Crescer em Cingapura rumo a resultados saudáveis (GUSTO)
Saúde metabólica em mulheres asiáticas e seus filhos
Este estudo visa testar as seguintes hipóteses em mulheres recrutadas no início da gravidez e cujos filhos serão acompanhados até, pelo menos, os 14 anos de idade.
- Mudanças epigenéticas em tecidos conceituais obtidos no nascimento refletem o ambiente ao qual o feto foi exposto durante o desenvolvimento.
- O padrão de marcas epigenéticas em promotores de genes obtidos a partir de DNA em tecidos de nascimento, juntamente com genótipo, fenótipo e exposições ambientais, pode ser utilizado para avaliar como o ambiente perinatal afeta os fenótipos metabólicos, neurodesenvolvimentais e outros subsequentes.

O dramático surgimento de doenças não transmissíveis (DNTs) na Ásia coincidiu com a rápida transição socioeconômica e nutricional que ocorre na região, com a prevalência de diabetes aumentando cinco vezes em Cingapura em menos de quatro décadas. Um aspecto único da epidemia foram as diferenças étnicas significativas na predisposição ao diabetes entre os asiáticos e na variação étnica entre o IMC e o risco de desenvolver resistência à insulina, um precursor do diabetes tipo 2. Além de fatores genéticos, fatores dietéticos, de estilo de vida e culturais podem ter influenciado a programação do desenvolvimento por meio de efeitos nos mecanismos associados à plasticidade do desenvolvimento. Uma peça central deste estudo é examinar como as mudanças epigenéticas no nascimento refletem as influências do desenvolvimento passado e, em associação com outros fatores, influenciam as futuras trajetórias de desenvolvimento e sua relação com as DNTs. Também serão enfocados os fatores ativos durante a infância - especialmente os primeiros 1000 dias de vida.
O estudo longitudinal de coorte de nascimento GUSTO nos permite examinar associações entre interações genéticas, ambientais e de estilo de vida nos três grupos étnicos distintos, chineses, malaios e indianos, presentes na população de Cingapura.
Os participantes foram acompanhados quanto ao resultado da gravidez, crescimento fetal e desenvolvimento da prole por mais de uma década. Os participantes continuarão a ser acompanhados à medida que as crianças entram na fase puberal dos 10 aos 14 anos.
Os dados serão coletados por meio de questionários e medidas clínicas. Os questionários incluem fatores socioeconômicos, dieta materna e infantil, histórico médico, comportamento, cognição, fatores de estilo de vida, estado de saúde e ambiente doméstico. Medições biofísicas serão obtidas a partir de medidas antropométricas dos participantes, materiais biológicos humanos, como sangue, swabs bucais, saliva, cabelo, urina, fezes, espécimes orais, como dentes de leite esfoliados, placa dentária e biofilmes de língua são coletados dos participantes ( mãe/pai/filho, quando aplicável) em seus pontos de tempo de acompanhamento com a coorte GUSTO mediante seu consentimento.

Critério de inclusão:
- Mulheres 18 anos e acima
- Gestantes atendidas na primeira consulta (< 14 semanas IG) nas maternidades do KKH e do NUH
- Cidadãos de Singapura ou Residentes Permanentes de Singapura
- Atualmente residente em Cingapura continental
- Intenção de eventualmente entregar em KKH ou NUH
- Intenção de residir em Singapura nos próximos 5 anos
- Intenção de doar cordão umbilical, sangue do cordão umbilical e placenta
- O feto deve ser racialmente homogêneo com ambos os conjuntos de avós da mesma etnia
Critério de exclusão:
- Mulheres cujas gestações terminam em abortos espontâneos serão excluídas
- Grávidas em quimioterapia
- Exclua mulheres com condições médicas significativas, por exemplo. Diabetes mellitus tipo 1, psicose, etc.
- Excluir mulheres em uso de certos medicamentos - por ex. drogas psicotrópicas.
- Os casamentos mistos serão excluídos

Grávidas, lactentes e crianças
Mulheres no início da gravidez que estão fazendo a ultrassonografia pré-natal do primeiro trimestre nas maternidades públicas do KK Women's and Children's Hospital (KKH) e do National University Hospital (NUH).
Somente mulheres com 18 anos ou mais que sejam cidadãs de Cingapura ou residentes permanentes de Cingapura.
Os participantes devem ter a intenção de eventualmente entregar em NUH ou KKH e residir em Cingapura pelos próximos 5 anos.
Vontade de doar cordão umbilical, sangue do cordão umbilical e placenta.
O feto deve ser racialmente homogêneo com ambos os conjuntos de avós da mesma etnia.
Os bebês nascidos dessas mães serão acompanhados até que a criança tenha pelo menos 14 anos de idade.
