Intervento postoperatorio di consapevolezza della telemedicina dopo la chirurgia della colonna lombare: studio pilota controllato randomizzato
L'obiettivo di questo studio clinico pilota è confrontare due interventi comportamentali per pazienti che si stanno riprendendo da un intervento chirurgico spinale lombare per una condizione degenerativa della colonna vertebrale. Lo studio si propone di rispondere alle seguenti domande:
- È fattibile e accettabile fornire gli interventi e condurre le procedure dello studio in questa popolazione di pazienti?
- Gli interventi apportano benefici ai pazienti per quanto riguarda il dolore, la funzionalità, l'uso di oppioidi e gli esiti psicosociali a 3 e 6 mesi dopo l'intervento chirurgico e un intervento ha più benefici dell'altro?
Ai partecipanti verrà chiesto di fare quanto segue:
- Completa i questionari di autovalutazione online prima dell'intervento chirurgico alla colonna vertebrale e circa 2 settimane, 3 mesi e 6 mesi dopo l'intervento chirurgico alla colonna vertebrale.
- Completare un'attività di dolore sensoriale prima dell'intervento chirurgico alla colonna vertebrale e circa 2 settimane, 3 mesi e 6 mesi dopo l'intervento chirurgico alla colonna vertebrale.
- Circa due settimane dopo l'intervento chirurgico, i partecipanti verranno assegnati in modo casuale a uno dei due interventi comportamentali: consapevolezza o educazione. Ai partecipanti verrà quindi chiesto di partecipare a 8 sessioni settimanali con un coach del trattamento dello studio tramite telemedicina (online con videocamera e microfono).

Questo è uno studio clinico pilota randomizzato che confronta un intervento basato sulla consapevolezza della telemedicina (MBI) con un intervento di educazione alla telemedicina (EDU) per i pazienti che si stanno riprendendo da un intervento chirurgico alla colonna vertebrale. Le procedure saranno le seguenti:
- Arruolare i pazienti prima della chirurgia della colonna lombare per una condizione degenerativa della colonna vertebrale presso il Vanderbilt Comprehensive Spine Center. Il reclutamento, l'arruolamento e lo screening avverranno durante una visita clinica preoperatoria di routine o per telefono.
- Due settimane dopo che i partecipanti arruolati hanno completato l'intervento chirurgico, saranno randomizzati in un rapporto 3: 2 rispettivamente alla condizione MBI o EDU. La randomizzazione sarà stratificata per uso cronico preoperatorio di oppioidi (sì/no). Gli incarichi di randomizzazione rimarranno nascosti a tutto il personale dello studio fino al momento della randomizzazione. Il valutatore dei risultati e gli operatori chirurgici rimarranno all'oscuro delle condizioni dello studio dei partecipanti.
- Entro quattro settimane dall'intervento, i partecipanti inizieranno l'intervento assegnato. Gli interventi consisteranno nell'incontro con un interventista per otto sessioni MBI settimanali individuali di telemedicina su una piattaforma di telemedicina online. Le sessioni MBI saranno guidate da uno psicologo clinico e dureranno 75 minuti (90 minuti per la prima sessione). Le sessioni EDU saranno condotte da un fisioterapista e dureranno 30 minuti. Le sessioni saranno audio registrate e monitorate per la fedeltà.
3) I partecipanti completeranno le valutazioni di autovalutazione online prima dell'intervento, due settimane dopo l'intervento (pre-intervento), tre mesi dopo l'intervento (post-intervento) e 6 mesi dopo l'intervento.
4) I partecipanti completeranno le valutazioni sensoriali quantitative della tolleranza al dolore, della soglia del dolore e della somma temporale del dolore prima dell'intervento, due settimane dopo l'intervento, tre mesi dopo l'intervento e 6 mesi dopo l'intervento.

Descrizione
Criterio di inclusione:
- Adulti di lingua inglese
- Dai 18 anni in su
- Programmato per una laminectomia e/o fusione al Vanderbilt Spine Center
- In programma per il loro primo intervento chirurgico alla colonna lombare
- Evidenza radiografica di una condizione degenerativa inclusa ma non limitata a stenosi spinale, spondilosi con o senza mielopatia e spondilolistesi
- Presenza di dolore alla schiena e/o agli arti inferiori persistente per almeno 3 mesi
- Accesso a Internet stabile.
- In grado di partecipare a sessioni settimanali a distanza con un terapista dello studio per 8 settimane dopo l'intervento chirurgico
Criteri di esclusione:
- Tecnica microchirurgica come procedura primaria (es. laminotomia isolata o microdiscectomia)
- Sottoporsi a un intervento chirurgico per l'indicazione primaria di una deformità spinale
- Avere un intervento chirurgico secondario a pseudoartrosi, trauma, infezione o tumore
- Attuale/storia di uno psicotico primario o disturbo del pensiero maggiore o ricovero in ospedale per motivi correlati alla psicosi (ad es. schizofrenia, disturbo psicotico breve, disturbo delirante). La psicosi indotta da farmaci non è esclusiva.
- Diagnosi della malattia di Alzheimer o di un'altra forma di demenza
- Lesione cerebrale traumatica (gravità superiore a lieve)
- Storia di disturbo bipolare o disturbo dissociativo
- Disturbo da uso di sostanze attive (nell'ultimo mese)
- Sintomi attuali del disturbo da stress post-traumatico (PTSD) (nell'ultimo mese)
- Ideazione suicidaria attiva con intento

L'MBI di telemedicina è adattato dalla terapia cognitiva basata sulla consapevolezza per il dolore cronico e consiste in otto sessioni individuali settimanali di 75 minuti (90 minuti assegnati per la prima sessione) con un interventista di consapevolezza che fornisce l'intervento tramite telemedicina (online con audio e videocamera ) in aggiunta al paziente che riceve le consuete cure postoperatorie.
I partecipanti inizieranno l'intervento dopo aver completato la valutazione postoperatoria di 2 settimane ed entro 4 settimane dall'intervento.
Ai partecipanti verrà chiesto di esercitare le abilità tra le sessioni di circa 20-40 minuti al giorno.

I partecipanti riceveranno un opuscolo educativo scritto al momento della randomizzazione (entro 2-4 settimane dopo l'intervento) e 8 sessioni settimanali di telemedicina della durata di 30 minuti in aggiunta alle consuete cure postoperatorie.
I materiali didattici vengono rivisti individualmente con il partecipante durante le sessioni settimanali di telemedicina (online con audio e videocamera) da un fisioterapista.

Using a punctate stimuli (von frey hairs of 100g and 180g of force to the finger), the difference between reported pain intensity during a single prick and the reported highest pain intensity during repeated brief pricks at a constant intensity.
Pain intensity was measured on a scale of 0 to 100.
The difference score was calculated by subtracting the pain intensity with single prick from the pain intensity with repeated pricks and could range from 0 to 100 units of difference on the pain intensity scale.
Lower values are a more favorable outcome.
