Un estudio de remisión sin tratamiento con nilotinib, multicéntrico y de un solo brazo en pacientes con leucemia mielógena crónica BCR-ABL1 positiva en fase crónica que han alcanzado el estado de enfermedad residual mínima duradera (MRD) en el tratamiento de primera línea con nilotinib.

El objetivo principal de este estudio fue determinar el porcentaje de pacientes que se encontraban en MMR a las 48 semanas de iniciar la fase de TFR (los pacientes que requirieron reinicio del tratamiento se consideraron como no respondedores).

Fase de consolidación del tratamiento con nilotinib (NTCS): Los pacientes que cumplieron con todos los criterios de inclusión/exclusión se inscribieron en la fase de consolidación y continuaron recibiendo nilotinib durante 52 semanas. Todos los pacientes fueron tratados con la dosis planificada de nilotinib de 300 mg BID (o con un nivel de dosis reducido de 400 mg QD si fuera necesario desde la perspectiva de la toxicidad). Criterios de inclusión:

  • Pacientes masculinos o femeninos ≥ 18 años de edad
  • Mínimo de 2 años calendario de tratamiento con nilotinib con al menos los últimos 12 meses de tratamiento con nilotinib antes de la preselección a la dosis diaria total aprobada de 600 mg BID o a una dosis reducida de 400 mg QD si es necesario desde la perspectiva de la tolerancia para BCR -LMC ABL positivo en fase crónica documentada en el momento del diagnóstico
  • Evidencia de transcritos típicos de BCR-ABL (b3a2 y/o b2a2) en el momento del diagnóstico de CML-CP, es decir, antes del primer inicio del tratamiento con TKI que son susceptibles de cuantificación estandarizada por RT-PCR".
  • Paciente en MR4.5 en la preselección en el laboratorio designado por Novartis
  • Estado funcional ECOG de 0-2
  • Función adecuada del órgano diana definida por:

Los pacientes que recibieron un mínimo de 2 años de tratamiento de primera línea con nilotinib y con resultados de PCR preselección en ≥ MR4.5 ingresaron a la fase de consolidación del estudio (52 semanas - nilotinib 300 mg BID). Los pacientes con enfermedad residual mínima (MRD) al final de esta fase entraron en la fase de remisión sin tratamiento (TFR) en la que no se administró ningún tratamiento. Los pacientes no elegibles entrarán en la fase de continuación del estudio. Los pacientes con MRD al final de la fase de continuación ingresarán a la fase TFR-2 del estudio donde no se administra ningún tratamiento. Los pacientes no elegibles entrarán en la fase de continuación prolongada del estudio. Si en algún momento durante la TFR o la TFR-2 el paciente pierde la MMR, se reiniciará inmediatamente el tratamiento con nilotinib (nilotinib 300 mg dos veces al día).
El nilotinib se usa en cápsulas comerciales disponibles (excepto en Japón, donde se usan suministros clínicos) de 150 mg y 200 mg de concentración. El tratamiento ocurre durante las fases de consolidación, continuación, continuación prolongada, reinicio y reinicio-2 del estudio.

The endpoint was calculated as the number of participants with at least one detected BCR-ABL1 mutation divided by the number of participants who experienced loss of MMR after nilotinib suspension.
