Estudio mecanicista del efecto de itepekimab sobre la inflamación de las vías respiratorias en pacientes con EPOC (AERIFY-3)
Estudio de fase 2a, abierto, de dos partes para evaluar el mecanismo de acción de itepekimab (anti-IL-33 mAb) sobre la inflamación de las vías respiratorias en pacientes con enfermedad pulmonar obstructiva crónica (EPOC)
Este estudio es un estudio de Fase 2a exploratorio, en dos partes, de 12 semanas de duración para evaluar el mecanismo de acción de itepekimab (anti-IL-33-mAb) y su impacto en la inflamación de las vías respiratorias en fumadores anteriores y actuales con EPOC, de 40 años. a 70 años.
Este estudio consta de participantes que han estado en tratamiento estándar (SoC) mono (agonista β2 de acción prolongada [LABA]) o antagonista muscarínico de acción prolongada [LAMA]), doble (corticosteroide inhalado [ICS] + LABA , LABA + LAMA o ICS + LAMA), o terapia de control triple (ICS + LABA + LAMA) para la EPOC durante al menos 3 meses antes de la Selección (Visita 1) con dosis y régimen estables para la terapia de control durante ≥1 mes antes de la Selección (Visita 1) y durante el período de selección. Los participantes permanecerán con sus medicamentos de control establecidos para la EPOC durante la duración del estudio, con la excepción de los corticosteroides sistémicos y/o los antibióticos utilizados para la exacerbación aguda de la EPOC (AEPOC).
La duración total del estudio para cada parte (Parte A y Parte B) es de aproximadamente 36 semanas:

Criterios de inclusión:
- El participante debe tener entre 40 y 70 años inclusive
- Diagnóstico médico de EPOC durante al menos 1 año (basado en la definición de la Iniciativa Global para la Enfermedad Pulmonar Obstructiva Crónica [GOLD]).
Historial de tabaquismo de ≥10 paquetes-año
- Para exfumadores: los participantes que informan que actualmente no fuman y que el abandono del hábito de fumar debe haber ocurrido ≥6 meses antes de la selección (visita 1) con la intención de dejar de fumar de forma permanente.
- Para fumadores actuales (no elegibles para la Parte A): Participantes que informan que actualmente están fumando tabaco (el participante fumó al menos 5 cigarrillos por día en promedio durante los últimos 7 días) en la selección (visita 1) y en la línea de base, y que están que actualmente no participa ni planea iniciar una intervención para dejar de fumar en la selección (visita 1) o durante el período de selección.
- Antecedentes informados por el participante de signos y síntomas de bronquitis crónica (tos productiva crónica durante al menos 3 meses en el año anterior a la selección en un participante en quien otras causas de tos crónica [p. ej., reflujo gastroesofágico tratado inadecuadamente o rinosinusitis crónica; o diagnóstico clínico de bronquiectasias] han sido excluidos).
Antecedentes de exacerbación documentados o autoinformados que hayan tenido ≥1 exacerbación moderada o grave en los 5 años anteriores a la selección (visita 1), con al menos 1 exacerbación tratada con corticosteroides sistémicos:
- Las exacerbaciones moderadas se definen como un empeoramiento agudo de los síntomas respiratorios que requiere corticosteroides sistémicos (intramusculares [IM], intravenosos [IV] u orales) y/o antibióticos.
- Las exacerbaciones graves se definen como AEPOC que requieren hospitalización u observación durante > 24 horas en el servicio de urgencias/centro de atención de urgencia.
- Participantes tratados con terapia controladora de SoC durante ≥3 meses antes de la selección (visita 1) y con una dosis y un régimen estables de terapia controladora durante al menos 1 mes antes de la visita de selección Y durante el período de selección, que incluye: terapia triple con LAMA + LABA + ICS o doble terapia con ICS + LABA o LABA + LAMA o ICS + LAMA, o monoterapia con LABA o LAMA.
- Participantes que hayan recibido la vacunación adecuada de acuerdo con las recomendaciones locales contra el síndrome respiratorio agudo severo coronavirus 2 (SARS-CoV-2), administrada un mínimo de 1 semana antes de la Selección (Visita 1).
- Índice de masa corporal (IMC) ≥18 kg/m2
Una participante femenina es elegible para participar si no está embarazada, no está amamantando y se aplica al menos una de las siguientes condiciones:
- No es una mujer en edad fértil (WOCBP) o
- Una WOCBP que acepta seguir la guía anticonceptiva durante el período de intervención y durante al menos 20 semanas después de la última dosis de la intervención del estudio.
Criterio de exclusión:
- Diagnóstico actual o diagnóstico previamente confirmado de asma de acuerdo con las pautas de la Iniciativa Global para el Asma (GINA), a menos que el asma se haya resuelto antes de los 18 años de edad y no haya recurrido.
- Para exfumadores (Partes A y B): Fumar activamente o vapear cualquier producto (p. ej., nicotina, tetrahidrocannabinol [THC]) dentro de los 6 meses anteriores a la selección (visita 1) o durante el período de selección. Para fumadores actuales (Parte B): vapeo de cualquier producto (p. ej., nicotina, THC) dentro de los 6 meses anteriores a la selección (visita 1) o durante el período de selección.
