El objetivo de este ensayo clínico piloto es comparar dos intervenciones conductuales para pacientes que se están recuperando de una cirugía de columna lumbar por una afección degenerativa de la columna. El estudio pretende dar respuesta a las siguientes preguntas:
- ¿Es factible y aceptable realizar las intervenciones y realizar los procedimientos del estudio en esta población de pacientes?
- ¿Las intervenciones benefician a los pacientes con respecto a los resultados relacionados con el dolor, funcionales, del uso de opioides y psicosociales a los 3 y 6 meses después de la cirugía, y una intervención tiene más beneficios que la otra?
Se pedirá a los participantes que hagan lo siguiente:
- Complete cuestionarios de autoinforme en línea antes de la cirugía de columna y alrededor de 2 semanas, 3 meses y 6 meses después de la cirugía de columna.
- Complete una tarea de dolor sensorial antes de la cirugía de columna y alrededor de 2 semanas, 3 meses y 6 meses después de la cirugía de columna.
- Alrededor de dos semanas después de la cirugía, los participantes serán asignados aleatoriamente a una de dos intervenciones conductuales: atención plena o educación. Luego, se les pedirá a los participantes que asistan a 8 sesiones semanales con un entrenador de tratamiento del estudio a través de telesalud (en línea con cámara y micrófono).

Este es un ensayo clínico aleatorizado piloto que compara una intervención basada en atención plena (MBI) de telesalud con una intervención educativa de telesalud (EDU) para pacientes que se recuperan de una cirugía de columna. Los procedimientos serán los siguientes:
- Inscriba a los pacientes antes de la cirugía de la columna lumbar por una afección degenerativa de la columna en el Vanderbilt Comprehensive Spine Center. El reclutamiento, la inscripción y la evaluación se realizarán en una visita clínica preoperatoria de rutina o por teléfono.
- Dos semanas después de que los participantes inscritos completen la cirugía, serán aleatorizados en una proporción de 3:2 a la condición MBI o EDU, respectivamente. La aleatorización se estratificará según el uso preoperatorio crónico de opioides (sí/no). Las asignaciones de aleatorización permanecerán ocultas para todo el personal del estudio hasta el momento de la aleatorización. El evaluador de resultados y los proveedores quirúrgicos permanecerán cegados a la condición del estudio de los participantes.
- Dentro de las cuatro semanas posteriores a la cirugía, los participantes comenzarán la intervención asignada. Las intervenciones consistirán en reunirse con un intervencionista durante ocho sesiones semanales individuales de MBI de telesalud a través de una plataforma de telesalud en línea. Las sesiones de MBI estarán dirigidas por un psicólogo clínico y tendrán una duración de 75 minutos (90 minutos para la primera sesión). Las sesiones de EDU estarán dirigidas por un fisioterapeuta y tendrán una duración de 30 minutos. Las sesiones serán grabadas en audio y monitoreadas para asegurar su fidelidad.
3) Los participantes completarán evaluaciones de autoinforme en línea antes de la cirugía, dos semanas después de la cirugía (antes de la intervención), tres meses después de la cirugía (posterior a la intervención) y 6 meses después de la cirugía.
4) Los participantes completarán evaluaciones sensoriales cuantitativas de tolerancia al dolor, umbral del dolor y resumen temporal del dolor antes de la cirugía, dos semanas después de la cirugía, tres meses después de la cirugía y 6 meses después de la cirugía.

Descripción
Criterios de inclusión:
- adultos de habla inglesa
- Mayores de 18 años
- Programado para una laminectomía y/o fusión en Vanderbilt Spine Center
- Programado para su primera cirugía de columna lumbar
- Evidencia radiográfica de una afección degenerativa que incluye, entre otros, estenosis espinal, espondilosis con o sin mielopatía y espondilolistesis
- Presencia de dolor en la espalda y/o en las extremidades inferiores que persista durante al menos 3 meses
- Acceso a internet estable.
- Capaz de participar en sesiones remotas semanales con un terapeuta del estudio durante 8 semanas después de la cirugía
Criterio de exclusión:
- Técnica microquirúrgica como procedimiento primario (es decir, laminotomía aislada o microdiscectomía)
- Someterse a una cirugía por la indicación principal de una deformidad de la columna
- Someterse a una cirugía secundaria a pseudoartrosis, traumatismo, infección o tumor
- Actual/antecedentes de un psicótico primario o trastorno mayor del pensamiento u hospitalización por motivos relacionados con la psicosis (p. esquizofrenia, trastorno psicótico breve, trastorno delirante). La psicosis inducida por medicamentos no es excluyente.
- Diagnóstico de la enfermedad de Alzheimer u otra forma de demencia
- Lesión cerebral traumática (gravedad mayor que leve)
- Antecedentes de trastorno bipolar o trastorno disociativo
- Trastorno por consumo de sustancias activas (en el último mes)
- Síntomas actuales del trastorno de estrés postraumático (TEPT) (en el último mes)
- Ideación suicida activa con intención

El MBI de telesalud está adaptado de la terapia cognitiva basada en Mindfulness para el dolor crónico y consta de ocho sesiones individuales semanales de 75 minutos (90 minutos asignados para la primera sesión) con un interventor de atención plena que brinda la intervención a través de telesalud (en línea con cámara de audio y video). ) además de que el paciente reciba sus cuidados posquirúrgicos habituales.
Los participantes comenzarán la intervención después de completar la evaluación posoperatoria de 2 semanas y dentro de las 4 semanas posteriores a la cirugía.
Se les pedirá a los participantes que practiquen las habilidades entre sesiones aproximadamente 20 a 40 minutos por día.

Pain intensity was measured on a scale of 0 to 100.
The difference score was calculated by subtracting the pain intensity with single prick from the pain intensity with repeated pricks and could range from 0 to 100 units of difference on the pain intensity scale.
Lower values are a more favorable outcome.
