Telemedizin-Achtsamkeit nach einer Wirbelsäulenoperation
Postoperative Telemedizin-Achtsamkeitsintervention nach einer Lendenwirbelsäulenoperation: Randomisierte kontrollierte Pilotstudie
Das Ziel dieser klinischen Pilotstudie ist es, zwei Verhaltensinterventionen für Patienten zu vergleichen, die sich von einer Lendenwirbelsäulenoperation wegen einer degenerativen Wirbelsäulenerkrankung erholen. Die Studie soll folgende Fragen beantworten:
- Ist es machbar und akzeptabel, die Interventionen durchzuführen und die Studienverfahren bei dieser Patientenpopulation durchzuführen?
- Nutzen die Interventionen den Patienten im Hinblick auf schmerzbezogene, funktionelle, opioide und psychosoziale Ergebnisse 3 und 6 Monate nach der Operation, und hat eine Intervention mehr Nutzen als die andere?
Die Teilnehmer werden gebeten, Folgendes zu tun:
- Füllen Sie vor der Wirbelsäulenoperation und etwa 2 Wochen, 3 Monate und 6 Monate nach der Wirbelsäulenoperation Online-Fragebögen zur Selbsteinschätzung aus.
- Führen Sie eine sensorische Schmerzaufgabe vor einer Wirbelsäulenoperation und etwa 2 Wochen, 3 Monate und 6 Monate nach einer Wirbelsäulenoperation durch.
- Etwa zwei Wochen nach der Operation werden die Teilnehmer nach dem Zufallsprinzip einer von zwei Verhaltensinterventionen zugeteilt – Achtsamkeit oder Bildung. Die Teilnehmer werden dann gebeten, an 8 wöchentlichen Sitzungen mit einem Studienbehandlungscoach über Telemedizin (online mit Kamera und Mikrofon) teilzunehmen.

Detaillierte Beschreibung
Dies ist eine randomisierte klinische Pilotstudie, die eine auf Achtsamkeit basierende telemedizinische Intervention (MBI) mit einer telemedizinischen Aufklärungsintervention (EDU) für Patienten vergleicht, die sich von einer Wirbelsäulenoperation erholen. Die Verfahren werden wie folgt sein:
- Melden Sie Patienten vor einer Lendenwirbelsäulenoperation wegen einer degenerativen Wirbelsäulenerkrankung im Vanderbilt Comprehensive Spine Center an. Rekrutierung, Registrierung und Screening erfolgen bei einem routinemäßigen präoperativen Klinikbesuch oder per Telefon.
- Zwei Wochen nach Abschluss der Operation werden die eingeschriebenen Teilnehmer in einem Verhältnis von 3:2 randomisiert entweder dem MBI- oder dem EDU-Zustand zugeteilt. Die Randomisierung wird nach chronischer präoperativer Opioidanwendung (ja/nein) stratifiziert. Die Randomisierungsaufträge bleiben bis zum Zeitpunkt der Randomisierung dem gesamten Studienpersonal verborgen. Der Ergebnisbewerter und die chirurgischen Anbieter bleiben gegenüber dem Zustand der Studienteilnehmer verblindet.
- Innerhalb von vier Wochen nach der Operation beginnen die Teilnehmer mit der zugewiesenen Intervention. Die Interventionen bestehen aus einem Treffen mit einem Interventionisten für acht wöchentliche MBI-Einzelsitzungen über Telemedizin über eine Online-Telemedizinplattform. MBI-Sitzungen werden von einem klinischen Psychologen geleitet und dauern 75 Minuten (90 Minuten für die erste Sitzung). EDU-Sitzungen werden von einem Physiotherapeuten geleitet und dauern 30 Minuten. Die Sitzungen werden mit Audio aufgezeichnet und auf Wiedergabetreue überwacht.
3) Die Teilnehmer füllen vor der Operation, zwei Wochen nach der Operation (Prä-Intervention), drei Monate nach der Operation (Post-Intervention) und 6 Monate nach der Operation online Selbstauskünfte aus.
4) Die Teilnehmer werden vor der Operation, zwei Wochen nach der Operation, drei Monate nach der Operation und 6 Monate nach der Operation quantitative sensorische Testbewertungen der Schmerztoleranz, der Schmerzschwelle und der zeitlichen Summe der Schmerzen durchführen.