- Participantes que se espera que estén expuestos regularmente al humo de tabaco ambiental (es decir, "de segunda mano") en un entorno cerrado durante los períodos de selección o tratamiento (ex fumadores únicamente).
- Nuevo electrocardiograma (ECG) anormal clínicamente significativo dentro de los 6 meses anteriores o en la selección (visita 1) que puede afectar la participación del participante en el estudio.
- Enfermedad pulmonar clínicamente significativa y actual distinta de la EPOC, por ejemplo, sarcoidosis, enfermedad pulmonar intersticial, bronquiectasias (diagnóstico clínico), diagnóstico de deficiencia de antitripsina α1 u otra enfermedad pulmonar diagnosticada.
- Diagnóstico de cor pulmonale, evidencia de insuficiencia cardíaca derecha o hipertensión pulmonar de moderada a grave.
- Participantes que requieren más de 2 L/min de tratamiento a largo plazo con oxígeno en reposo. Pueden inscribirse los participantes que utilicen hasta 4 l/min de oxígeno suplementario durante el ejercicio. Se permite el oxígeno durante el sueño.
- Hipercapnia que requiere presión positiva en las vías respiratorias de dos niveles (BiPAP).
- Exacerbación moderada o grave de la EPOC (AEPOC) en las 8 semanas anteriores a la selección (visita 1) o durante el período de selección.
- Antecedentes de neumonectomía, lobectomía, segmentectomía o procedimiento de broncoscopia terapéutica (incluida la reducción de volumen broncoscópica). Nota: Los antecedentes de biopsia pulmonar quirúrgica o resección en cuña no son criterios de exclusión.
- Cualquier cirugía o procedimiento mayor (incluidos los que requieren sedación consciente) planeado durante el estudio. Se permiten procedimientos menores de la piel.
- Cardiopatía isquémica inestable, incluido infarto agudo de miocardio en el plazo de 1 año antes de la selección (visita 1), o angina inestable en los 6 meses anteriores a la selección (visita 1) o durante el período de selección.
- Arritmias cardíacas, incluida la fibrilación auricular paroxística (p. ej., intermitente). Los participantes con contracciones ventriculares prematuras (PVC) aisladas o contracciones auriculares prematuras (PAC) pueden ser considerados para su inclusión.
- Miocardiopatía, definida como insuficiencia cardíaca en estadio III-IV (Asociación del Corazón de Nueva York) u otro trastorno cardiovascular relevante que pueda afectar la participación del participante en el estudio.
- Cualquier enfermedad subyacente que requiera el uso de profilaxis para la endocarditis.
- Hipertensión no controlada (es decir, presión arterial sistólica [PA] >180 mm Hg o PA diastólica >110 mm Hg con o sin uso de terapia antihipertensiva).
- Participantes con tuberculosis activa (TB), TB latente, antecedentes de TB tratada de forma incompleta, sospecha de infección de TB extrapulmonar (TBI), o que tienen un alto riesgo de contraer TB (como contacto cercano con personas con TB activa o latente) o recibieron Vacunación contra Bacillus Calmette Guérin (BCG) en las 12 semanas anteriores a la selección (visita 1).
- Antecedentes de infección por el virus de la inmunodeficiencia humana (VIH) o serología VIH 1/2 positiva en la selección (visita 1).
- Sospecha o confirmación de infección por enfermedad por coronavirus 2019 (COVID-19) o contacto con exposición conocida a COVID19 en la selección (visita 1) o durante el período de selección; historial conocido de infección por COVID19 dentro de las 6 semanas anteriores a la selección (visita 1); antecedentes de necesidad de ventilación mecánica u oxigenación por membrana extracorpórea (ECMO) secundaria a COVID-19 dentro de los 12 meses anteriores a la selección (visita 1); participantes que han tenido una infección por COVID-19 antes de la selección (visita 1) que aún no se han recuperado lo suficiente para participar en los procedimientos de un ensayo clínico.
- Evidencia de infección aguda o crónica que requiere tratamiento sistémico con medicamentos antibacterianos, antivirales, antifúngicos, antiparasitarios o antiprotozoarios dentro de las 6 semanas anteriores a la Selección (Visita 1) o durante el período de selección, infecciones virales significativas dentro de las 6 semanas anteriores a la Selección (Visita 1) o durante el período de detección que puede no haber sido tratado con tratamiento antiviral (p. ej., influenza que recibe solo tratamiento sintomático).
- Participantes con enfermedad autoinmune activa o participantes que reciben terapia inmunosupresora para enfermedad autoinmune (p. ej., artritis reumatoide, enfermedad inflamatoria intestinal, cirrosis biliar primaria, lupus eritematoso sistémico, esclerosis múltiple).
- Antecedentes de malignidad en los 5 años anteriores a la selección (visita 1), o durante el período de selección, excepto el carcinoma de cuello uterino in situ completamente tratado y el carcinoma de células escamosas o basocelulares no metastásico completamente tratado y resuelto.
- Herpes zóster sintomático en los 3 meses anteriores a la selección.
- Uso previo de itepekimab.
