la selección.
- Evidencia de enfermedad metabólica clínicamente significativa (por ejemplo, incluida hipopotasemia en curso o actual [es decir, potasio <3,5 mEq/dL en la selección]), gastrointestinal, neurológica, psiquiátrica, endocrina o hepática (por ejemplo, hepatitis B y C); malignidad; u otras anomalías (que no sean la indicación que se está estudiando).

- Trastornos hemorrágicos conocidos o antecedentes familiares de trastornos hemorrágicos.
- Cualquier enfermedad o condición en la que esté contraindicado el uso de DLM, BDQ, OPC 167832, rifampicina, isoniazida, pirazinamida o etambutol.
- Cualquier tratamiento previo para M tuberculosis en los últimos 2 años.

- Evidencia clínica de TB extrapulmonar grave (p. ej., TB miliar, TB abdominal, TB urogenital, TB osteoartrítica, meningitis tuberculosa).
- Evidencia de silicosis pulmonar, fibrosis pulmonar u otra afección pulmonar considerada grave por el investigador (que no sea TB). En particular, cualquier afección subyacente que pueda interferir con la evaluación de las imágenes de rayos X, la recolección de esputo o la interpretación de los resultados del esputo, o comprometer de otro modo la participación del sujeto en el ensayo.
- Cualquier insuficiencia renal caracterizada por aclaramiento de creatinina/tasa de filtración glomerular estimada (eGFR) de < 60 ml/min/1,73 m2, o insuficiencia hepática caracterizada por alanina transaminasa o aspartato transaminasa > 2,0 × límite superior normal del rango de referencia del laboratorio clínico o bilirrubina > 2,0 × límite superior normal del rango de referencia del laboratorio clínico, en la selección.
- Glucosa de detección (sin ayuno) ≥ 200 mg/dL o hemoglobina glicosilada (HbA1c) ≥ 6,5 %.
- QTcF > 450 ms en participantes masculinos (> 470 ms en participantes femeninas), bloqueo auriculoventricular II o III, bloqueo bifasicular, en la selección o arritmias ventriculares clínicamente significativas actuales o anteriores. Otras anomalías del ECG, si el investigador las considera clínicamente significativas.
- Participantes que reciben cualquiera de los medicamentos prohibidos (consulte la Sección 6.5.1) dentro de los períodos especificados o que probablemente requieran terapia concomitante prohibida durante el ensayo.
- Mujeres participantes que estén amamantando o que tengan un resultado positivo en la prueba de embarazo antes de recibir la primera dosis de IMP o RHEZ el día 1.
- Historial actual de abuso significativo de drogas y/o alcohol que probablemente resulte en un cumplimiento deficiente de los requisitos del ensayo o que suponga un riesgo para el bienestar del participante durante el transcurso del ensayo.
- Antecedentes de hepatitis actual o portadores del antígeno de superficie de la hepatitis B (HBsAg) y/o anti-virus de la hepatitis C (VHC).
- Se excluyen los participantes que den positivo en cocaína u otras drogas de abuso (excluyendo estimulantes recetados conocidos y otros medicamentos recetados y marihuana) en la selección. Los niveles detectables de alcohol, marihuana, barbitúricos u opiáceos en la prueba de detección de drogas no son excluyentes si, en la opinión documentada del investigador, el participante no cumple con los criterios del Manual diagnóstico y estadístico de trastornos mentales, quinta edición para el trastorno por consumo de sustancias de moderado a grave y la prueba positiva no indica una condición clínica que afectaría la seguridad del participante o la interpretación de los resultados del ensayo, y el monitor médico acepta la participación antes del tratamiento.
- Antecedentes de haber tomado un fármaco en investigación dentro de los 30 días anteriores al ingreso al ensayo.
- Antecedentes de dificultad para donar sangre.
- Donación de sangre o plasma dentro de los 30 días anteriores a la dosificación.
- Antecedentes de trastornos mentales graves que, a juicio del investigador, excluirían al participante de participar en este ensayo.
- Cualquier exposición previa conocida a OPC-167832, DLM o BDQ.
- Participantes con comorbilidades médicas significativas que, a juicio del investigador, no deberían participar en el ensayo.
- Los participantes con una puntuación de Karnofsky < 60 serán excluidos del ensayo.
- Participantes que dieron positivo para la infección activa por coronavirus del síndrome respiratorio agudo severo (SARS-CoV-2) en la selección.
- Los participantes con coinfección por el VIH que no estén en un régimen antirretroviral estable que consista en tenofovir, emtricitabina/lamivudina, dolutegravir (es decir, > 3 meses), o que tengan una carga viral detectable, o que tengan un recuento de CD4 < 350 células/mm3 serán excluidos del juicio.

