Onderzoek naar remissie zonder behandeling met nilotinib bij CML-patiënten (chronische myeloïde leukemie). (ENESTfreedom)
Een eenarmig, multicenter, behandelingsvrij remissieonderzoek met nilotinib bij patiënten met BCR-ABL1-positieve chronische myeloïde leukemie in de chronische fase die een duurzame minimale residuele ziekte (MRD)-status hebben bereikt tijdens de eerstelijnsbehandeling met nilotinib.

De primaire doelstelling van deze studie was het bepalen van het percentage patiënten dat 48 weken na aanvang van de TFR-fase in MMR was (patiënten bij wie de behandeling opnieuw moest worden gestart, werden beschouwd als non-responders).
Behandelingsconsolidatiefase met nilotinib (NTCS): Patiënten die voldeden aan alle inclusie-/exclusiecriteria werden opgenomen in de consolidatiefase en kregen gedurende 52 weken nilotinib. Alle patiënten werden behandeld met de geplande nilotinib-dosis van 300 mg tweemaal daags (of met een verlaagd dosisniveau van 400 mg eenmaal daags indien nodig vanuit het oogpunt van toxiciteit). Om in aanmerking te komen voor de TFR-fase, moesten patiënten voldoen aan de protocolspecifieke definitie van duurzame MRD. De vier laatste per kwartaal uitgevoerde PCR-beoordelingen moeten aan de volgende criteria hebben voldaan:
- De laatste beoordeling was MR4.5 (BCR-ABL ≤ 0,0032% IS)
- Geen beoordeling slechter dan MR4.0 (BCR-ABL >0,01% Is en
- Maximaal twee beoordelingen tussen MR4.0 en MR4.5 (0,0032% IS<BCR-ABL ≤ 0,01% IS)
Nilotinib TFR-fase: Patiënten die in aanmerking kwamen om de TFR-fase in te gaan na voltooiing van de consolidatiefase van 52 weken, stopten met het gebruik van nilotinib op de eerste dag van de TFR-fase. De duur van deze fase was tot 10 jaar nadat de laatste patiënt in de TFR-fase kwam. BCR-ABL-waarden werden gedurende de eerste 48 weken elke vier weken gecontroleerd, de volgende 48 weken elke zes weken en de laatste periode elke 12 weken.
Nilotinib-behandelingsherstartfase (NTRI): als een patiënt MMR-verlies (BCR-ABL >0,1% IS) had in de TFR-fase, hervatte de patiënt de behandeling met nilotinib. Patiënten werden tot 10 jaar met nilotinib behandeld nadat de laatste patiënt in de nilotinib TFR-fase was gekomen. Patiënten bij wie de behandeling met nilotinib opnieuw moest worden gestart, werden gedurende de eerste 24 weken om de vier weken gecontroleerd op het BCR-ABL-niveau en vervolgens om de 12 weken bij patiënten die weer MMR kregen. De frequentie van BCR-ABL-monitoring bij patiënten die geen MMR terugkregen binnen de eerste 24 weken na hervatting van de behandeling was ten minste elke 12 weken of vaker zoals klinisch geïndiceerd.

Beschrijving
Inclusiecriteria:
- Mannelijke of vrouwelijke patiënten ≥ 18 jaar
- Minimaal 2 kalenderjaren behandeling met nilotinib met ten minste de laatste 12 maanden behandeling met nilotinib voorafgaand aan pre-screening met een goedgekeurde totale dagelijkse dosis van 600 mg tweemaal daags of met een verlaagde dosis van 400 mg eenmaal daags indien vereist vanuit het oogpunt van tolerantie voor BCR -ABL-positieve CML in gedocumenteerde chronische fase op het moment van diagnose
- Bewijs van typische BCR-ABL-transcripten (b3a2 en/of b2a2) op het moment van de CML-CP-diagnose, d.w.z. voorafgaand aan de eerste start van de TKI-behandeling, die vatbaar zijn voor gestandaardiseerde RT-PCR-kwantificatie"
- Patiënt in MR4.5 bij prescreening in het door Novartis aangewezen laboratorium
- ECOG-prestatiestatus van 0-2
Adequate eindorgaanfunctie zoals gedefinieerd door:
- Direct bilirubine ≤ 1,5 x ULN behalve voor i) patiënten met gedocumenteerd syndroom van Gilbert voor wie elke bilirubinewaarde is toegestaan en ii) voor patiënten met asymptomatische hyperbilirubinemie (levertransaminasen en alkalische fosfatase binnen normaal bereik).
