At vokse op i Singapore mod sunde resultater (GUSTO)
Metabolisk sundhed hos asiatiske kvinder og deres børn
Denne undersøgelse har til formål at teste følgende hypoteser hos kvinder rekrutteret tidligt i graviditeten, og hvis børn vil blive fulgt op til mindst 14 års alderen.
- Epigenetiske ændringer i konceptuelt væv opnået ved fødslen afspejler det miljø, som fosteret blev udsat for under udviklingen.
- Mønstret af epigenetiske mærker i genpromotorer opnået fra DNA i fødselsvæv, sammen med genotype, fænotype og miljøeksponeringer, kan bruges til at vurdere, hvordan det perinatale miljø påvirker efterfølgende metaboliske, neuroudviklingsmæssige og andre fænotyper.

Den dramatiske fremkomst af ikke-smitsomme sygdomme (NCD'er) i Asien er faldet sammen med den hurtige socioøkonomiske og ernæringsmæssige overgang, der finder sted i regionen, hvor forekomsten af diabetes er femdoblet i Singapore på mindre end fire årtier. Et unikt aspekt af epidemien har været de betydelige etniske forskelle i disposition for diabetes blandt asiater og i den etniske variation mellem BMI og risikoen for at udvikle insulinresistens, en forløber for type 2-diabetes. Bortset fra genetiske faktorer kan kost, livsstil og kulturelle faktorer have påvirket udviklingsprogrammeringen gennem effekter på mekanismerne forbundet med udviklingsmæssig plasticitet. En central del af denne undersøgelse er at undersøge, hvordan epigenetiske ændringer ved fødslen både afspejler tidligere udviklingspåvirkninger og, i forbindelse med andre faktorer, påvirker fremtidige udviklingsbaner og dets forhold til NCD'er. Også faktorer, der er aktive i barndommen - især de første 1000 dage af livet - vil blive fokuseret på.
Den longitudinelle GUSTO fødselskohorteundersøgelse giver os mulighed for at undersøge sammenhænge mellem genetiske, miljømæssige og livsstilsinteraktioner i de tre forskellige etniske grupper, kinesere, malaysere og indere, der er til stede i den singaporeanske befolkning.
Deltagerne er blevet fulgt op for deres graviditetsresultat, fostervækst og afkoms udvikling i over et årti. Deltagerne vil fortsat blive fulgt op, efterhånden som børnene går ind i pubertetsfasen fra 10-14 års alderen.
Data vil blive indsamlet gennem spørgeskemaer og kliniske målinger. Spørgeskemaerne omfatter socioøkonomiske faktorer, moderens og barnets kost, sygehistorier, adfærd, kognition, livsstilsfaktorer, sundhedstilstand og hjemmemiljø. Bio-fysiske målinger vil blive opnået fra antropometriske målinger af deltagere, humane biologiske materialer såsom blod, mundprøver, spyt, hår, urin, afføring, orale prøver såsom eksfolierede mælketænder, tandplak og tungebiofilm indsamles fra deltagerne ( mor / far / barn, hvor det er relevant) på deres opfølgningstidspunkter med GUSTO-kohorten efter deres samtykke.

Inklusionskriterier:
- Kvinder 18 år og derover
- Gravide kvinder til første besøg (< 14 uger GA) på fødeafdelingerne på KKH og NUH
- Singapore-borgere eller fastboende i Singapore
- I øjeblikket bosat på fastlandet Singapore
- Intention om på sigt at levere i KKH eller NUH
- Intention om at bo i Singapore i de næste 5 år
- Intention om at donere navlestreng, navlestrengsblod og placenta
- Fosteret bør være racemæssigt homogent med begge sæt bedsteforældre af samme etnicitet
Ekskluderingskriterier:
- Kvinder, hvis graviditet ender med aborter, vil blive udelukket
- Gravide kvinder i kemoterapi
- Udeluk kvinder med betydelige medicinske tilstande f.eks. Type 1 diabetes mellitus, psykose mm.
- Udelukke kvinder på visse lægemidler – f.eks. psykofarmaka.
- Blandede ægteskaber vil være udelukket

Gravide mødre, spædbørn og børn
Kvinder i deres tidlige graviditet, der deltager i første trimester -antenatal ultralydsscanning på de offentlige moderskabsenheder på KK Women's and Children’s Hospital (KKH) og National University Hospital (NUH).
Kun kvinder i alderen 18 år og derover, der er Singapore -borgere eller Singapore faste beboere.
Deltagerne er nødt til at have til sidst at levere i Nuh eller KKH og opholde sig i Singapore i de næste 5 år.
Vilje til at donere ledning, ledningsblod og placenta.
Fosteret skal være racemæssigt homogent med begge sæt bedsteforældre med samme etnicitet.
Babyer født af disse mødre vil blive fulgt op, indtil barnet er mindst 20 år gammel.