An adverse event (AE) was defined as any untoward medical occurrence in a clinical trial participant administered an investigational medicinal product or trial treatment and which does not necessarily have a causal relationship with this treatment. TEAEs are AEs with an onset date on or after the start of treatment. Serious AE are AEs that leads to death, is life threatening, requires or prolongs hospitalization, significant disability, congenital anomaly or birth defect, and other medically important serious advent. AEs were graded on a severity 3-point scale as follows: Mild: Discomfort noticed, but no disruption to daily activity.; Moderate: Discomfort sufficient to reduce or affect normal daily activity.;Severe: Inability to work or perform normal daily activity. Laboratory assessments included clinical chemistry (Albumin, Alkaline Phosphatase, Aspartate Aminotransferase, Total Bilirubin, Calcium, Creatinine, estimated Glomerular Filtration Rate (eGFR), Glucose, Fasting and Non-Fasting, Cholesterol, Inorganic Phosphorus, Magnesium, Potassium, Sodium, Triglycerides and Uric Acid), hematology (Activated Partial Thromboplastin Time, Prothrombin Time, Partial Thromboplastin Time, International Normalized Ratio (INR), Absolute CD4+ Count, Absolute Lymphocytes, Absolute Neutrophil Count, Hemoglobin, Platelet Count, White Blood Cell), and urinalysis (Urine Glucose, Blood and Protein).
As pre-specified in statistical analysis plan (SAP), Division of AIDS (DAIDS) criteria was used and abnormalities were graded as follows:Grade 1 - Mild; Grade 2 - Moderate; Grade 3 - Severe; Grade 4 - Life Threatening; Grade 5 - Death.

Vital signs measurements included systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate, body weight, and body temperature.
Blood pressure (i.e., SBP, DBP) and heart rate were measured in the supine and sitting positions after the participant had been in each position for at least 3 minutes.
The participants were categorized based on the clinically relevant vital sign values as per DAIDS criteria pre-specified in SAP.
Each vital sign parameter was graded using the DAIDS criteria as follows: Grade 1 - Mild; Grade 2 - Moderate; Grade 3 - Severe; Grade 4 - Potentially Life Threatening; Grade 5 - Death.

Sputum LAM concentrations were measured on up to 3 samples collected at baseline and postbaseline at Week 8 and at end of treatment.
A participant was classified as having achieved negative sputum conversion in LAM if the participant has the first of 2 visits of at least 1 week apart with sputum LAM negative (below the lower limit of quantification) and without a positive sputum LAM result in between.
Numeric sputum LAM concentration data was converted to a binary variable using the rule: if the sputum LAM concentration was less than 10 picograms per milliliter (pg/mL) then it was interpreted as a negative result.
If the sputum LAM concentration was greater than or equal to 10 pg/mL then it was interpreted as positive result.
SCC for LAM was considered to have been achieved if there were negative results at two consecutive visits with non-missing results.

Acquired resistance was defined as a post-baseline resistant result at any timepoint after a baseline susceptible result.
Baseline was defined as Day -1, if Day -1 was missing then Day 1 was used, if day 1 was missing then Week 1 was used.
Resistance data was collected for streptomycin, isoniazid, rifampicin, ethambutol, pyrazinamide, bedaquiline and delamanid.
Susceptibility testing (DST) for anti-TB medications was performed on positive M tuberculosis isolates from Day -1 or Day 1 cultures, and on the end of treatment sputum specimen.
Percentage of participants who were susceptible at baseline and developed resistance to the indicated drug (class) at any post-baseline visits was summarized.

Time to Sputum Culture Conversion (SCC)

Sputum culture conversion occurs when a participant has the first of 2 consecutive visits of at least 7 days apart with sputum cultures negative and without a positive sputum culture result in between, as well as no positive sputum culture after the negative results.
If a participant had a positive MGIT culture result throughout the study period, then the participant was considered as not having achieved SCC within the period under consideration.
The time to SCC in this case was censored.
Time to SCC was calculated from date of first dose using MGIT cultures without the mitigation method measures.