- SGOT(AST) en SGPT(ALT) ≤ 3 x ULN, d.w.z. gelijk aan ≤ Graad 1 NCI-CTCAE v.4.03
- Serumlipase ≤ 2 x ULN, d.w.z. equivalent aan ≤ Graad 2 NCI-CTCAE v.4.03
- Alkalische fosfatase ≤ 2,5 x ULN
- Serumcreatinine < 1,5 x ULN
Patiënten moeten de volgende elektrolytwaarden binnen normale limieten hebben of gecorrigeerd om binnen normale limieten te zijn met supplementen voorafgaand aan de eerste dosis studiemedicatie:
- Kalium (aanbevolen om problemen met QT en/of ritmeafwijkingen te voorkomen)
- Magnesium (aanbevolen te bewaren om problemen met QT en/of ritmeafwijkingen te voorkomen)
- Totaal calcium (gecorrigeerd voor serumalbumine)
Patiënten moeten een normale beenmergfunctie hebben zoals gedefinieerd:
- Absoluut aantal neutrofielen (ANC) ≥ 1,5 x 10E9/L
- Hemoglobine ≥ 9,0 g/dl
- Bloedplaatjes ≥ 100 x 10E9/L
Gedocumenteerde CML in de chronische fase moet voldoen aan alle criteria die zijn gedefinieerd door:
- < 15% blasten in perifeer bloed en beenmerg,
- < 30% blasten plus promyelocyten in perifeer bloed en beenmerg,
- < 20% basofielen in het perifere bloed,
- ≥ 100 x 109/L (≥ 100.000/mm3) bloedplaatjes,
- Geen bewijs van extramedullaire leukemische betrokkenheid, met uitzondering van hepatosplenomegalie
- Patiënten moeten een minimale totale dagelijkse dosis nilotinib van 400 mg verdragen
Uitsluitingscriteria:
- Eerdere behandeling met andere BCR-ABL-remmers dan nilotinib gedurende meer dan een totale cumulatieve duur van 4 weken
- Eerdere behandeling met alfa-interferon van welke duur dan ook
- Eerdere antikankermiddelen voor CML anders dan nilotinib behalve cytoreductie na CML-diagnose tot maximaal 4 weken na de eerste dosis nilotinib
- Bekende tweede chronische fase van CML na eerdere progressie naar AP/BC
- Slecht gecontroleerde diabetes mellitus (gedefinieerd als HbA1c > 9%)
Verminderde hartfunctie, waaronder een van de volgende:
- LVEF < 45% of onder de institutionele ondergrens van het normale bereik (afhankelijk van welke hoger is)
- Onvermogen om het QT-interval op het ECG te bepalen, behalve bij patiënten met bewijs van een meetbaar QT-interval ten tijde van de CML-diagnose (bijv. voorafgaand aan de eerste start van de TKI-behandeling) en die geen gedocumenteerde klinische tekenen van cardiovasculaire aandoeningen en/of klinische tekenen van geleidingsstoornissen hebben.
- Compleet linker bundeltakblok
- Rechter bundeltakblok plus linker voorste of achterste hemiblok
- Gebruik van een ventriculaire pacemaker
- Congenitaal lang QT-syndroom of een bekende familiegeschiedenis van lang QT-syndroom
- Geschiedenis van of aanwezigheid van klinisch significante ventriculaire of atriale tachyaritmieën
- Klinisch significante bradycardie in rust
- QTc > 450 msec op het gemiddelde van drie seriële basislijn-ECG's (met behulp van de QTcF-formule). Als QTcF > 450 msec en de elektrolyten niet binnen het normale bereik liggen, moeten de elektrolyten worden gecorrigeerd en moet de patiënt opnieuw worden getest op QTc. tijdstip van CML-diagnose (bijv. voorafgaand aan de eerste start van de TKI-behandeling) en die geen gedocumenteerde klinische tekenen van cardiovasculaire aandoeningen en/of klinische tekenen van geleidingsstoornissen hebben.
