Yksityiskohtainen kuvaus
Kohonnut verenpaine (BP) on yleisin aivohalvauksen aiheuttaja sekä muiden sydän- ja verisuonisairauksien, kuten sepelvaltimotaudin ja sydämen vajaatoiminnan, riskitekijä ihmisillä, joilla on diabetes tai ei. Strategioita, jotka alentavat tehokkaasti verenpainetta, ovat lääkehoito ja elämäntapojen muuttaminen. Elämäntapamuutosten, erityisesti ruokavalion lähestymistapojen, on osoitettu alentavan verenpainetta ihmisillä, joilla ei ole diabetesta. Verenpaineen alentamisesta ja elämäntapojen muutoksista, paitsi laihduttamisesta, diabeetikoilla on kuitenkin silmiinpistävä pula. DASH4D-kokeilu on suunniteltu tarjoamaan tämä todiste.
DASH4D-koe perustuu tutkijoiden kokemukseen neljästä NIH:n tukemasta ruokintatutkimuksesta (DASH, DASH-Sodium, OmniHeart ja OmniCarb). Tutkimukseen otetaan mukaan noin 100 aikuista, joilla on tyypin 2 diabetes, systolinen verenpaine 120-159 mmHg ja diastolinen verenpaine.
Ydinsuunnittelu on neljän jakson, yhden paikan risteyttävä ruokintatutkimus, jossa on 5 viikon jaksot. Osallistujille ruokitaan neljä isokalorista ruokavaliota, jotka on esitetty satunnaisessa järjestyksessä:
- DASH4D-ruokavalio, jossa on vähemmän natriumia
- DASH4D-ruokavalio, jossa on enemmän natriumia
- vertailuruokavalio, jossa on vähemmän natriumia
- vertaa ruokavaliota korkeamman natriumin kanssa.
Ensisijainen kiinnostava kontrasti on vähemmän natriumia sisältävä DASH4D-ruokavalio verrattuna enemmän natriumia sisältävään vertailuruokavalioon.
DASH4D-ruokavalio on samanlainen kuin alkuperäinen DASH-ruokavalio, mutta siinä on vähemmän hiilihydraatteja ja enemmän tyydyttymättömiä rasvoja kuin alkuperäinen DASH-ruokavalio, ja siksi se vastaa paremmin diabetesta sairastavien ruokavaliosuosituksia kuin alkuperäinen DASH-ruokavalio. Vertailuruokavalio heijastaa sitä, mitä monet diabeetikot tällä hetkellä kuluttavat. Pienen natriumsaannin, noin 1500 mg/vrk (2000 kcal), on osoitettu alentavan verenpainetta henkilöillä, joilla ei ole diabetesta, ja sitä suositellaan joissakin ravitsemussuosituksissa. Suurempi natriumin saanti, noin 3700 mg/vrk (2000 kcal), perustuu arvioituun keskimääräiseen saantiin Yhdysvalloissa.
Tulokset mitataan jokaisen ruokintajakson lopussa. Ensisijainen tulos on jakson lopun, toimistopohjainen systolinen verenpaine. Muita tuloksia ovat diastolinen verenpaine, glykemian mittaukset, plasman lipidiriskitekijät, potilaan oireet ja arvioitu sydän- ja verisuonitautiriski.
Kuten tutkijoiden aikaisemmat ruokintatutkimukset, tutkijat odottavat, että DASH4D-tutkimuksen tulokset ovat välittömästi sovellettavissa kansanterveyteen ja kliinisiin ohjeisiin ja vaikuttavat ravitsemuspolitiikkaan. Lisäksi koe tarjoaa tiukan alustan arvioida ruokavalion ja natriumin saannin vaikutusta moniin muihin tuloksiin tyypin 2 diabetesta sairastavilla henkilöillä.

Kuvaus
SISÄLLYTTÄMISKRITEERIT:
- Ikä 18 tai vanhempi
- Tyypin 2 diabetes, jonka HbA1c on ≥6,5 %, tai diabeteksen hoito diabeteslääkkeillä
- Systolinen verenpaine lähtötilanteessa 120-159 mmHg (perustuu kolmen seulontakäynnin keskiarvoon)
- Diastolinen verenpaine lähtötilanteessa
- Halukkaat ja pystyvät syömään paikan päällä yhden aterian päivässä, 3 päivänä viikossa ja syömään vain ja kaikkea tutkimusruokavalioiden osana tarjottua ruokaa kontrolloitujen ruokintajaksojen aikana (sisäänajo ja neljä 5 viikon ruokintajaksoa). Huomaa, että todellinen ruokailutiheys paikan päällä voi olla alle 3 päivää viikossa COVID-rajoitusten vuoksi, mutta osallistujien on silti oltava paikan päällä hakemassa ruokaa ja punnittava 3 päivää viikossa, ja heitä odotetaan silti. aterioiden seuranta (henkilökohtaisesti tai etänä) kerran päivässä, 3 päivänä viikossa.

Lääkkeiden poissulkemiset

- Verenpainelääkkeet
- Natrium-glukoosi-kokuljettaja 2:n (SGLT2) estäjät tai glukagonin kaltaiset peptidi-1 (GLP-1) -reseptoriagonistit
- Stimulantit, mukaan lukien suun kautta otettavat lääkkeet astmaan tai krooniseen obstruktiiviseen keuhkosairauteen (COPD)
- Hormonikorvaushoito tai kilpirauhashormoni

Lääkehistorian poikkeukset
- Tyypin 1 diabetes

After the participant has rested quietly in the seated position for at least 5 minutes, blood pressure will be measured in triplicate, using an Omron device, with each reading separated by 30 seconds, and the average of the three readings (in mmHg) will also be recorded.
Blood pressure will be measured on five separate occasions during the last 2 weeks of the feeding period, and the mean systolic blood pressure from these five blood pressures (15 total readings) will be used as the primary outcome.

Participants will be asked to rate each listed symptom as: did not occur, mild, moderate, or severe.