Einschlusskriterien:
- Englisch sprechende Erwachsene
- Ab 18 Jahren
- Geplant für eine Laminektomie und/oder Fusion im Vanderbilt Spine Center
- Geplant für ihre erste Operation an der Lendenwirbelsäule
- Röntgennachweis eines degenerativen Zustands, einschließlich, aber nicht beschränkt auf Spinalkanalstenose, Spondylose mit oder ohne Myelopathie und Spondylolisthese
- Vorhandensein von Rücken- und/oder Schmerzen in den unteren Extremitäten, die mindestens 3 Monate andauern
- Zugriff auf stabiles Internet.
- Kann 8 Wochen nach der Operation an wöchentlichen Fernsitzungen mit einem Studientherapeuten teilnehmen
Ausschlusskriterien:
- Mikrochirurgische Technik als primäres Verfahren (d. h. isolierte Laminotomie oder Mikrodiskektomie)
- Eine Operation wegen der primären Indikation einer Wirbelsäulendeformität
- Operation infolge einer Pseudarthrose, eines Traumas, einer Infektion oder eines Tumors
- Aktuelle/Vorgeschichte einer primären psychotischen oder schweren Denkstörung oder Krankenhausaufenthalt aus Gründen, die mit einer Psychose zusammenhängen (z. Schizophrenie, kurzzeitige psychotische Störung, wahnhafte Störung). Eine medikamenteninduzierte Psychose ist nicht ausschließend.
- Diagnose der Alzheimer-Krankheit oder einer anderen Form der Demenz
- Traumatische Hirnverletzung (größer als leichter Schweregrad)
- Vorgeschichte einer bipolaren Störung oder dissoziativen Störung
- Störung des Wirkstoffkonsums (im vergangenen Monat)
- Aktuelle Symptome einer posttraumatischen Belastungsstörung (PTBS) (im vergangenen Monat)
- Aktive Selbstmordgedanken mit Absicht

Experimental: Telemedizinische achtsamkeitsbasierte Intervention (MBI)

Das Telemedizin-MBI ist eine Adaption der auf Achtsamkeit basierenden kognitiven Therapie für chronische Schmerzen und besteht aus acht wöchentlichen 75-minütigen Einzelsitzungen (90 Minuten für die erste Sitzung) mit einem Achtsamkeitsinterventionisten, der die Intervention über Telemedizin (online mit Audio- und Videokamera) durchführt ) zusätzlich zu der üblichen postoperativen Versorgung des Patienten.
Die Teilnehmer beginnen mit der Intervention nach Abschluss der 2-wöchigen postoperativen Bewertung und innerhalb von 4 Wochen nach der Operation.
Die Teilnehmer werden gebeten, ihre Fähigkeiten zwischen den Sitzungen etwa 20-40 Minuten pro Tag zu üben.

Die Teilnehmer erhalten zum Zeitpunkt der Randomisierung (innerhalb von 2-4 Wochen nach der Operation) eine schriftliche Schulungsbroschüre und 8 wöchentliche Telemedizinsitzungen von 30 Minuten zusätzlich zur üblichen postoperativen Betreuung.
Die Schulungsmaterialien werden während wöchentlicher Telemedizinsitzungen (online mit Audio- und Videokamera) von einem Physiotherapeuten individuell mit dem Teilnehmer überprüft.

Pain intensity was measured on a scale of 0 to 100.
The difference score was calculated by subtracting the pain intensity with single prick from the pain intensity with repeated pricks and could range from 0 to 100 units of difference on the pain intensity scale.
Lower values are a more favorable outcome.