- Voorgeschiedenis of klinische tekenen van myocardinfarct binnen 1 jaar na aanvang van de studie
- Geschiedenis van onstabiele angina pectoris binnen 1 jaar na aanvang van de studie
- Andere klinisch significante hartaandoeningen (bijv. congestief hartfalen, cardiomyopathie of ongecontroleerde hypertensie)
- Geschiedenis van acute pancreatitis binnen 1 jaar na deelname aan de studie of medische geschiedenis van chronische pancreatitis in het verleden
- Bekende aanwezigheid van significante aangeboren of verworven bloedingsstoornis die geen verband houdt met kanker
- Ernstige en/of ongecontroleerde gelijktijdige medische ziekte die naar de mening van de onderzoeker onaanvaardbare veiligheidsrisico's kan veroorzaken of de naleving van het protocol in gevaar kan brengen (bijv. ongecontroleerde diabetes, ongecontroleerde infectie)
- Geschiedenis van een andere actieve maligniteit binnen 5 jaar voorafgaand aan deelname aan het onderzoek, met uitzondering van eerdere of gelijktijdige basaalcelkanker en eerder in situ curatief behandeld carcinoom
- Behandeling met andere onderzoeksgeneesmiddelen (gedefinieerd als niet gebruikt in overeenstemming met de goedgekeurde indicatie) binnen 4 weken na dag 1
- Patiënten die niet hersteld zijn van een eerdere operatie
- Patiënten die actief worden behandeld met sterke CYP3A4-remmers en/of -inductoren, en de behandeling kan niet worden stopgezet of worden overgeschakeld op een andere medicatie voordat met het onderzoeksgeneesmiddel wordt begonnen.
- Patiënten die actief worden behandeld met kruidengeneesmiddelen die sterke CYP3A4-remmers en/of -inductoren zijn, en de behandeling kan niet worden stopgezet of worden overgeschakeld op een andere medicatie voordat met het onderzoeksgeneesmiddel wordt begonnen. Deze kruidengeneesmiddelen kunnen Echinacea (waaronder E. purpurea, E. angustifolia en E. pallida), piperine, artemisinine, sint-janskruid en ginkgo zijn.
- Patiënten die momenteel worden behandeld met medicijnen die het potentieel hebben om het QT-interval te verlengen en de behandeling kan niet veilig worden stopgezet of worden overgeschakeld op een ander medicijn voordat met het onderzoeksgeneesmiddel wordt begonnen. - Verstoring van de gastro-intestinale (GI) functie of GI-ziekte die de absorptie van het onderzoeksgeneesmiddel significant kan veranderen (bijv. ulceratieve ziekte, ongecontroleerde misselijkheid, braken, diarree, malabsorptiesyndroom, dunnedarmresectie of maagbypassoperatie)
- Zwangere of zogende (zogende) vrouwen, waarbij zwangerschap wordt gedefinieerd als de toestand van een vrouw na de bevruchting en tot het einde van de zwangerschap, bevestigd door een positieve hCG-laboratoriumtest.
Vrouwen in de vruchtbare leeftijd, gedefinieerd als alle vrouwen die fysiologisch in staat zijn om zwanger te worden, tenzij ze zeer effectieve anticonceptiemethoden gebruiken tijdens het onderzoek en gedurende 14 dagen na de laatste dosis nilotinib. Zeer effectieve anticonceptie wordt gedefinieerd als:
- Totale onthouding (wanneer dit in overeenstemming is met de gewenste en gebruikelijke levensstijl van de patiënt. Periodieke onthouding (bijv. kalender-, ovulatie-, symptothermische, post-ovulatiemethoden) en onthouding zijn geen aanvaardbare anticonceptiemethoden
- Vrouwelijke sterilisatie (chirurgische bilaterale ovariëctomie met of zonder hysterectomie ondergaan) of afbinden van de eileiders ten minste zes weken voordat de studiebehandeling wordt gestart. In geval van alleen ovariëctomie, alleen wanneer de reproductieve status van de vrouw is bevestigd door een follow-up hormoonspiegelonderzoek.
