En personas con y sin diabetes, la presión arterial (PA) elevada es la principal causa de accidente cerebrovascular y un factor de riesgo importante para otras enfermedades cardiovasculares, incluidas la enfermedad coronaria y la insuficiencia cardíaca. Las estrategias que efectivamente reducen la PA incluyen la terapia con medicamentos y la modificación del estilo de vida. Se ha demostrado que las modificaciones del estilo de vida, en particular los enfoques dietéticos, reducen la PA en personas sin diabetes. Sin embargo, hay una sorprendente escasez de pruebas sobre la reducción de la PA y las modificaciones del estilo de vida, aparte de la pérdida de peso, en personas con diabetes. El ensayo DASH4D está diseñado para proporcionar esta evidencia.
El ensayo DASH4D se basa en la experiencia de los investigadores en cuatro estudios de alimentación patrocinados por NIH (DASH, DASH-Sodium, OmniHeart y OmniCarb). El ensayo inscribirá a aproximadamente 100 adultos con diabetes tipo 2, PA sistólica de 120-159 mmHg y PA diastólica
El diseño central es un estudio de alimentación cruzado de cuatro períodos, en un solo sitio, con períodos de 5 semanas. Los participantes reciben cada una de las cuatro dietas isocalóricas, presentadas en orden aleatorio:
- Dieta DASH4D con menos sodio
- Dieta DASH4D con más sodio
- dieta de comparación con baja en sodio
- Dieta de comparación con mayor contenido de sodio.
El principal contraste de interés es la dieta DASH4D con menos sodio frente a la dieta de comparación con más sodio.
La dieta DASH4D es similar a la dieta DASH original, pero es más baja en carbohidratos y más grasa insaturada que la dieta DASH original y, por lo tanto, es más consistente con las recomendaciones dietéticas para personas con diabetes que la dieta DASH original. La dieta de comparación refleja lo que muchas personas con diabetes consumen actualmente. Se ha demostrado que la ingesta más baja de sodio de aproximadamente 1500 mg/día (a 2000 kcal) reduce la PA en personas sin diabetes y se ha recomendado en algunas pautas dietéticas. La mayor ingesta de sodio de aproximadamente 3700 mg/día (a 2000 kcal) se basa en la ingesta promedio estimada en los EE. UU.
Los resultados se miden al final de cada período de alimentación. El resultado primario es la presión arterial sistólica al final del período en el consultorio. Otros resultados son la PA diastólica, las medidas de glucemia, los factores de riesgo de lípidos plasmáticos, los síntomas del paciente y el riesgo estimado de enfermedad cardiovascular.
De manera similar a los estudios de alimentación anteriores de los investigadores, los investigadores esperan que los resultados del ensayo DASH4D se apliquen de inmediato a las pautas clínicas y de salud pública e influyan en la política de nutrición. Además, el ensayo proporcionará una plataforma rigurosa para evaluar el impacto de la dieta y la ingesta de sodio en una amplia gama de otros resultados en personas con diabetes tipo 2.

Descripción
CRITERIOS DE INCLUSIÓN:
- 18 años o más
- Diabetes mellitus tipo 2 definida por HbA1c ≥6,5 % o tratamiento de la diabetes con medicamentos para la diabetes
- PA sistólica inicial de 120-159 mmHg (basado en el promedio de 3 visitas de detección)
- PA diastólica basal
- Dispuesto y capaz de comer en el sitio una comida al día, 3 días a la semana, y comer solo y todos los alimentos provistos como parte de las dietas del estudio durante los períodos de alimentación controlados (preinclusión y cuatro períodos de alimentación de 5 semanas). Tenga en cuenta que la frecuencia real de comidas en el lugar puede ser inferior a 3 días a la semana debido a restricciones relacionadas con COVID, pero los participantes aún deberán estar en el lugar para recoger alimentos y pesarse 3 días a la semana, y aún se esperará monitorear las comidas (en persona o de forma remota) para una comida por día, 3 días a la semana.

CRITERIO DE EXCLUSIÓN:
Exclusiones de laboratorio
- Potasio sérico ≥5,2 mmol/L o
- Tasa de filtración glomerular estimada (eGFR)
- HbA1c>9,0%
Exclusiones de medicamentos
Dosis inestable (es decir, cambio en los 2 meses anteriores a la selección o antes de la aleatorización) de cualquiera de los siguientes:
- Medicamentos antihipertensivos
- Inhibidores del cotransportador de sodio-glucosa 2 (SGLT2) o agonistas del receptor del péptido 1 similar al glucagón (GLP-1)
- Estimulantes, incluidos medicamentos orales para el asma o la enfermedad pulmonar obstructiva crónica (EPOC)
- Terapia de reemplazo hormonal o hormona tiroidea
- Agentes psicotrópicos que aumentan el peso, incluidos agentes antipsicóticos, litio y mirtazapina

- Suplemento de potasio, excepto si es parte de un multivitamínico
- Insulina prandial o de acción corta
- Agonista del receptor GLP-1 si está en dosis de pérdida de peso
- Warfarina (Coumadin)
- Corticosteroide oral crónico (el uso intermitente está bien)
- Medicamentos para bajar de peso
- Falta de voluntad para mantener la misma dosis de suplementos vitamínicos, minerales y botánicos.

Exclusiones de historial médico
- Diabetes tipo 1
- Hipoglucemia que requirió hospitalización o la asistencia de otra persona en los últimos 12 meses

After the participant has rested quietly in the seated position for at least 5 minutes, blood pressure will be measured in triplicate, using an Omron device, with each reading separated by 30 seconds, and the average of the three readings (in mmHg) will also be recorded.
Blood pressure will be measured on five separate occasions during the last 2 weeks of the feeding period, and the mean systolic blood pressure from these five blood pressures (15 total readings) will be used as the primary outcome.
