Description détaillée
Chez les personnes diabétiques et non diabétiques, l'hypertension artérielle (TA) est la principale cause d'accident vasculaire cérébral et un facteur de risque majeur pour d'autres maladies cardiovasculaires, notamment les maladies coronariennes et l'insuffisance cardiaque. Les stratégies qui abaissent efficacement la TA comprennent la pharmacothérapie et la modification du mode de vie. Il a été démontré que les modifications du mode de vie, en particulier les approches diététiques, abaissent la tension artérielle chez les personnes non diabétiques. Cependant, il existe un manque frappant de preuves sur la baisse de la tension artérielle et les modifications du mode de vie, autres que la perte de poids, chez les personnes atteintes de diabète. L'essai DASH4D est conçu pour fournir ces preuves.
L'essai DASH4D s'appuie sur l'expérience des chercheurs dans quatre études d'alimentation parrainées par les NIH (DASH, DASH-Sodium, OmniHeart et OmniCarb). L'essai recrutera environ 100 adultes atteints de diabète de type 2, de TA systolique de 120 à 159 mmHg et de TA diastolique
La conception de base est une étude d'alimentation croisée à quatre périodes, à site unique, avec des périodes de 5 semaines. Les participants reçoivent chacun des quatre régimes isocaloriques, présentés dans un ordre aléatoire :
- Régime DASH4D avec moins de sodium
- Régime DASH4D avec plus de sodium
- régime de comparaison à faible teneur en sodium
- régime de comparaison avec plus de sodium.
Le principal contraste d'intérêt est le régime DASH4D avec une faible teneur en sodium par rapport au régime de comparaison avec une teneur plus élevée en sodium.
Le régime DASH4D est similaire au régime DASH d'origine, mais contient moins de glucides et plus de graisses insaturées que le régime DASH d'origine et, par conséquent, est plus conforme aux recommandations diététiques pour les personnes atteintes de diabète que le régime DASH d'origine. Le régime de comparaison reflète ce que de nombreuses personnes atteintes de diabète consomment actuellement. Il a été démontré qu'un apport réduit en sodium d'environ 1 500 mg/jour (à 2 000 kcal) abaisse la tension artérielle chez les personnes non diabétiques et a été recommandé dans certaines directives diététiques. L'apport en sodium plus élevé d'environ 3700 mg/jour (à 2000 kcal) est basé sur l'apport moyen estimé aux États-Unis.
Les résultats sont mesurés à la fin de chaque période d'alimentation. Le critère de jugement principal est la TA systolique en fin de période, en cabinet. Les autres résultats sont la PA diastolique, les mesures de la glycémie, les facteurs de risque des lipides plasmatiques, les symptômes du patient et le risque estimé de maladie cardiovasculaire.
À l'instar des études d'alimentation antérieures des chercheurs, les chercheurs s'attendent à ce que les résultats de l'essai DASH4D soient immédiatement applicables aux directives cliniques et de santé publique et influencent la politique nutritionnelle. En outre, l'essai fournira une plate-forme rigoureuse pour évaluer l'impact de l'alimentation et de l'apport en sodium sur un large éventail d'autres résultats chez les personnes atteintes de diabète de type 2.

La description
CRITÈRE D'INTÉGRATION:
- 18 ans ou plus
- Diabète sucré de type 2 défini par HbA1c ≥ 6,5 % ou traitement du diabète avec des médicaments contre le diabète
- TA systolique de base de 120-159 mmHg (basée sur la moyenne sur 3 visites de dépistage)
- PA diastolique de base
- Volonté et capable de manger sur place pour un repas par jour, 3 jours par semaine, et de ne manger que et de tous les aliments fournis dans le cadre des régimes à l'étude pendant les périodes d'alimentation contrôlées (périodes de rodage et quatre périodes d'alimentation de 5 semaines). Notez que la fréquence réelle des repas sur place peut être inférieure à 3 jours par semaine en raison des restrictions liées au COVID, mais les participants devront toujours être sur place pour ramasser de la nourriture et être pesés 3 jours par semaine, et seront toujours attendus d'avoir des repas surveillés (en personne ou à distance) pour un repas par jour, 3 jours par semaine.

