Kasvaminen Singaporessa kohti terveitä tuloksia (GUSTO)
Aineenvaihdunta aasialaisilla naisilla ja heidän lapsillaan
Tämän tutkimuksen tarkoituksena on testata seuraavia hypoteeseja naisilla, jotka on värvätty raskauden alkuvaiheessa ja joiden lapsia seurataan vähintään 14-vuotiaaksi asti.
- Syntyessä saatujen käsitteellisten kudosten epigeneettiset muutokset heijastavat ympäristöä, jolle sikiö oli alttiina kehityksen aikana.
- Syntymäkudosten DNA:sta saatujen geenipromoottoreiden epigeneettisten merkkien mallia yhdessä genotyypin, fenotyypin ja ympäristöaltistuksen kanssa voidaan hyödyntää arvioitaessa, kuinka perinataalinen ympäristö vaikuttaa myöhempiin metabolisiin, hermoston kehitykseen ja muihin fenotyyppeihin.

Yksityiskohtainen kuvaus
Ei-tarttuvien tautien (NCD) dramaattinen ilmaantuminen Aasiassa on sattunut samaan aikaan alueella tapahtuvan nopean sosioekonomisen ja ravitsemuksellisen muutoksen kanssa, ja diabeteksen esiintyvyys on viisinkertaistunut Singaporessa alle neljässä vuosikymmenessä. Yksi epidemian ainutlaatuinen piirre on ollut merkittävät etniset erot aasialaisten diabetekselle alttiuksissa sekä BMI:n ja insuliiniresistenssin, tyypin 2 diabeteksen edeltäjän, välisen etnisen vaihtelun välillä. Geneettisten tekijöiden lisäksi ruokavalio-, elämäntapa- ja kulttuuritekijät ovat saattaneet vaikuttaa kehitysohjelmointiin vaikuttamalla kehityksen plastisuuteen liittyviin mekanismeihin. Tämän tutkimuksen keskeinen osa on tutkia, kuinka epigeneettiset muutokset syntymässä sekä heijastavat menneitä kehitysvaikutuksia että yhdessä muiden tekijöiden kanssa vaikuttavat tuleviin kehityskulkuihin ja sen suhteeseen NCD:hen. Myös lapsuudessa aktiivisiin tekijöihin - erityisesti ensimmäisiin 1000 elinpäivään - kiinnitetään huomiota.
Pitkittäinen GUSTO-syntymäkohorttitutkimus antaa meille mahdollisuuden tutkia geneettisten, ympäristöllisten ja elämäntapavuorovaikutusten välisiä assosiaatioita kolmessa eri etnisessä ryhmässä, kiinalaisissa, malaijilaisissa ja intialaisissa, joita esiintyy Singaporen väestössä.
Osallistujia on seurattu raskauden lopputulosta, sikiön kasvua ja jälkeläisten kehitystä yli vuosikymmenen ajan. Osallistujia seurataan edelleen, kun lapset tulevat murrosikään 10–14-vuotiaina.
Tiedot kerätään kyselylomakkeilla ja kliinisillä mittauksilla. Kyselylomakkeet sisältävät sosioekonomisia tekijöitä, äidin ja lapsen ruokavaliota, sairaushistoriaa, käyttäytymistä, kognitiota, elämäntapatekijöitä, terveydentilaa ja kotiympäristöä. Biofysikaaliset mittaukset saadaan osallistujien antropometrisista mittauksista, osallistujilta kerätään ihmisen biologisia materiaaleja, kuten verta, poskipuikkoja, sylkeä, hiuksia, virtsaa, ulostetta, suunäytteitä, kuten kuoritut maitohampaat, hammasplakki ja kielen biofilmit ( äiti / isä / lapsi tarvittaessa) seuranta-aikoina GUSTO-ryhmän kanssa heidän suostumuksellaan.

- Naiset 18 vuotta ja vanhemmat
- Ensimmäisellä käynnillä raskaana olevat naiset (< 14 viikkoa GA) KKH:n ja NUH:n synnytysyksiköillä
- Singaporen kansalaiset tai Singaporen pysyvät asukkaat
- Asuu tällä hetkellä Singaporen mantereella
- Tarkoitus toimittaa lopulta KKH:ssa tai NUH:ssa
- Aikomus asua Singaporessa seuraavat 5 vuotta
- Aikomus luovuttaa napanuoraverta ja istukka
- Sikiön tulee olla rodullisesti homogeeninen molempien samaa etnistä alkuperää olevien isovanhempien kanssa
Poissulkemiskriteerit:
- Naiset, joiden raskaudet päättyvät keskenmenoon, suljetaan pois
- Kemoterapiaa saavat raskaana olevat naiset
- Sulje pois naiset, joilla on merkittäviä sairauksia, esim. Tyypin 1 diabetes mellitus, psykoosi jne.
- Sulje pois naiset, jotka käyttävät tiettyjä lääkkeitä - esim. psykotrooppiset lääkkeet.
- Seka-avioliitot suljetaan pois

Raskaana olevat äidit, imeväiset ja lapset
Naiset alkuraskaudessa, jotka käyvät ensimmäisen raskauskolmanneksen synnytystä edeltävässä ultraäänitutkimuksessa KK:n Naisten- ja lastensairaalan (KKH) ja Yliopistollisen sairaalan (NUH) yleisillä synnytysyksiköillä.
Vain 18-vuotiaat ja sitä vanhemmat naiset ovat Singaporen kansalaisia tai Singaporen vakinaisia asukkaita.
Osallistujien on tarkoitus lopulta toimittaa NUH:ssa tai KKH:ssa ja asua Singaporessa seuraavat 5 vuotta.
Halukkuus luovuttaa napanuoraverta ja istukka.
Sikiön tulee olla rodullisesti homogeeninen molempien samaa etnistä alkuperää olevien isovanhempien kanssa.
Näistä äideistä syntyneitä vauvoja seurataan, kunnes lapsi on vähintään 14-vuotias.
