Salud metabólica en mujeres asiáticas y sus hijos
Este estudio tiene como objetivo probar las siguientes hipótesis en mujeres reclutadas en las primeras etapas del embarazo y cuyos hijos serán seguidos hasta al menos los 14 años de edad.
- Los cambios epigenéticos en los tejidos conceptuales obtenidos al nacer reflejan el entorno al que estuvo expuesto el feto durante el desarrollo.
- El patrón de marcas epigenéticas en los promotores de genes obtenidos del ADN en los tejidos del nacimiento, junto con el genotipo, el fenotipo y las exposiciones ambientales, se puede utilizar para evaluar cómo el entorno perinatal afecta los subsiguientes fenotipos metabólicos, del neurodesarrollo y de otro tipo.

La dramática aparición de enfermedades no transmisibles (ENT) en Asia ha coincidido con la rápida transición socioeconómica y nutricional que tiene lugar en la región, con la prevalencia de diabetes quintuplicándose en Singapur en menos de cuatro décadas. Un aspecto único de la epidemia han sido las importantes diferencias étnicas en la predisposición a la diabetes entre los asiáticos y en la variación étnica entre el IMC y el riesgo de desarrollar resistencia a la insulina, un precursor de la diabetes tipo 2. Aparte de los factores genéticos, la dieta, el estilo de vida y los factores culturales podrían haber influido en la programación del desarrollo a través de los efectos sobre los mecanismos asociados con la plasticidad del desarrollo. Una pieza central de este estudio es examinar cómo los cambios epigenéticos al nacer reflejan las influencias del desarrollo pasado y, en asociación con otros factores, influyen en las trayectorias futuras del desarrollo y su relación con las ENT. También se centrarán los factores activos durante la infancia, especialmente los primeros 1000 días de vida.
El estudio longitudinal de cohortes de nacimiento GUSTO nos permite examinar las asociaciones entre las interacciones genéticas, ambientales y de estilo de vida en los tres grupos étnicos distintos, chinos, malayos e indios, presentes en la población de Singapur.
Los participantes han sido seguidos por el resultado de su embarazo, el crecimiento fetal y el desarrollo de la descendencia durante más de una década. Los participantes continuarán siendo objeto de seguimiento a medida que los niños entren en la fase puberal entre los 10 y los 14 años.
Los datos se recopilarán a través de cuestionarios y mediciones clínicas. Los cuestionarios incluyen factores socioeconómicos, la dieta de la madre y el niño, antecedentes médicos, comportamiento, cognición, factores de estilo de vida, estado de salud y entorno familiar. Las medidas biofísicas se obtendrán a partir de las medidas antropométricas de los participantes, los materiales biológicos humanos como sangre, hisopos bucales, saliva, cabello, orina, heces, muestras orales como dientes de leche exfoliados, placa dental y biopelículas de la lengua se recolectan de los participantes ( madre/padre/hijo cuando corresponda) en sus momentos de seguimiento con la cohorte GUSTO con su consentimiento.

Criterios de inclusión:
- Mujeres de 18 años en adelante
- Mujeres embarazadas que asisten a la primera visita (< 14 semanas EG) en las unidades de maternidad de KKH y NUH
- Ciudadanos de Singapur o residentes permanentes de Singapur
- Actualmente residente en Singapur continental
- Intención de entregar eventualmente en KKH o NUH
- Intención de residir en Singapur durante los próximos 5 años
- Intención de donar cordón, sangre de cordón y placenta
- El feto debe ser racialmente homogéneo con ambos grupos de abuelos de la misma etnia.
Criterio de exclusión:
- Se excluirán las mujeres cuyos embarazos terminen en abortos espontáneos.
- Mujeres embarazadas en quimioterapia
- Excluya a las mujeres con condiciones médicas significativas, p. Diabetes mellitus tipo 1, psicosis, etc.
- Excluir a las mujeres que toman ciertos medicamentos, p. Drogas psicotropicas.
- Se excluirán los matrimonios mixtos.

Madres embarazadas, bebés y niños
Mujeres en su embarazo temprano que asisten a la ecografía prenatal del primer trimestre en las unidades de maternidad públicas del Hospital de Mujeres y Niños KK (KKH) y el Hospital Universitario Nacional (NUH).
Solo mujeres mayores de 18 años que sean ciudadanas de Singapur o residentes permanentes de Singapur.
Los participantes deben tener la intención de dar a luz eventualmente en NUH o KKH y residir en Singapur durante los próximos 5 años.
Voluntad de donar cordón umbilical, sangre de cordón umbilical y placenta.
El feto debe ser racialmente homogéneo con ambos grupos de abuelos de la misma etnia.
Los bebés nacidos de estas madres serán objeto de seguimiento hasta que el niño tenga al menos 14 años de edad.
