Telehealth Mindfulness po operaci páteře
Pooperační intervence telehealth všímavosti po operaci bederní páteře: pilotní randomizovaná kontrolovaná studie
Cílem této pilotní klinické studie je porovnat dvě behaviorální intervence u pacientů, kteří se zotavují po operaci bederní páteře pro degenerativní stav páteře. Cílem studie je odpovědět na následující otázky:
- Je možné a přijatelné provádět intervence a provádět postupy studie u této populace pacientů?
- Jsou intervence přínosné pro pacienty s ohledem na bolest související, funkční, užívání opiátů a psychosociální výsledky 3 a 6 měsíců po operaci a má jeden zákrok větší přínos než druhý?
Účastníci budou požádáni, aby provedli následující:
- Před operací páteře a přibližně 2 týdny, 3 měsíce a 6 měsíců po operaci páteře vyplňte online dotazníky s vlastní zprávou.
- Dokončete úkol senzorické bolesti před operací páteře a přibližně 2 týdny, 3 měsíce a 6 měsíců po operaci páteře.
- Přibližně dva týdny po operaci budou účastníci náhodně rozděleni do jedné ze dvou behaviorálních intervencí – všímavost nebo výchova. Účastníci budou poté požádáni, aby se zúčastnili 8 týdenních sezení s koučem studijní léčby přes telehealth (online s kamerou a mikrofonem).

Jedná se o pilotní randomizovanou klinickou studii, která srovnává intervenci na základě telehealth mindfulness-based intervence (MBI) s telehealth vzdělávací intervencí (EDU) u pacientů zotavujících se po operaci páteře. Postupy budou následující:
- Zapište pacienty před operací bederní páteře pro degenerativní stav páteře do Vanderbilt Comprehensive Spine Center. Nábor, registrace a screening proběhnou při běžné předoperační návštěvě kliniky nebo po telefonu.
- Dva týdny poté, co zapsaní účastníci dokončí operaci, budou randomizováni v poměru 3:2 podle stavu MBI nebo EDU. Randomizace bude stratifikována podle chronického předoperačního užívání opioidů (ano/ne). Randomizační přiřazení zůstanou skryta všem zaměstnancům studie až do doby randomizace. Hodnotitel výsledků a poskytovatelé chirurgických zákroků zůstanou zaslepeni vůči stavu účastníka studie.
- Do čtyř týdnů po operaci zahájí účastníci přidělenou intervenci. Intervence budou sestávat ze setkání s intervencí na osmi týdenních individuálních telehealth MBI sezeních přes online platformu telehealth. MBI sezení povede klinický psycholog a bude trvat 75 minut (90 minut na první sezení). EDU sezení povede fyzioterapeut a bude trvat 30 minut. Relace budou nahrávány a sledovány z hlediska věrnosti.
3) Účastníci vyplní vlastní hodnocení online před operací, dva týdny po operaci (před intervencí), tři měsíce po operaci (po intervenci) a 6 měsíců po operaci.
4) Účastníci dokončí kvantitativní senzorické testování tolerance bolesti, prahu bolesti a časové shrnutí bolesti před operací, dva týdny po operaci, tři měsíce po operaci a 6 měsíců po operaci.

Popis
Kritéria pro zařazení:
- Anglicky mluvící dospělí
- Ve věku 18 let a starší
- Naplánováno na laminektomii a/nebo fúzi ve Vanderbilt Spine Center
- Naplánováno na jejich první operaci bederní páteře
- Rentgenový důkaz degenerativního stavu včetně, ale bez omezení, spinální stenózy, spondylózy s myelopatií nebo bez myelopatie a spondylolistézy
- Přítomnost bolesti zad a/nebo dolních končetin trvající nejméně 3 měsíce
- Přístup ke stabilnímu internetu.
- Schopnost účastnit se týdenních, vzdálených sezení se studijním terapeutem po dobu 8 týdnů po operaci
Kritéria vyloučení:
- Mikrochirurgická technika jako primární výkon (tj. izolovaná laminotomie nebo mikrodiscektomie)
- Operace pro primární indikaci deformity páteře
- Operace sekundární k pseudoartróze, traumatu, infekci nebo nádoru
- Současná/anamnéza primární psychotické nebo závažné poruchy myšlení nebo hospitalizace z důvodů souvisejících s psychózou (např. Schizofrenie, krátkodobá psychotická porucha, porucha s bludy). Psychóza vyvolaná léky není výjimkou.
- Diagnóza Alzheimerovy choroby nebo jiné formy demence
- Traumatické poranění mozku (větší než mírné závažnosti)
- Anamnéza bipolární poruchy nebo disociativní poruchy
- Porucha užívání účinných látek (za poslední měsíc)
- Aktuální příznaky posttraumatické stresové poruchy (PTSD) (za poslední měsíc)
- Aktivní sebevražedné myšlenky s úmyslem

Experimentální: Intervence založená na telehealth mindfulness (MBI)
Osm týdenních relací telehealth všímavosti poskytovaných jeden na jednoho s instruktorem všímavosti

Telehealth MBI je adaptováno z kognitivní terapie chronické bolesti založené na všímavosti a skládá se z osmi týdenních 75minutových individuálních sezení (90 minut vyhrazených pro první sezení) s intervencí pro všímavost, která poskytuje intervenci přes telehealth (online pomocí audio a video kamery ) kromě toho, že se pacientovi dostává obvyklé pooperační péče.
Účastníci zahájí intervenci po dokončení 2týdenního pooperačního hodnocení a do 4 týdnů po operaci.
Účastníci budou požádáni, aby procvičovali dovednosti mezi sezeními přibližně 20-40 minut denně.

Účastníci obdrží písemnou edukační brožuru v době randomizace (během 2-4 týdnů po operaci) a 8 týdenních telehealth sezeních trvajících 30 minut navíc k obvyklé pooperační péči.
Vzdělávací materiály jsou s účastníkem individuálně prověřovány během týdenních sezení telehealth (online pomocí audio a video kamery) fyzioterapeutem.

Using a punctate stimuli (von frey hairs of 100g and 180g of force to the finger), the difference between reported pain intensity during a single prick and the reported highest pain intensity during repeated brief pricks at a constant intensity.
Pain intensity was measured on a scale of 0 to 100.
The difference score was calculated by subtracting the pain intensity with single prick from the pain intensity with repeated pricks and could range from 0 to 100 units of difference on the pain intensity scale.
Lower values are a more favorable outcome.