- Mannelijke sterilisatie (minstens 6 maanden voorafgaand aan de inschrijving). Voor vrouwelijke patiënten in het onderzoek moet de gesteriliseerde mannelijke partner de enige partner zijn voor die patiënt.
Gebruik van een combinatie van twee van de volgende:
- Gebruik van orale, geïnjecteerde of geïmplanteerde hormonale anticonceptiemethoden of andere vormen van hormonale anticonceptie die vergelijkbare werkzaamheid hebben (faalpercentage <1%), bijvoorbeeld hormonale vaginale ring of transdermale hormonale anticonceptie.
- Plaatsing van een intra-uterien apparaat (IUD) of intra-uterien systeem (IUS)
- Barrièremethoden voor anticonceptie: condoom of occlusiekapje (diafragma- of cervix-/gewelfkapje) met zaaddodend schuim/gel/film/crème/vaginale zetpil.
In het geval van gebruik van orale anticonceptie dienen vrouwen gedurende minimaal 3 maanden stabiel op dezelfde pil te zijn geweest voordat ze de studiebehandeling begonnen Vrouwen worden als postmenopauzaal beschouwd en zijn niet vruchtbaar als ze 12 maanden natuurlijke (spontane) amenorroe hebben gehad met een geschikt klinisch profiel (bijv. geschikt voor de leeftijd, voorgeschiedenis van vasomotorische symptomen) of chirurgische bilaterale ovariëctomie (met of zonder hysterectomie) of afbinden van de eileiders hebben ondergaan ten minste zes weken voorafgaand aan inschrijving. Alleen in het geval van een ovariëctomie wordt de vrouw pas beschouwd als niet vruchtbaar als de reproductieve status van de vrouw is bevestigd door een follow-up van hormoonspiegelonderzoek.
Als een studiepatiënt zwanger wordt of vermoedt zwanger te zijn tijdens de studie of binnen 30 dagen na de laatste dosis nilotinib, moet de onderzoeksarts onmiddellijk worden geïnformeerd en moet de lopende studiebehandeling met nilotinib onmiddellijk worden stopgezet.

Experimenteel: Nilotinib gevolgd door behandelingsvrij
Patiënten die minimaal 2 jaar eerstelijnsbehandeling met nilotinib kregen en met pre-screen PCR-resultaten van ≥ MR4,5 gingen de consolidatiefase van het onderzoek in (52 weken - nilotinib 300 mg tweemaal daags).
Patiënten met Minimal Residual Disease (MRD) aan het einde van deze fase kwamen in de fase van behandeling zonder remissie (TFR), waarin geen behandeling werd gegeven.
Patiënten die niet in aanmerking komen, gaan de vervolgfase van de studie in.
Patiënten met MRD aan het einde van de voortzettingsfase gaan de TFR-2-fase van het onderzoek in waarin geen behandeling wordt gegeven.
Patiënten die niet in aanmerking komen, gaan de verlengde voortzettingsfase van de studie in.
Als de patiënt op enig moment tijdens TFR of TFR-2 MMR verliest, zal de behandeling met nilotinib onmiddellijk opnieuw worden gestart (nilotinib 300 mg tweemaal daags).

Nilotinib wordt gebruikt als in de handel verkrijgbare capsules (behalve in Japan waar klinische benodigdheden worden gebruikt) met een sterkte van 150 mg en 200 mg.
De behandeling vindt plaats tijdens consolidatie-, voortzettings-, verlengde voortzettings-, herstart- en herstart-2-fasen van het onderzoek.