- Avoir accès à un appareil mobile ou à un ordinateur avec des capacités de vidéoconférence, ou être disposé à utiliser un appareil de vidéoconférence fourni par l'étude
CRITÈRE D'EXCLUSION:
Exclusions de laboratoire
- Potassium sérique ≥ 5,2 mmol/L ou
- Débit de filtration glomérulaire estimé (eGFR)
- HbA1c>9,0 %
Exclusions de médicaments
Dose instable (c'est-à-dire, changement dans les 2 mois précédant le dépistage ou avant la randomisation) de l'un des éléments suivants :
- Médicaments anti-hypertenseurs
- Inhibiteurs du co-transporteur sodium-glucose 2 (SGLT2) ou agonistes des récepteurs du glucagon-like peptide-1 (GLP-1)
- Stimulants, y compris les médicaments oraux pour l'asthme ou la maladie pulmonaire obstructive chronique (MPOC)
- Hormonothérapie substitutive ou hormone thyroïdienne
- Agents psychotropes augmentant le poids, y compris les agents antipsychotiques, le lithium et la mirtazapine
Utilisation de l'un des médicaments suivants :
- Supplément de potassium, sauf s'il fait partie d'une multivitamine
- Insuline prandiale ou à courte durée d'action
- Agoniste des récepteurs GLP-1 si sur dose de perte de poids
- Warfarine (Coumadin)
- Corticostéroïde oral chronique (l'utilisation intermittente est acceptable)
- Médicaments amaigrissants
- Refus de conserver la même dose de vitamines, de minéraux et de suppléments botaniques
- Tout médicament non compatible avec la participation tel que déterminé par les enquêteurs
Exclusions des antécédents médicaux
- Diabète de type 1
- Hypoglycémie nécessitant une hospitalisation ou l'assistance d'une autre personne au cours des 12 derniers mois
- Maladie cardiovasculaire active ou tout événement survenu au cours des 6 mois précédents, y compris pontage aortocoronarien (PAC), angioplastie coronarienne transluminale percutanée (ACTP), infarctus du myocarde (IM), accident vasculaire cérébral (AVC) ou exacerbation d'insuffisance cardiaque congestive (ICC) nécessitant admission à l'hôpital
- Diagnostic ou traitement du cancer au cours des 2 dernières années (un cancer de la peau autre que le mélanome ou un cancer localisé du sein ou de la prostate ne nécessitant pas de traitement systémique est acceptable)
- Maladie intestinale inflammatoire active, résection intestinale, syndrome de malabsorption, pancréatite (épisode au cours de la dernière année) ou antécédents de chirurgie bariatrique
- Grossesse ou allaitement ou grossesse planifiée
- Toute visite au service des urgences (ED) pour asthme ou maladie pulmonaire obstructive chronique (MPOC) au cours des 6 derniers mois
- Toute autre maladie ou affection grave non compatible avec la participation telle que déterminée par les enquêteurs
Exclusions physiques
- Poids corporel> 420 livres
- Tour de bras ≥50cm
- Perte ou gain de poids > 5,0 % du poids corporel au cours des 2 mois précédents
Style de vie et autres exclusions
- Allergies, préférences, intolérances ou exigences alimentaires importantes qui interféreraient avec le respect du régime alimentaire
- Incapable d'auto-surveiller la glycémie si nécessaire
- Consommation de plus de 14 boissons alcoolisées par semaine ou consommation de plus de 6 boissons en une ou plusieurs fois par semaine
- Trouble lié à l'utilisation de substances actives qui interférerait avec la participation
- Participation ou planification pour commencer un programme de perte de poids
- Participation actuelle à un autre essai clinique

After the participant has rested quietly in the seated position for at least 5 minutes, blood pressure will be measured in triplicate, using an Omron device, with each reading separated by 30 seconds, and the average of the three readings (in mmHg) will also be recorded.
Blood pressure will be measured on five separate occasions during the last 2 weeks of the feeding period, and the mean systolic blood pressure from these five blood pressures (15 total readings) will be used as the primary outcome.
